Long-term results after vascularised joint transfer for finger joint reconstruction.

INTRODUCTION: Vascularised complete joint transfer from the finger or the second toe offers the unique possibility of reconstructing a joint defect on the thumb or fingers using autologous tissue, which fully preserves its growth potential. Indications for vascularised joint transfer on the finger in children are set because of lack of therapy options offering normal growth potential. In adults vascularised joint transfer is indicated in case of contraindication for prosthetic joint replacement or arthrodesis. PATIENTS AND METHODS: In a retrospective clinical study 16 vascularised joint trans

Silicone and Pyrocarbon Artificial Finger Joints.

Artificial finger joint design has been developed through different stages through the past. PIP (proximal interphalangeal) and MCP (metacarpophalangeal) artificial finger joints have come to replace the amputation and arthrodesis options; although, these artificial joints are still facing challenges related to reactive tissues, reduced range of motion, and flexion and extension deficits. Swanson silicone artificial finger joints are still common due to the physician's preferability of silicone with the dorsal approach during operation. Nevertheless, other artificial finger joints such as the

Fracture dislocations of the carpometacarpal joints of the fingers.

OBJECTIVE: Fracture dislocations of the multiple carpometacarpal joints [CMCJ] of the fingers are uncommon injuries that can significantly compromise hand function and durability if managed sub-optimally. These injuries are at risk of being missed as they are commonly a part of major high energy trauma with associated more obvious and immediately threatening injuries getting all the attention. The clinical and radiological parameters which could help a surgeon to detect and analyse these injuries well are discussed. The management of these injuries with emphasis on the pattern of K-wire fixati

Double dislocation of finger interphalangeal joints.

A 62-year-old, right-hand-dominant man who had dementia and lived in an Elderly Mentally Infirm (EMI) nursing home was admitted through Accident & Emergency (A&E) department following unwitnessed injury to the left little finger. His examination revealed a swollen and deformed left little finger with a laceration along the middle crease on the volar aspect and head of proximal phalanx visible through this. Distally sensations and capillary refill was normal. X-rays showed a double dislocation of both proximal and distal interphalangeal joints. The finger was reduced under ring block and the la

Arthroscopy of the proximal interphalangeal joints of the finger.

Cadaveric studies were carried out to evaluate the technique, portals and possible indications for arthroscopy of the proximal interphalangeal joints of the finger. We suggest horizontal placement of the hand instead of using a traction tower, as it is important to be able to flex the joint freely. The recommended arthroscopic portals are either between the central slip and the lateral bands of the extensor mechanism or between the lateral band and the collateral ligament. A blunt technique of introduction is used to avoid iatrogenic cartilage damage and possible digital nerve injury.

Tension-band arthrodesis in the finger joints.

Tension-band arthrodesis was employed in 26 finger joints with various pathological lesions. This is a simple procedure based on the principles of compression osteosynthesis. Stability thus achieved makes immobilization of the hand unnecessary and allows full motion of all adjacent joints.

[Arthroscopy of the finger and thumb joints, including the thumb saddle joint].

BACKGROUND: Arthroscopy of the finger joints was first described by Chen in the 1970s. Menon (1996) and Berger (1997) published the first articles on arthroscopic treatment of the trapeziometacarpal joint. Since then, the techniques and procedures of arthroscopy of thumb and finger joints have evolved. TRAPEZIOMETACARPAL JOINT: Arthroscopy for the trapeziometacarpal joint was first used to diagnose rhizarthrosis especially in early stages, and therapeutic procedures have been developed. Debridement and synovectomy with capsular shrinkage and arthroscopic hemi- or total resection for advanced

Tensile and Bending Strength of Birch and Beech Lamellas Finger Jointed with Conventional and Newly Developed Finger-Joint Profiles.

In this study, the tensile and bending strength of birch and beech lamellas finger jointed with conventional (Standard) and newly developed finger-joint profiles (New) are presented. Polyurethane (PUR), Melamine-Urea-Formaldehyde (MUF) and Phenol-Resorcinol-Formaldehyde (PRF) adhesive systems were used to bond the finger joints. The objective of the New profiles was to reduce the stress concentrations within the finger joint by cutting the cross-grooved fingers perpendicular to the main orientation of the finger-joint profile. In the first trials of the development, larger cross-grooved finger

Isolated Small Finger Distal Interphalangeal Joint Dupuytren's Contracture.

