Are the orthostatic fluid shifts to the calves augmented in autonomic failure?

BACKGROUND: In autonomic failure (AF), blood pressure (BP) falls upon standing which is commonly ascribed to defective vasoconstriction and excessive pooling. Observations on the amount of pooling in AF are contradictory. METHODS: We evaluated pooling using strain-gauge plethysmography (SGP) during head-up tilt (HUT) with a parachute harness fixed to the tilt table to avoid muscle tension in the lower limbs and thus to maximise pooling. 23 healthy subjects and 12 patients with AF were tilted for 5 min. BP and calf volume changes, as measured by SGP, were measured continuously. Multiple regres

Stockmayer fluid with a shifted dipole. I. Bulk behavior.

Shifting the point dipole from the center of a Stockmayer particle is a simple geometric modification, whose implications on liquid structure, dielectric response, and phase behavior remain incompletely understood. We combine molecular dynamics simulations with analytical theory to provide a unified physical interpretation of how dipole shifts reshape microscopic correlations and propagate to macroscopic properties. We show that dipole-shifting breaks the fore-aft symmetry of the local electrostatic field, producing only modest changes in radial packing but strong alterations in angular struct

Frusemide results in an extracellular to intracellular fluid shift in horses.

REASONS FOR PERFORMING STUDY: Frusemide (Lasix) is commonly used diuretic in horse racing and equine clinical practice. While pharmacology, pharmacodynamics, renal and haematological effects of frusemide have been studied in horses, its effects on the distribution of fluid within the horse remain unknown. OBJECTIVE: To quantify the effects of frusemide on extracellular and intracellular fluid shifts. METHODS: Horses were infused with 1 mg/kg body mass (n = 7) or 2 mg/kg (n = 9) i.v. frusemide. Total body water (TBW), extracellular fluid volume (ECFV) and plasma volume (PV) were measured usin

Electrolyte-based calculation of fluid shifts after infusing 0.9% saline in severe hyperglycemia.

BACKGROUND: Early treatment of severe hyperglycemia involves large shifts of body fluids that entail a risk of hemodynamic instability. We studied the feasibility of applying a new electrolyte equation that estimates the degree of volume depletion and the distribution of infused 0.9% saline in this setting. METHODS: The new equation was applied to plasma and urinary concentrations of sodium and chloride measured before and 30 min after a 30-min infusion of 1 L of 0.9% saline on two consecutive days in 14 patients with severe hyperglycemia (mean age 50 years). The extracellular fluid (ECF) vol

The consequences of sudden fluid shifts on body composition in critically ill patients.

INTRODUCTION: Estimation of body composition as fat-free mass (FFM) is subjected to many variations caused by injury and stress conditions in the intensive care unit (ICU). Body cell mass (BCM), the metabolically active part of FFM, is reported to be more specifically correlated to changes in nutritional status. Bedside estimation of BCM could help to provide more valuable markers of nutritional status and may promote understanding of metabolic consequences of energy deficit in the ICU patients. We aimed to quantify BCM, water compartments and FFM by methods usable at the bedside for evaluatin

Fluid losses and hydration status of industrial workers under thermal stress working extended shifts.

AIMS: To assess whether workers under significant thermal stress necessarily dehydrated during their exposure and whether "involuntary dehydration" was inevitable, as supported by ISO 9866 and other authorities. Other objectives were to quantify sweat rates against recommended occupational limits, to develop a dehydration protocol to assist with managing heat exposures, and to understand the role of meal breaks on extended shifts in terms of fluid replacement. METHODS: A field investigation to examine the fluid consumption, sweat rates, and changes in the hydration state of industrial workers

Extracellular and Intracellular Fluid Shifts on the Onset of Transcutaneous Auricular Vagus Nerve Stimulation.

In this paper, we propose multifrequency body impedance measurement technique for monitoring the onset of vagus nerve stimulation. In response to transcutaneous electrical vagus stimulation, changes occur in body fluids i.e. fluid shifts in extracellular and intracellular media that can be assessed by Cole parameters Re and Ri before and after stimulus, by measuring single side impedance spectroscopy (from 3 kHz to 1 MHz) between wrist and ankle. Following a resting period of 5 minutes in supine position, auricular vagus nerve was stimulated for 3 minutes and BIS performed every 10 seconds, on

Gingival Crevicular Fluid Proteomic Shift: From Gingivitis to Periodontitis.

INTRODUCTION AND AIMS: Gingivitis is a reversible, mild inflammation, whereas periodontitis is a chronic condition with irreversible tissue destruction. Despite the widespread use of clinical indicators, their lack of molecular specificity limits mechanistic diagnosis. Gingival crevicular fluid, a noninvasive medium reflecting localized immune activity, has been insufficiently explored in proteomic studies to distinguish gingivitis from periodontitis. This study revealed quantitative proteomics and network analysis to identify distinct molecular characteristics differentiating the two conditio

Fluid Shifts and Muscle Loss in Critical Care: Accuracy of Ultrasound Versus Bioelectrical Impedance Analysis.

