The definition of atypical femoral fractures should include periprosthetic femoral fractures (PAFFs).

UNLABELLED: Periprosthetic hip fractures may have features of atypical femoral fractures. PURPOSE: Atypical femoral fracture (AFF) is a rare complication of treatment with bisphosphonates (BPs) or denosumab. The American Society for Bone and Mineral Research (ASBMR) Task Force definition for AFFs excludes periprosthetic fractures. The purpose of this study was to determine prodromal symptoms, frequency, treatment, and outcomes of periprosthetic AFFs (PAFFs) in persons prescribed a BP or denosumab for osteoporosis and later diagnosed with a periprosthetic hip fracture. METHODS: Participants w

Distal Femoral Physeal Fractures.

Physeal fractures of the distal femoral are rare injuries accounting for less than 2% of all physeal injuries, and tend to have a worse prognosis than similar injuries in other locations. This article reviews the evaluation (including imaging), classification, and treatment of these injuries, and discusses their most important complications and their management, including vascular injury and growth arrest.

[Femoral neck stress fractures].

Femoral neck stress fractures are relatively rare and caused by repetitive high pressure on the bone with insufficient time to recover. These fractures are often seen in fanatic runners or military personnel, who cover great distances. Patients with a femoral neck stress fracture present with mild pain at the front of the thigh or groin. Radiological imaging includes a plain X-ray and/or MRI-scan. Differentiation between a tension- and compression type stress fracture is important, since their treatment strategies differ. Generally, tension type fractures are unstable and demand operative fixa

Femoral nerve block in extracapsular femoral neck fractures.

We randomised 50 patients with extracapsular fractures of the femoral neck to receive either a bupivacaine femoral nerve block or systemic analgesia alone. A femoral nerve block was found to be an easy and effective procedure which significantly reduced perioperative analgesic requirements and postoperative morbidity.

Femoral shaft fractures associated with unstable pelvic fractures.

From December 1986 to December 1989, 23 consecutive adult patients with femoral shaft fractures associated with unstable pelvic fractures were prospectively studied. The mortality rate was 8.7%. An anterior frame of an external fixator was sometimes inserted emergently in patients with multiple injuries to improve their bedside care. Early reamed femoral intramedullary nailing accompanying closed or open reduction with internal fixation, external fixation or both for unstable pelvic fractures was the favored treatment method. Twenty cases were followed for an average of 26 months. The union ra

Alendronate-associated femoral insufficiency fractures and femoral stress reactions.

PURPOSE: To report characteristics and outcomes of 8 patients who underwent intramedullary nailing for alendronate-associated femoral insufficiency fractures. METHODS: Records of 7 women and one man aged 54 to 91 (mean, 72) years who underwent intramedullary nailing for spontaneous or low-energy, alendronateassociated femoral insufficiency fractures were retrospectively reviewed. Contralateral femurs were assessed for stress reactions. RESULTS: All patients had at least 4 (range, 4-10) years of alendronate use. Five patients had experienced prodromal pain lasting one day to 5 months. Five pa

Update on Atypical Femoral Fractures.

➤ Atypical femoral fractures (AFFs) are stress fractures between the lesser trochanter and the metaphyseal flare that are most commonly related to prolonged (3 to 5 years) antiresorptive medication use.➤ An important finding is a visible transverse line in the lateral cortex, known as the "dreaded black line." Complete fractures are transverse and have minimal comminution.➤ Prodromal symptoms including hip, groin, thigh, and knee pain are present in more than one-half of cases and are usually misdiagnosed.➤ Nonoperative treatment for all patients with AFF includes withdrawal of bisphosphonates

Femoral neck fractures: current management.

Femoral neck fractures are a commonly encountered injury in orthopaedic practice and result in significant morbidity and mortality. It is essential that surgeons are able to recognize specific fracture patterns and patient characteristics that indicate the use of particular implants and methods to effectively manage these injuries. Use of the Garden and Pauwels classification systems has remained the practical mainstay of femoral neck fracture characterization that help dictate appropriate treatment. Operative options include in situ fixation, closed or open reduction and internal fixation, he

Fractures of the femoral neck.

Fractures of the femoral neck are increasing at an exponential rate as a result of the longevity of the general population. The Garden and Pauwels classifications both are routinely used to describe displacement and stability of femoral neck fractures. Osteonecrosis and nonunion remain problematic because of the compromised blood supply to the femoral head in displaced fractures. Nondisplaced fractures and displaced fractures in patients physiologically younger than 65 years are treated with closed or open reduction and internal fixation. Anatomic reduction is the single most important step in

[Treatment of femoral neck fractures].

