How cesarean section rates can be reduced through an effective financial strategy: A protocol for systematic review.

BACKGROUND: The incidence of cesarean section (c-section) has been increasing after the introduction of national health coverage. There is potential evidence that unnecessary c-sections can be reduced through an effective financial strategy, which would make it possible to increase health equity in the future. Consistent with global trends, the rate of c-section in Indonesia increased from 1.6% in 1991 to 17.6% in 2017, while the World Health Organization standard rate is 10% to 15%. This study aims to explore and analyze strategies to reduce c-section rates and to report evidence-based resear

Factors impacting-stillbirth and neonatal death audit in Malawi: a qualitative study.

BACKGROUND: Over one million babies are stillborn or die within the first 28 days of life each year due to preventable causes and poor-quality care in resource-constrained countries. Death audit may be a valuable tool for improving quality of care and decreasing mortality. However, challenges in implementing audit and their subsequent action plans have been reported, with few successfully implemented and sustained. This study aimed to identify factors that affect stillbirth and neonatal death audit at the facility level in the southern region of Malawi. METHODS: Thirty-eight semi-structured i

Large language models versus NASS common coding scenarios: an analysis of accuracy and financial implications using 2026 CPT AMA Codebook Guidelines.

BACKGROUND CONTEXT: Accurate current procedural terminology (CPT) coding is essential for compliant revenue cycle management in spine surgery. However, increasing documentation burdens and the complexity of bundling rules often lead to significant revenue leakage or audit risk. PURPOSE: To evaluate the baseline efficacy of three state-of-the-art large language models (LLMs) in generating accurate CPT codes compared to the "Gold Standard" North American Spine Society (NASS) Common Coding Scenarios and to quantify the downstream financial impact. STUDY DESIGN/SETTING: Comparative artificial in

Reducing Clinical Waste in Flexible Cystoscopy: A Prospective Quality Improvement Study.

AIMS: Flexible cystoscopy is a very common procedure that relies heavily on single-use consumables, resulting in significant procedural waste. This study aimed to evaluate whether a targeted waste-segregation intervention could reduce clinical waste generation during flexible cystoscopy without compromising clinical practice. METHODS: A prospective audit of 50 consecutive diagnostic flexible cystoscopies performed at a tertiary urology centre was undertaken. Twenty-five procedures were evaluated before intervention, followed by structured contamination-based waste segregation training for cli

Auditing a court assessment and advice service for defendants with mental health difficulties: utilizing electronic patient records.

This study is an audit of the Somerset Court Advice and Assessment Service (CAAS) throughout its first year of implementation. It reports that the service successfully met the six desired objectives as set out in its Service Level Agreement. Further to this, it reports that the use of National Health Service electronic patient records within a court setting facilitated the provision of apposite and timely information to the court. Specific findings were that deliberate self-harm/suicidal ideation and mood disorders were the primary reasons for a person requiring CAAS involvement. Violence agai

Improving the ward-based care of patients post-thyroidectomy.

Nursing staff play a crucial role in managing surgical patients in the postoperative period. However, with an increasing risk of subspecialty wards facing closure as a result of financial pressures within the NHS, the knowledge base and expertise of ward-based nursing staff of surgical subspecialties is becoming increasingly limited. Using patients undergoing thyroidectomy as an example, a quality improvement multidisciplinary audit is presented, which shows how simple targeted interventions through a multidisciplinary approach can help maintain high standards of patient care in a secondary ca

Associations Among Stressful Events, Social Support, and Alcohol Use in Women and Men.

Recent stressors may increase the risk of alcohol misuse. However, the number and duration of recent stress, whether social support (SS) moderates these effects, and whether this differs for men and women, are unclear. This cross-sectional study examined the effects of these factors on alcohol use severity, amount, and frequency in 462 community adult women and men. Linear regression (controlling for sex, age, and education) indicated that more stressful life events and longer stress duration were associated with a greater probability of any alcohol use and greater alcohol use severity as meas

Factors affecting return to work after hand injury.

The study of 61 hand-injured subjects suggested that it is more than surgical and technical excellence that facilitates the return-to-work status among the hand-injured wage earners. Financial need, level of activities of daily living, and participation in occupational therapy were found to be directly related to return-to-work status. Additional findings indicated no difference between the rate of return to work for those whose injuries affected their dominant hand over those who had nondominant hand injuries. The amount of medical care needed and whether it was delivered as an outpatient or

Are there patients missing from community heart failure registers? An audit of clinical practice.

BACKGROUND: General practitioners in the UK are financially incentivised, via the Quality Outcomes Framework, to maintain a record of all patients at their practice with heart failure and manage them appropriately. The prevalence of heart failure recorded in primary care registers (0.7-1.0%) is less than reported in epidemiological studies (3-5%). Using an audit of clinical practice, we set out to investigate if there are patients 'missing' from primary care heart failure registers and what the underlying mechanisms might be. DESIGN: The design of this study was as an audit of clinical practi

A framework for identifying cost efficiencies in malaria routine data quality audits: methodology and application in Zambia.

BACKGROUND: Routine data quality audits (RDQA) are a critical activity conducted by many country malaria programs to assess the reliability of reported malaria data, but it requires significant human and financial resources. In collaboration with the Zambia National Malaria Elimination Centre, PATH's Malaria Control and Elimination Partnership in Africa (MACEPA) proposed a framework for identifying cost efficiencies in RDQAs. To demonstrate this approach, previously collected RDQA data from Zambia was used as a case study. METHODS: A systematic, replicable framework to identify cost efficienc

Pilot Trial Results of Mlambe: An Economic and Relationship-Strengthening Intervention to Address Heavy Drinking and Adherence to Antiretroviral Therapy in Malawi.

We evaluated the feasibility, acceptability, and preliminary efficacy of an economic and relationship-strengthening intervention to reduce heavy alcohol use among couples living with HIV in Malawi (Mlambe). Mlambe consisted of training on financial literacy and relationship skills, combined with 1:1 matched savings accounts to invest in an income-generating activity. In a randomized controlled trial, we compared Mlambe to enhanced usual care (EUC). We enrolled 78 married couples having a partner on antiretroviral therapy (ART) who reported heavy alcohol use based on the AUDIT-C. Using targets
