Der Chirurg; Zeitschrift fur alle Gebiete der operativen Medizen2001ger
UNLABELLED: Alloarthroplasty of the hip joint in the elderly has improved significantly the outcome of femoral neck fractures. The indication for a hip replacement is based on the biological age of the patient and the type of fracture. In our hospital in a 5-years period (1996-2000) we treated operatively 305 patients with femoral neck fractures (248 medial, 57 lateral). Of these, 239 patients received a endoprosthesis (190 total hip joint replacements, 49 hemialloarthroplasty). The other 66 patients (most of them younger patients) were treated by osteosynthesis. The complication rate of the e

Ipsilateral, concomitant femoral neck and shaft fracture is a rare fracture pattern encountered by orthopaedic surgeons. We review 11 cases of this fracture pattern. Treatment was by reduction and stabilization of the femoral neck fracture with one or more cannulated screws, followed by fixation of the femoral shaft using a Russell-Taylor reconstruction nail or a Russell-Taylor Delta reconstruction nail. When properly sequenced, this has proven to be a successful treatment method in these dual fractures, resulting in a low complication rate. No avascular necrosis of the femoral head or nonunio

A total of 110 fractures of the neck of femur were studied in 108 patients 17 to 50 years old. The aims were to analyze why these fractures occur in young adults, and what results can be expected after internal fixation. Severe trauma caused about half of the fractures. In the remaining group chronic and disabling disorders were usually present at the time of injury. Seventeen patients were known to abuse alcohol. Aseptic necrosis with late segmental collapse occurred in 41 per cent of the hips (45/110), comparatively more often in patients who were 45 years or younger. Only 16 of the 73 patie

Thirty-eight displaced intracapsular fractures of the femoral neck treated by the Deyerle method of internal fixation have been added to the previously reported series of sixty-three cases from the Walter Reed Army Medical Center Orthopaedic Service. A significant change in the incidence of non-union was found. In the over-all series of 101 patients, the incidence of non-union was 13.8 per cent and the incidence of segmental collapse due to avascular necrosis was 13.8 per cent. The roentgenographic and functional over-all end results were rated excellent in 67.7 per cent and poor in 32.3 per c

AIM OF THE STUDY: Treatment of intracapsular femoral neck fractures, especially in elder patients, has been a topic of ongoing discussion. This study analyzes the results of 97 osteosynthetically-treated femoral neck fractures. It had to be proved whether the Dynamic Martin Screw (DMS) represents an alternative to the already existing implantation devices. Until now no systematic results have been reported on this topic.
METHODS: Over a period of seven years, 389 patients with femoral neck fractures were treated. In 101 of these patients operations with preserved femoral heads were carried ou

Femoral neck fractures are important causes of morbidity and mortality. Patients with end-stage renal diseases are 4,4-fold more likely to sustain a hip fracture that the general population. We present our own experience with treatment of femoral neck fractures in hemodialysis patients. The study included 12 patients (6 females and 6 males), mean age 51 years (range 41-77). They were dialysed for a mean duration of 125 months (range 12-271). The femoral neck fracture was treated by bipolar hip arthroplasty. The follow up was from 3 month to 6 years. We did not observe serious complications aft

Ipsilateral femoral neck and shaft fractures are uncommon injuries that present a surgical challenge. Patients are relatively young, are usually victims of high-energy trauma, and have frequently sustained multisystem injuries. A comminuted midshaft femoral fracture secondary to axial loading should alert the treating physician to the possibility of an associated femoral neck fracture. This is important in light of the frequency of unrecognized ipsilateral femoral neck fractures. Several treatment options are described in the literature, but no clear consensus exists regarding the optimal trea

Ipsilateral femoral neck and shaft fractures occur in 2.5% to 6% of femur fractures. The injury results from high energy trauma. Victims are usually young, with multiple associated injuries. The diagnosis of the neck fracture is delayed in 19% to 31% of patients. The neck fracture line is almost vertical and nondisplaced, or minimally displaced in 26% to 59% of cases. The shaft fracture is often midshaft and open, and/or comminuted in 47% to 67% of cases. Ipsilateral knee injuries occur in 20% to 40% of patients. Two major complications, osteonecrosis of the femoral head and nonunion of the ne

