[Nonoperative treatment of isolated fractures of the distal fibula].

Stable ankle fractures can be successfully managed nonoperatively. Unstable fractures on the other hand should be treated surgically as operative management consistently leads to better outcomes. How to define stability in ankle fractures continues to be a controversial issue. Better understanding of the mechanism of injury as well as improved knowledge of ankle biomechanics now enables surgeons to define stability after isolated fractures of the distal fibula with high accuracy. The following article describes commonly used methods to assess ankle stability after isolated fractures of the lat

Diastasis with low distal fibula fractures: an anatomic rationale.

Tibiofibular diastasis occurs infrequently in displaced ankle fractures that include a low fibula fracture. This is because the interosseous ligament, the major ligament resisting diastasis, lies above the level of the fracture. The anatomic study of the syndesmotic complex of the ankle reported here showed that the interosseous ligament has a variable attachment on the fibula, differing between specimens in its distance above the synovial reflection or joint line. Although most specimens had attachments approximately 1 cm or greater above the joint line, one of 18 specimens attached just abov

Radiologic evaluation of ankle fracture malunions of the fibula.

Ankle fractures involving the distal fibula are common injuries. Malreductions and subsequent malunions of the distal fibula can result in worse clinical outcomes and posttraumatic arthritis. The ability to accurately evaluate and identify malreductions and malunions of the distal fibula is important. A number of different radiographic and CT measurements have been described to assess fibular length and rotation. This review highlights various radiologic measures and discusses their advantages and limitations.

Maisonneuve injury with no fibula fracture: A case report.

BACKGROUND: Ankle syndesmosis injury is difficult to diagnose accurately at the initial visit. Missed diagnosis or improper treatment can lead to chronic complications. Complete syndesmosis injury with a concomitant rupture of the interosseous membrane (IOM) is more unstable and severe. The relationship between this type of injury and Maisonneuve injury, in which the syndesmosis is also injured, has not been discussed in the literature previously. CASE SUMMARY: A 16-year-old patient sustained left medial malleolar fracture, and the associated inferior tibiofibular syndesmotic instability was

Bilateral stress fracture of the fibulae and periostitis of the tibiae.

OBJECTIVE: This study describes a unique case of bilateral stress fractures of the fibulae and provides a literature review. CLINICAL PRESENTATION AND INTERVENTION: A 16-year-old female badminton player presented with pain around the bilateral distal lateral legs. She had mild bilateral varus deformity at the knee joint, and the bilateral ankles showed valgus deformity in standing posture. Radiographs and computed tomography showed periosteal reactions on the bilateral distal fibulae. Technetium-99m bone scintigraphy demonstrated increased uptake in the bilateral distal fibulae and the bilate

Outcome after surgery for Maisonneuve fracture of the fibula.

The aim of this study was to assess outcome in patients treated surgically for Maisonneuve fractures and to highlight factors associated with a suboptimal result. The authors present a review of 14 patients with Maisonneuve fractures. The mean age was 35.5 years. The most common injury mechanism was a twist incurred whilst participating in sport. The goal of treatment was the anatomic restoration of the ankle mortice with accurate renewal of fibular length. All patients had one or two suprasyndesmotic screws inserted percutaneously under fluoroscopic guidance. These were successfully removed u

Operative treatment for Maisonneuve fracture of the proximal fibula.

This study reviewed operative treatment of Maisonneuve fracture of the fibula in 26 patients. Operative treatment consisted of restoration of the fibular length, repair of the lateral and medial ankle ligamentous structures, and placement of one or two suprasyndesmotic screws. After average follow-up of 6.4 years, the clinical results were satisfactory in 23 (88.4%) patients. Operative treatment is the treatment of choice for Maisonneuve fractures, and a satisfactory outcome may be anticipated after appropriate management of any associated bony and syndesmotic injuries.

Fracture-dislocation of the ankle with anterior dislocation of the fibula.

A fracture-dislocation of the ankle with fixed anterior displacement of the proximal fibular fragment is reported. The mechanism of this injury was likely eversion of the foot which produced rupture of the deltoid ligament and distal tibiofibular diastasis, and lateral dislocation of the talus. The proximal fibular fragment was probably displaced laterally and returned to a position anterior to the fibular notch of the tibia. This injury was extremely unstable and required open reduction with internal fixation, not only to reduce the anterior displacement of the fibula but also to repair the a

Fibula physeal fracture causing syndesmotic diastasis.

We report an unusual physeal fibula fracture seen in a 12 year old child. The fragment was rotated and incarcerated in the distal tibiofibular joint causing syndesmotic diastasis. The fragment required open reduction and the fibula was stabilised with k-wires. The patient made an excellent recovery.

[Pseudarthroses of the fibula following fracture of the lower leg].

