Percutaneous (Periportal) Hip Arthroscopy.

Capsulotomy in hip arthroscopy is essential for optimal visualization and instrument maneuverability. Commonly used capsulotomy techniques, such as interportal and T-type capsulotomy, result in damage to the iliofemoral ligament, a major stabilizer of the hip joint. The periportal technique, a relatively novel capsulotomy technique, involves the creation of 2 to 3 relatively small portals, thereby preserving the capsuloligamentous complex while allowing for the passage of an arthroscope and instruments into the joint space. Emerging evidence suggests that the periportal approach leads to impro

Editorial Commentary: Are We Checking Our Bias in Patients With Fibromyalgia That Need Hip Arthroscopy?

Like all diagnoses and comorbidities, patients with hip pain should be managed with individualized treatment plans. Patients with fibromyalgia undergoing hip arthroscopy are no different. Even if their absolute scores remain lower than those without fibromyalgia, these patients can achieve clinically meaningful improvements in patient-reported outcomes after treatment of femoroacetabular impingement. As orthopaedic providers, we should be aware of potential bias against patients with chronic musculoskeletal pain and critically evaluate the literature that supports clinically relevant improveme

Editorial Commentary: Femoral Version and Capsular Thickness Analysis in Hip Preservation Surgery-Surgical Indications Are as Important as Surgical Technique.

Optimal treatment of patients with femoroacetabular impingement syndrome requires both thoughtful decision-making and skillful operative technique. Traditional evidence-based literature and routine clinical practice overemphasize the role of the alpha angle on the femoral side and lateral center edge angle on the acetabular side. Femoral and acetabular version are critical values that influence outcomes and warrant measurement and consideration. Without version analysis, an uniformed, possibly poor, decision may be made. The literature and clinical practice also place dichotomous emphasis on l

Femoroacetabular Impingement in Ice Hockey Athletes.

Femoroacetabular impingement (FAI) in ice hockey is a concern for many athletes. The biomechanics of skating and the injury mechanism, prevalence, identification, and treatment protocols currently available for FAI in ice hockey athletes are important for all coaches and practitioners to understand. This article discusses the underlying anatomical issues and biomechanical considerations surrounding FAI. Furthermore, this article describes the interventions that can be used when encountering FAI and well-established protocols to aid in the return to play. Finally, prevention strategies that can

Editorial Commentary: It's "90% Mental": Mental Distress Influences Symptom Perception in Hip Arthroscopy.

There is a well-established relationship between mental distress, mental health disorders, and the perception of pain and physical dysfunction. While determining the root cause is often challenging, increasing levels of mental distress are associated with increasingly disruptive manifestation of physical symptoms and vice versa. In femoroacetabular impingement syndrome, patients with preoperative mental distress exhibit more impactful levels of symptoms. Patients do show symptomatic improvement with appropriate surgical management, even in the presence of mental distress. Patients and surgeons

Comparable Torque to Failure Using the Simple Stich Versus the Figure-of-Eight Configuration for Hip Capsular Closure Following an Interportal Capsulotomy: A Cadaveric Study.

PURPOSE: To measure and compare the torque to failure and stiffness of the capsular repair construct consisting of four-suture simple stitches to a two-figure of eight stitches repair construct in external rotation following an interportal capsulotomy. METHODS: Six pairs of fresh-frozen cadaveric hemipelves were divided into two capsular repair groups. All hips underwent a 40-mm interportal capsulotomy from the 12 o'clock position to the 3 o'clock position. Capsular closure was performed using either the two stitches in a figure of eight or with four simple stitches. Afterward, each hemipelvi

Imaging evaluation of the hip after arthroscopic surgery for femoroacetabular impingement.

Arthroscopic surgery for femoroacetabular impingement (FAI) is increasingly frequently performed. Initial reports were that complications were very low, but as experience has increased, a number of long-term complications, in addition to factors related to poor clinical outcomes, have been identified. This review describes the normal and abnormal postoperative imaging appearance of the hip after arthroscopy for FAI. Abnormalities discussed include incomplete resection or over-resection of the impingement lesion, heterotopic ossification, cartilage damage, chondrolysis, instability and dislocat

Hip labral cyst caused by psoas impingement.

Hip labral impingement can cause labral tears and secondary paralabral cyst formation. Femoroacetabular impingement is the main cause of labral impingement, but other conditions such as iliopsoas tendon impingement are described. There is no description of labral cyst resulting from psoas impingement treated arthroscopically in the literature. We present the case of a young sportsman with groin pain caused by psoas impingement with a labral tear and secondary paralabral cyst who was treated arthroscopically by cyst debridement, psoas tenotomy, and labral repair.

Protrusio acetabuli: contraindication or indication for hip arthroscopy? And the case for arthroscopic treatment of global pincer impingement.

Protrusio acetabuli has been considered a contraindication for hip arthroscopy. We present the case of a 33-year-old man with bilateral symptomatic protrusio acetabuli-the most extreme form of global pincer femoroacetabular impingement-and cam femoroacetabular impingement. We demonstrate the feasibility of the arthroscopic correction of severe deformities and describe key surgical steps permitting central compartment access, subtotal acetabuloplasty, labral reconstruction, and femoroplasty of the right hip, followed by later subtotal acetabuloplasty, labral refixation, and femoroplasty of the

Editorial Commentary: The First Operation Determines the Ceiling: Revision Hip Arthroscopy in Adolescents Offers Gains but Not Full Restoration.

