Perinatal and maternal outcomes of fetal macrosomia.

OBJECTIVE: To determine the perinatal and maternal outcome of the macrosomic infants. STUDY DESIGN: A case-control, retrospective study is performed in the Department of Gynecology and Obstetrics, Istanbul University Cerrahpasa Medical Faculty, between 1988-1992. The maternal and neonatal records of infants with birthweight of at least 4000g (n=1000) were reviewed. Another 1000 cases amongst the newborns delivered in the same period between 2500 and 3999g formed the control group. The obstetrical outcome variables of the groups including mode of delivery and the incidence of maternal and peri

Estimating fetal weight in the management of macrosomia.

The purpose of this review is to examine the evidence that, including estimates of fetal macrosomia in patient care, will decrease adverse perinatal outcomes. A literature search for the years 1980 to 1999 was used. Shoulder dystocia and brachial plexus injuries occur more often in macrosomic than in non-macrosomic neonates. However, 26 to 58 percent of shoulder dystocias and 24 to 44 percent of brachial plexus injuries occur to babies weighing less than 4000 gm. Persistence of impairment is extremely rare. Neither historical nor clinical factors have strong positive predictive values for macr

[The role of echography in the management of fetal macrosomia].

Macrosomia remains an important cause of perinatal morbidity and mortality and maternal morbidity, which arise mainly from birth injury and asphyxia and increased rate or cesarean section respectively. In spite of many attempts to identify the patients at risk for fetal macrosomia, the perfect prediction has not yet emerged. In the last years, many papers have reported methods for ultrasonographic determination of fetal weight. It should be emphasized that most of these reports have cautioned that sonographic predictive accuracy declines as fetal weight increases toward 4000 g. In our study, s

Gestational diabetes and fetal macrosomia: a dissenting opinion.

OBJECTIVES: To examine the effect of interaction between parity, overweight/obesity, gestational weight gain, and gestational diabetes mellitus (GDM) on the incidence of fetal macrosomia. METHODS: We used a population-based dataset to establish the incidence of macrosomia (birth weight >4,000 g) in singleton births at ≥38 weeks' gestation. The cohort included women who were (1) overweight/obese or had normal body mass index (BMI) before pregnancy, (2) nulliparous or multiparous, (3) with appropriate or excessive weight gain, and (4) without GDM, with GDM controlled by nonpharmacological treat

Recurrence of fetal macrosomia in non-diabetic pregnancies.

Fetal macrosomia (birth weight >/=4,500 g) is known to increase a number of adverse maternal and perinatal outcomes. Although there is a clear association between maternal diabetes mellitus and fetal macrosomia, the majority of macrosomic infants are born to non-diabetic mothers. We wished to determine the recurrence rate of macrosomia in non-diabetic pregnancy and to see if a history of multiple prior macrosomic infants confers additional risk. A retrospective analysis of 14,461 term, singleton, infants born to non-diabetic mothers in 1997 and 1998 was performed, using a computerised hospital

An Evidence-Based Approach to Defining Fetal Macrosomia.

OBJECTIVE: This study aims to determine the risk of adverse outcomes associated with the current diagnostic criteria for fetal macrosomia. Study DESIGN: We evaluated three techniques for characterizing birth weight as a predictor of shoulder dystocia or third- or fourth-degree laceration in 79,879 vaginal deliveries. First, we compared deliveries with birth weights above or below 4,500 g. We then performed logistic regression using birth weight as a continuous predictor, both with and without fractional polynomial transformation. Finally, we calculated the number of cesarean sections required

Nondiabetic Fetal Macrosomia: Causes, Outcomes, and Clinical Management.

IMPORTANCE: Fetal macrosomia (FM) is common in clinical practice and carries increased risk of adverse maternal and neonatal health outcomes. Maternal diabetes mellitus (DM) is a well-known cause of macrosomia with significant research and guidelines focusing on macrosomia in this population. Less is known about causes, prevention, and clinical management for suspected FM in individuals without diabetes. OBJECTIVE: The objective of this review is to describe the risk factors associated with nondiabetic FM, review risks associated with macrosomia in pregnancy, and potential treatment considera

Early Prediction of Fetal Macrosomia Through Maternal Lipid Profiles.

The prevalence of fetal macrosomia is steadily increasing worldwide, reaching up to 20%. Fetal macrosomia complicates pregnancy and delivery. Current prediction strategies are inaccurate, and most patients with fetal macrosomia go into labor with an "unknown status". The aim of this study was to develop a system for predicting fetal macrosomia based on the lipid profiles of pregnant women's blood serum. In total, 110 patients were included in this study: 30 patients had gestational diabetes mellitus (GDM) and 80 did not. During the observation, blood samples were collected at three time points

Early Pregnancy Waist Circumference for Prediction of Fetal Macrosomia.