Background: Critically ill patients experience rapid muscle wasting during their ICU stay. Ultrasound (US) and bioelectrical impedance analysis (BIA) are widely used to assess muscle mass; however, their accuracy may be affected by fluid balance alterations. This study aimed to compare the reliability of US and BIA in detecting muscle loss under varying fluid balance conditions in ICU patients. Methods: In this prospective observational study, adult ICU patients with an ICU stay of ≥7 days were evaluated on Days 1, 5, and 7. Muscle thickness was measured using US, and phase angle (PhA) using B

Fluid shifts due to microgravity and their effects on bone: a review of current knowledge.

Bone loss during long-term space flight is potentially a significant problem. Bone mineral density measurements show that bone is lost at a rate of about 1% per month from the lumbar spine, and 1.5% per month from the hip. Bone density changes are not uniform throughout the skeleton, and there is accumulating evidence that the distribution of bone density changes may be related to fluid shifts observed in microgravity. The paper summarises the data relating fluid shift of bone density changes in microgravity. In addition, microgravity and ground-based experiment were carried out to investigate

Chloride-depletion metabolic alkalosis induces ECF volume depletion via internal fluid shifts in nephrectomized dogs.

We recently reported that chloride-depletion metabolic alkalosis (CDMA) results in renal losses of Na, K, and water. In these studies we investigated whether CDMA (induced using a new model that avoids external changes in Na and water balance) was also associated with internal Na and water shifts out of the ECF. CDMA was induced using haemofiltration in functionally nephrectomized dogs. Plasma ultrafiltrate was substituted quantitatively with a solution duplicating each dog's plasma electrolyte composition in control animals, and with a solution containing HCO3 as the sole anion in CDMA animal

Feasibility of serial perioperative lung ultrasound to monitor fluid shifts during thulium laser prostate enucleation.

PURPOSE: Irrigation fluid absorption (IFA) during transurethral surgery of the prostate continues to pose a risk for fluid overload and electrolyte imbalance, yet reliable methods for quantifying absorption remain limited. Lung ultrasound (LU), increasingly used to detect pulmonary pathology, provides a noninvasive approach to assess fluid accumulation. METHODS: In this prospective single-center observational study, 36 patients aged ≥ 60 years undergoing elective urethral-sparing Thulium: YAG anatomical endoscopic enucleation of the prostate (AEEP; ThuLEP) were examined. LU was performed preo

Pathogenesis of obstructive sleep apnoea in hypertensive patients: role of fluid retention and nocturnal rostral fluid shift.

Obstructive sleep apnoea (OSA) is highly prevalent in hypertensive patients, particularly those with drug resistance. Evidence from animal experiments, epidemiologic studies and clinical trials strongly suggest a causal link. Mechanistic studies argue for increased sympathetic neural activity and endothelial dysfunction. However, disturbances in fluid volume regulation and distribution may also be involved in the pathogenesis of these two conditions. Several studies have shown a high prevalence of OSA in fluid-retaining states including hypertension, a direct relationship between the severity

Pilot study of fluid shift assessment using bioelectrical impedance spectroscopy.

Maintaining fluid balance between intracellular and extracellular compartments is essential for physiological stability, yet disturbances in this equilibrium are common in conditions such as dehydration, sepsis, and perioperative stress. Traditional techniques for quantifying body water distribution, including isotope dilution, are accurate but invasive and unsuitable for repeated or bedside monitoring. Bioelectrical impedance spectroscopy (BIS) offers a non-invasive, rapid, and frequency-dependent approach to estimating total body water (TBW), extracellular water (ECW), and intracellular wate

Shift in pH of biological fluids during storage and processing: effect on bioanalysis.

The pH of ex vivo plasma, bile and urine was monitored at different times and temperatures of storage, and following different sample processing methods such as ultrafiltration, centrifugation, precipitation and evaporation. The results showed that the pH of ex vivo plasma, bile and urine increased upon storage, and following sample processing and could lead to significant degradation of pH-labile compounds. Several compounds were used to illustrate the impact of pH shifts on drug stability and interpretation of results obtained from in vivo studies. For example, after 1 h of incubation (37 de

A Paradigm Shift in Hydrocephalus Management: The Promise of Endovascular Cerebrospinal Fluid Diversion.

Normal pressure hydrocephalus (NPH) predominantly affects the elderly and is the most common form of adult-onset hydrocephalus. Clinically, it is characterized by Hakim's triad-gait disturbance, dementia, and urinary incontinence-and is marked by abnormal cerebrospinal fluid accumulation without increased intracranial pressure. Although the exact pathophysiology is not fully understood, proposed mechanisms include glymphatic dysfunction, blood-brain barrier disruption, and hypoperfusion. In carefully selected patients, the standard treatment, ventriculoperitoneal shunting, can provide signific

Artificial gravity: an effective countermeasure for microgravity-induced headward fluid shift?

Long-duration spaceflight is associated with pathophysiological changes in the intracranial compartment hypothetically linked to microgravity-induced headward fluid shift. This study aimed to determine whether daily artificial gravity (AG) sessions can mitigate these effects, supporting its application as a countermeasure to spaceflight. Twenty-four healthy adult volunteers (16 men) were exposed to 60 days of 6° head-down tilt bed rest (HDTBR) as a ground-based analog of chronic headward fluid shift. Subjects were divided equally into three groups: no AG (control), daily 30-min intermittent AG