Dupuytren's contracture is a disease involving abnormal myofibroblast proliferation and collagen deposition leading to the formation of pathologic cords in the hand. Given that Dupuytren's contractures rarely extend to the distal interphalangeal joint (DIP), affecting only 5% of patients, there are few cases reported in the literature. Collagenase injection is a frequently used option for minimally invasive treatment of Dupuytren's disease with greater than a 20-degree joint contracture. Unfortunately, there is limited research on the effectiveness of these injections in isolated DIP joint def

First assessment of thermoacoustic tomography for in vivo detection of rheumatoid arthritis in the finger joints detection of rheumatoid arthritis in the finger joints.

BACKGROUND: The diagnosis of rheumatoid arthritis (RA) is complicated because of the complexity of symptoms and joint structures. Current clinical imaging techniques for the diagnosis of RA have strengths and weaknesses. Emerging imaging techniques need to be developed for the diagnosis or auxiliary diagnosis of RA. PURPOSE: This study aimed to demonstrate the potential of thermoacoustic tomography (TAT) for in vivo detection of RA in the finger joints. METHODS: Finger joints were imaged by a TAT system using three different microwave illumination methods including pyramidal horn antenna, an

[Arthroses of the finger joints and thumb saddle joint and occupationally related factors].

In a one-to-one matched case control study, 37 cases with rhizarthrosis (31 female, 6 male) and 44 cases with osteoarthritis in the finger joints (35 female, 9 male) were compared to their matches equal in sex and age regarding occupational strains. For calculating the odds ratios matching was maintained. In females the risk of rhizarthrosis was elevated for typists (OR 5.0, CI 1.27-19.59) and for work involving dexterity (OR 2.0, CI 0.77-5.23). In both sexes an elevated odds ratio for osteoarthritis in the finger joints was found for repetitive work (OR 3.8, CI 1.52-9.49).

[Long fingers proximal interphalangeal joint trauma].

Sprains and dislocations of the proximal interphalangeal joint (PIP) are very common but are, nevertheless, often missed. They require an appropriate treatment to prevent stiffness and deformities. Initial assessment should include anteroposterior and true lateral radiographs. Intra-articular fractures are referred to the specialist. Clinical examination to detect laxity is essential and will guide the treatment. The treatment is based on the restoration of joint congruency and achieving early mobilization. Surgical indication is rare. The evolution is slow, pain and joint swelling can persist

Anatomy and Biomechanics of the Finger Proximal Interphalangeal Joint.

A complete understanding of the normal anatomy and biomechanics of the proximal interphalangeal joint is critical when treating pathology of the joint as well as in the design of new reconstructive treatments. The osseous anatomy dictates the principles of motion at the proximal interphalangeal joint. Subsequently, the joint is stabilized throughout its motion by the surrounding proximal collateral ligament, accessory collateral ligament, and volar plate. The goal of this article is to review the normal anatomy and biomechanics of the proximal interphalangeal joint and its associated structure

The Treatment of Primary Arthritis of the Finger and Thumb Joint.

BACKGROUND: Primary finger and thumb joint arthritis is common, with a markedly rising prevalence from age 50 onward. As the population as a whole ages, the need for effective, stage-appropriate treatment of this condition is increasing. METHODS: This review is based on pertinent publications retrieved by a selective search in the PubMed and Cochrane Library databases. RESULTS: Pain on movement and morning stiffness are commonly reported symptoms. Thorough physical examination and plain x-rays are mandatory. In the early stages of primary finger and thumb joint arthritis, a conservative, mul

Rotational dislocation of the proximal interphalangeal joint of the finger.

We describe a case of rotational dislocation of the proximal interphalangeal joint of the ring finger. This injury was not initially appreciated and therefore closed reduction failed. The clinical findings included puckering of the skin on the dorsum of the joint and rotational incongruity on radiographs. At open reduction there was interposition of the lateral band. Identification of the pathology allows early successful treatment.