By clinical, radiographic and scintigraphic means we attempted to examine, whether the implantation of a simple head-endoprosthesis after fractures of the femoral neck still has to be considered as an appropriate operating method or whether on principle, primary endoprosthesis should be used. The results prove that in suitable cases and with correct technique the head-endoprosthesis still has justification, especially as the "protrusio acetabuli", known only as complication of pure head-endoprosthesis, is mostly avoidable. Reference to the choice of cases, operating conditions and appropriate

In this study we wanted to demonstrate the relationship between the fracture of the femoral neck and femoral head necrosis. For this purpose we reviewed the new literature, the dates of the ASIF documentation and some cases from the Davos Hospital. Head necrosis appears in all types of fractures and is independent of the implant. We can find femoral head necrosis in all age groups but the risk is higher the younger the patient. The necrosis can develop many years after the accident. There are not enough long-term studies to explain this phenomenon.

Hemiarthroplasty is a common treatment for femoral neck fractures in the elderly population. The main complications are periprosthetic dislocation and infection, which potentially impact morbidity and quality of life and may contribute to mortality. This procedure can be technically demanding, and adequate closure of the capsule and soft tissue cannot be emphasized enough. One advantage of a bipolar prosthesis is that it can be easily converted to a total hip arthroplasty without replacing the femoral component and with approximately the same complication rates as a revision total hip arthropl

The femoral neck fractures in children are very rare. It is difficult to elaborate a standard of treatment of this disorder in children, because of a little amount of publications on this subject. The common complications which occur are physis premature closure, coxa vara, avascular necrosis of the head and neck of femur and ocasionally nonunion. The paper analyses 18 cases of fracture in children at the age of 4 to 16 years old (average 8 years old) who were treated at the clinic since 1990 to 2003. Cohort of patients included 7 boys and 11 girls. Fractures were classified according to Delbe

Fifty-four femoral neck fractures in young adults (20-50 years old; Group I) were compared with 397 fractures in the elderly population (older than 60 years of age; Group III). Patients in Group I were more frequently men and had often sustained vertical shear fractures in high-energy injuries. Treatment consisted mostly of open reduction and internal fixation (88%). There were no infections, pulmonary embolisms, or deaths. The rate of avascular necrosis was 10%; the rate of delayed union or nonunion, 17%; and the revision rate, 20%. Reoperation did not preclude an excellent final outcome. Pat

A fracture of the shaft of the femur in adults is common after road traffic accidents. Until recently it was not known that a fracture of the femoral neck can occur while fixing the femoral shaft fracture with an intramedullary nail. We report three patients in whom femoral neck fractures occurred during femoral nailing. Fractures of the femoral neck were detected by routine postoperative radiographs of the hip joint. All the three patients were treated non-operatively and the fractures healed without complication. The cause of the femoral neck fracture seems to be forceful use of an awl in th

Fractures of the femoral neck in childhood and adolescence are a rare accident injury. Despite their rarity they are significant for the individual child because of the probability of complications, reported in up to 60% of the cases in the literature. They are caused on the one hand by direct damage to the growth joint, which according to recent studies is regarded as a single, continuous, transverse growth plate which includes the trochanteric joint; and on the other by damage resulting from a circulation disorder. Most authors advocate emergency surgical treatment to relieve the hemarthrosi

INTRODUCTION: Femoral neck fractures in the elderly are a major event and are rising in incidence over the last decade. Advancing age and numerous comorbidities largely account for high mortality rate and require geriatric expertise. Treatment options are total hip arthroplasty (THA), hemiarthroplasty (HA) or osteosynthesis. Literature suggests THA or HA for better outcomes, although no clear guidelines exist.
MATERIAL AND METHODS: A retrospective chart review was performed of 63 patients (80 ± 11 years; 32 women, 31 men) with Garden one femoral neck fractures treated between June 2018 and Ju

OBJECTIVE: The research was conducted to assess the therapeutic efficacy of the femoral neck system (FNS) and cannulated compression screw (CCS) in treating femoral neck fractures classified as Pauwels type II and III in young to middle-aged patients.
METHODS: This retrospective cohort study included 46 patients treated at Yanbian University Affiliated Hospital between January 2021 and March 2023. Patients were allocated into two groups: FNS (n = 24) and CCS (n = 22). Clinical indicators, including bone healing duration, Visual Analog Scale (VAS), Harris Hip Score (HHS), and postoperative com