Due to the high incidence (600-900 patients/year >65 years old), the expected increase in frequency by a factor of five by 2050, and the proportionately shrinking capacity in trauma centers, femoral neck fractures are relevant to health care both economically and politically. Surgical treatment within 6 h improves results of osteosynthesis, within 24 h reduces general complications, and within 48 h reduces mortality. The literature displays great regional differences in methods and results. There is however general agreement that the hip joint should be preserved in young, active patients, reg

Management of periprosthetic femoral fractures.

We followed up 37 periprosthetic fractures to assess the factors related to the outcomes and the loosening of prostheses. The average union time is 3.1 months (range, 2-6.2 months). The most common types were fractures around the stem tip and displaced (89.1%). In our study, the loosening rate (32.4%) was low compared with other reports. These might be due to the cementless stem, but the difference was not significant (P>.05). After management of the fracture with a plate, we noted 5 complications: 2 nonunion and 3 refractures. The causes of these complications were related to screw in the lev

[Femoral neck fractures in childhood].

In a retrospective multicenter study we followed-up 30 fractures of the neck of the femur in children aged 11 years (1.5-15 years) for 4.8 years (0.5-20.0 years). It is the aim of this study to analyse factors possibly related to outcome, like age at injury, type of fracture, interval between injury and treatment, method of stabilization and postoperative interval until full weight bearing. At follow-up subjective parameters (pain, weather sensitiveness), clinical parameters (range of motion, claudication, leg length discrepancies) and radiological parameters (hip series) were investigated and

Distal femoral physeal problem fractures.

Treatment options in the past for distal femoral physeal fractures have varied from closed reduction to open reduction with internal fixation to balanced skeletal traction. In this study, ten patients with distal femoral physeal fractures treated with closed reduction and casting or skeletal traction are reviewed. Seven fractures lost position in comparison with original reduction films. Nine patients developed subsequent deformity. No consensus exists regarding the use of open versus closed treatment with internal fixation. This review of closed treatment yielded a high rate of unacceptable r

Femoral fractures in Paget's disease.

Since the original description of osteitis deformans, the treatment of femoral fractures occurring in this disease process has been difficult. A retrospective study of 35 fractures treated over an 8-year period was undertaken to identify the factors that influenced the results. The fractures were classified according to four fracture patterns: subcapital, intertrochanteric, subtrochanteric, and shaft. Chart and radiographic analyses were then performed. The results identified subtrochanteric fractures, occurring in a somewhat younger population, to be the problem fracture. The other three frac

Proximal femoral locking compression plate for proximal femoral fractures.

PURPOSE: To review short-term outcomes of proximal femoral locking compression plate (PF-LCP) fixation for proximal femoral fractures in terms of postoperative complications and failure rates. METHODS: Medical records of 21 men and 5 women aged 22 to 85 (mean, 49.7) years who underwent internal fixation with the PF-LCP for proximal femoral fractures were reviewed. Younger patients (mean age, 38.7 years) were more commonly involved in high-energy trauma with multiple musculoskeletal injuries, whereas older patients (mean age, 67.7 years) were more commonly involved in low-energy trauma. Distal femoral fractures in children.

The physis of the distal femur contributes to 70% of femoral growth and 37% of the total limb growth; therefore, physeal injury can lead to important alterations of axes and length. Distal metaphyseal corner-type fracture prior to walking is classically associated with child abuse. In children aged >10 years, sports-related fractures and car accidents are significant contributors. Imaging includes a two-plane radiographic study of the knee. It is recommended to obtain radiographs that include the entire femur to rule out concomitant injuries. In cases of high suspicion of distal metaphyseal fr

Management of femoral shaft fractures.

Femoral shaft fractures are severe injuries and are often associated with a high impact trauma mechanism, frequently seen in multiple injured patients. In contrast an indirect trauma mechanism can lead to a complex femoral shaft fracture especially in elderly patients with minor bone stock quality. Hence management of femoral shaft fractures is often directed by co-morbidities, additional injuries and the medical condition of the patient. Timing of fracture stabilization is depended on the overall medical condition of the patient, but definite fracture fixation can often be implemented in the

Bisphosphonate-related subtrochanteric femoral fractures.

BACKGROUND: Hip fractures are important because they are associated with an increased rate of death, substantial morbidity, enormous costs, and devastating effects on independence and quality of life. Bisphosphonates have proven efficacy in reducing the incidence of spine and nonspine fractures. Several case series and multiple individual case reports from around the world suggest that some subtrochanteric fractures and those of the femoral shaft might occur in patients who have been treated with long-term bisphosphonate therapy. CASE SUMMARY: We present a case in the United Kingdom, where a