Revision hip arthroscopy in adolescents has become increasingly common as more young athletes undergo primary surgery for femoroacetabular impingement and labral pathology. In my experience, revision procedures can provide meaningful pain relief and functional gains, but the hip rarely behaves like a "primary" hip once again. Clinical data consistently show lower absolute outcome scores, lower clinical benefit rates, and a higher risk of rerevision after adolescent revision compared with primary arthroscopy. For me, the central message is simple: the first operation matters profoundly. Precise

Hip Labral Reconstruction With a Synthetic Graft: Development and Preclinical Validation.

OBJECTIVES: Hip Osteoarthritis (OA) affects a significant component of the adult population, placing itself among the most important causes of disability and need for total hip replacement. Femoroacetabular impingement (FAI) is an anatomic alteration of the proximal femur and/or acetabulum leading to chondro-labral damage and playing a prominent role in OA pathogenesis. Thus, treating FAI is fundamental to relieve hip pain and further joint tissue deterioration. Labral reconstruction is considered the treatment of choice, in particular using tendon allografts or autografts which, however, have

Editorial Commentary: Hip Arthroscopic Capsular Management With Interportal Capsulotomy Versus T-Type Ultimately Depends on Surgeon Preference and Distal Visualization Necessity.

Capsular management is an important technical component during hip arthroscopy to effectively treat femoroacetabular impingement syndrome. Capsulotomy both provides visualization and assists with instrument and implant placement. Traditional capsulotomy methods include both interportal capsulotomy (IP) and T-type. Studies have, however, shown unrepaired capsulotomies or capsular healing defects can be a source of hip pain and instability. The biomechanical and clinical literature support both capsulotomy types if adequate repair is performed, with very few differences in outcomes between the 2

Analysis of femoroacetabular impingement by a triade of label-free optical spectroscopy techniques.

Understanding the biochemical mechanisms of hip joint diseases remains challenging. Femoroacetabular impingement (FAI) is a condition where developmental deformities of the hip joint reduce mobility and cause tissue damage, with characteristic red staining observed in affected cartilage. Here we show how combining three complementary optical analysis techniques- laser-induced breakdown spectroscopy, Raman spectroscopy, and diffuse reflectance spectroscopy- reveals tissue alterations without invasive procedures. Our analysis identifies elevated amide I levels indicating bone damage, reduced hyd

Concomitant sacroiliac joint abnormalities in patients with femoroacetabular impingement.

PURPOSE: Despite the increasing understanding of femoroacetabular impingement (FAI), the impact of specific patient characteristics, including inflammatory pathologies like axial spondyloarthritis (axSpA), on its pathophysiology and clinical outcomes following treatment remains inadequately defined. Therefore, the purpose of this scoping review was to evaluate the relationship between FAI and sacroiliac (SI) joint abnormalities and FAI and axSpA. METHODS: The study was conducted following the framework established by Arksey and O'Malley and Levac et al., adhering to the PRISMA scoping review

Hip Pain and Movement Dysfunction Associated With Nonarthritic Hip Joint Pain - 2023 Revision Using the Evidence to Guide Musculoskeletal Rehabilitation Practice.

Nonarthritic hip joint pain is a collection of conditions involving intra-articular structures of the hip, including femoroacetabular impingement syndrome (FAIS), developmental dysplasia of the hip (DDH), hip instability, acetabular labral tears, osteochondral lesions, loose bodies, and ligamentum teres tears. Pain in the hip region can also involve non-musculoskeletal, lumbosacral spine, intra-articular, and extra-articular sources. Treatment for people with nonarthritic hip joint pain should focus on impairments and be evidence based, with assessments at baseline and at least one other follo

Editorial Commentary: Degeneration of Acetabular Cartilage of any Degree Is a Predictor of Worse Outcome After Hip Arthroscopy for Labral Repair and Femoroacetabular Impingement Syndrome: The Greater the Damage, the Worse the Outcome.

As the volume of hip arthroscopy continues to rise, determining appropriate indications for the procedure is of paramount importance. Outcomes can be predicted based on well-powered studies focused on determining which patients meet thresholds for achieving the minimal clinically important difference, the patient acceptable symptomatic state, and substantial clinical benefit after treatment. Recent research has shown that acetabular cartilage damage predicts lesser outcomes, and the more advanced the damage, the worse the results. What is concerning is that even the smallest degree of chondrom

Editorial Commentary: Combined Hip Version Abnormalities May Not Negatively Influence Patient Outcomes, Depending How Version Is Measured and Combined Version Is Categorized.

Hip arthroscopy for femoroacetabular impingement syndrome (FAIS) continues to expand in its indications while showing excellent clinical outcomes. However, reported outcomes in the setting of version abnormalities vary widely across the literature. Historically, femoral and acetabular retroversion have been associated with inferior results, yet more recent studies suggest that a broader range of version morphologies may be successfully treated arthroscopically. A key limitation in the literature is the lack of standardization in measuring and defining version. Multiple techniques exist for ass