Fetal macrosomia is associated with adverse short- and long-term outcomes for the mother and the child. Present models to predict fetal macrosomia cannot be used in all settings, and their precision could be improved. We assessed if waist circumference could replace or outperform weight for early pregnancy prediction of macrosomia. We included 5827 women in this population-based cohort study and assessed the influence of early pregnancy waist circumference and weight on the prediction of macrosomia with logistic regression analysis. We generated receiver operating characteristic (ROC) curves a

Maternal and neonatal complications of fetal macrosomia: cohort study.

OBJECTIVE: To estimate the risks of maternal and neonatal complications in pregnancies with macrosomia. METHODS: This was a retrospective cohort study conducted at a large maternity unit in the UK between January 2009 and December 2016. The incidence of maternal and neonatal complications in pregnancies with macrosomia, defined as birth weight (BW) > 4000 g, and in those with severe macrosomia, defined as BW > 4500 g, was compared with that in pregnancies with normal BW (2500-4000 g). Regression analysis was performed to determine odds ratios (ORs) for complications in macrosomic pregnancies

Ultrasonographic prediction of fetal macrosomia. Association with cesarean delivery.

OBJECTIVE: To investigate whether the incorrect ultrasonographic prediction of macrosomia affects the cesarean delivery rate among nonmacrosomic neonates. STUDY DESIGN: For this retrospective, cohort study, comprehensive ultrasonographic records were reviewed at two centers. Patients with singleton, nonanomalous gestations whose ultrasonography predicted an estimated fetal weight > or = 4,000 g composed one cohort (n = 135), while the other cohort (n = 129) consisted of patients whose ultrasonography predicted an estimated fetal weight between 3,000 and 3,999 g. Induction of labour at or near term for suspected fetal macrosomia.

BACKGROUND: Women with a suspected large-for-dates fetus or a fetus with suspected macrosomia (birthweight greater than 4000 g) are at risk of operative birth or caesarean section. The baby is also at increased risk of shoulder dystocia and trauma, in particular fractures and brachial plexus injury. Induction of labour may reduce these risks by decreasing the birthweight, but may also lead to longer labours and an increased risk of caesarean section. OBJECTIVES: To assess the effects of a policy of labour induction at or shortly before term (37 to 40 weeks) for suspected fetal macrosomia on t

Active management of suspected fetal macrosomia: the right approach?

OBJECTIVE: First, to analyze the impact on induction of labor, cesarean section and shoulder dystocia (ShD) rates after the application of an active management protocol in suspected isolated fetal macrosomia. Second, to determine the diagnostic yield of the third trimester scan for the prediction of neonatal macrosomia. Third, to compare baseline characteristics and perinatal outcomes in women with neonates ≥4000 g with and without prenatal suspicion of macrosomia. METHODS: A retrospective cohort study was conducted between July 2021 and July 2022 in a tertiary center. An active management pr

Screening circulating proteins to identify biomarkers of fetal macrosomia.

OBJECTIVE: Fetal macrosomia is a major risk factor for shoulder dystocia, which can lead to birth asphyxia, maternal and neonatal traumatic injuries, and perinatal death. If macrosomia is diagnosed in the antenatal period, labour can be induced to decrease shoulder dystocia. But current clinical methods to diagnose fetal macrosomia antenatally perform with poor accuracy. Therefore, improved methods to accurately diagnose fetal macrosomia are required. Blood biomarkers that predict fetal macrosomia could be one such novel diagnostic strategy. We undertook a nested case-control study from a pros

Treatment of suspected fetal macrosomia: a cost-effectiveness analysis.

OBJECTIVE: Treatment of fetal macrosomia presents challenges to practitioners because a potential outcome of shoulder dystocia with permanent brachial plexus injury is costly both to families and to society. Practitioner options include labor induction, elective cesarean delivery, or expectant treatment. We performed a cost-effective analysis to evaluate the treatment strategies that were preferred to prevent the most permanent brachial plexus injuries with the least amount of dollars spent. STUDY DESIGN: Using decision analysis techniques, we compared 3 strategies for an infant with an estim

Fetal macrosomia. ACOG Technical Bulletin Number 159--September 1991.

When macrosomia exists, shoulder dystocia is a primary obstetric concern. Current methods for estimating birth weight prior to delivery are imprecise, and macrosomia often cannot be predicted. Certain conditions or combinations of conditions should increase the index of suspicion for shoulder dystocia. When these conditions are identified, consideration should be given to the management of potential shoulder dystocia. Despite due care and appropriate clinical judgment, however, injury during the birth process cannot always be prevented.

A retrospective identification of risk factors associated with fetal macrosomia.

Despite extensive work on macrosomia, it is impossible to predict women at risk. Current prediction strategies which include clinical examination and ultrasound are imprecise. This study aims to determine the risk factors associated with macrosomia. It was a descriptive, retrospective chart review of women delivered of macrosomic neonates over a two-year period from 2015-2016. Detailed clinical and demographic information was recorded. Statistical analysis was carried out using SPSS (version 25.0 IBM, Armonk, New York, USA). Of 22 244 singleton deliveries, 415 were macrosomic infants (1.9%). T