There are few reports in the literature on nonunion of the fibula following simultaneous fracture of the tibia and fibula. They apparently develop in cases where when only the tibia is stabilized and early functional treatment follows. Six patients suffering from painful nonunion of the fibula were treated in our trauma center from 1985 to 1987. Primary fixation was achieved with a plate in three cases, external fixation in two, and a Küntscher nail in one case. Because of infection, the nail was replaced by external fixation after 2 weeks. After healing of the tibia, the patients developed pa

Fibula-first fixation in same-level distal third tibia and fibula fractures: A pilot randomized controlled trial.

IntroductionFractures of the tibia and fibula occurring at the same level can be technically challenging. Fibular plating (FP) prior to intramedullary nailing (IMN) may facilitate tibial reduction, stabilization, and subsequent tibial healing. The primary goal of this study was to compare tibial fracture healing between FP and no fibular plating (NFP) in patients with same-level distal third tibia/fibula fractures.MethodsPatients ≥18 years with distal third tibia/fibula fractures occurring at the same level were randomized to FP or NFP prior to infrapatellar IMN. The role of fibula fixation in combined distal-third tibia and fibula fractures: a systematic literature review.

OBJECTIVE: The role of concomitant fibula fracture fixation in distal-third tibia fractures regarding alignment, union, and functional outcome is still a topic of debate. In this review, we summarize the available comparative literature regarding fibula fixation in distal third lower leg fractures. MATERIALS AND METHODS: A systematic literature review of articles published between January 2000 and January 2022 in the PubMed, Cochrane, and EMBASE databases about this topic was performed. RESULTS: 746 unique studies were identified of which four randomized-controlled trials and six retrospecti

Healed ORIF in a fibula fracture - Hardware removal or hardware retention for the fibula in a patient with minor symptoms?

- A 31yo female is involved in a fall while climbing on some wet rocks. She was brought into the emergency department and was found to have suffered a closed fracture of the right fibula with a deltoid ligament injury. Distal neurovascular exam was normal. She is otherwise unhurt. She had ORIF of her ankle injury and it has gone onto heal with good results. She comes back at 6 months and complains of vague pain over her lateral ankle and hardware with a normal physical exam. She wonders about hardware removal, (Figs. 1 and 2). PAST MEDICAL HISTORY: - previous history of breast reduction surger

Fibula fracture stabilization with a guide wire as supplementary fixation in tibia fractures.

The tibia is the most commonly fractured long bone. Although the goals of fracture management are straightforward, methods for achieving anatomical alignment and stable fixation are limited. Type of management depends on fracture pattern, local soft-tissue involvement, and systemic patient factors. Tibial shaft fractures with concomitant fibula fractures, particularly those at the same level, may be difficult to manage because of their inherent instability. Typically, management of lower extremity fractures is focused on the tibia fixation, and the associated fibula fracture is managed without

Evaluation of surgical costs associated with isolated distal fibula fractures.

BACKGROUND: Operative fixation of distal fibula fractures is a common surgical procedure within orthopaedics. Understanding the variables that influence cost for surgical management of such injuries is increasingly important. In this study, we aimed to evaluate the cost differences in isolated distal fibula fixation. METHODS: In total, 394 patients were identified who underwent isolated distal fibula fracture fixation between 2018 and 2025 based on Current Procedural Terminology (CPT) coding (27792). Patient demographics, fixation strategy, implant type, and surgical setting were analyzed. Mu

Intramedullary nail fixation of non-traditional fractures: Clavicle, forearm, fibula.

Locked intramedullary fixation is a well-established technique for managing long-bone fractures. While intramedullary nail fixation of diaphyseal fractures in the femur, tibia, and humerus is well established, the same is not true for other fractures. Surgical fixations of clavicle, forearm and ankle are traditionally treated with plate and screw fixation. In some cases, fixation with an intramedullary device is possible, and may be advantageous. However, there is however a concern regarding a lack of rotational stability and fracture shortening. While new generation of locked intramedullary d

Intramedullary Screw Fixation Versus Traditional Plating for Distal Fibula Fractures.

The traditional method of treating fibular fractures in unstable ankle injuries involves open reduction and internal fixation with a plate and screw construct. Less invasive percutaneous fixation techniques with intramedullary fibular screws have been utilized for many years to reduce wound and implant complications while maintaining a stable ankle mortise. However, there have been no direct case-control studies comparing percutaneous intramedullary fibular screw fixation to the traditional open reduction and internal fixation with plates and screws. In our study, we compared radiographic and

Comparison of Locking Versus Nonlocking Plates for Distal Fibula Fractures.

Locking plates might offer a biomechanical fixation advantage for distal fibula fractures with comminution or osteoporotic bone. In January 2011, our unit introduced a bone-specific locking plate for the distal fibula. The aim of the present study was to compare it against more conventional plating system implants for lateral malleolar fixation in terms of outcomes, crude costs, and complications. We retrospectively reviewed a consecutive cohort of patients with closed ankle fractures who presented within a 24-month period. The clinical and radiographic outcomes were compared among conventiona
