Assessment of tarsometatarsal skeletal malalignment in congenital foot deformities such as clubfoot and vertical talus is possible with ultrasound. Sonography can be useful in deciding on the therapeutic concept and evaluating therapeutic effects.

Rheumatoid hindfoot deformity presents with hindfoot eversion, flattening of the longitudinal arch and abduction of the forefoot. Splayfoot, as the typical rheumatoid forefoot deformity, is mostly associated with various toe malformations, i.e. hallux valgus, hammer toe and claw toe, which may either be attributed to hindfoot malalignment or develop as a separate entity. The algorithm of treatment, comprising clinical assessment of both lower limbs, includes both orthotic shoe devices and surgical treatment. In rheumatoid flatfoot, arthrodesis of the hindfoot with lengthening of the lateral colu

The lack of muscle control in neurological disorders often leads to deformities of the foot. The equinus (a contracture of the triceps surae muscle), the abduction and clubfoot deformity are the most common ones. The present paper describes the deformities and stresses the functional relevance for the patient. Therapeutic principles of the conservative orthopaedic means are described with special emphasis on the need of an adequate height of the aide and the foot position. Casting and the application of Botulinun toxine are described having a major side effect of muscle weakening. A combinatio

The spectrum of normal variations of children's feet is extremely broad and often difficult to separate from pathological conditions. Especially the flexible flatfoot and the pes adductus normally disappear during growth, and even if they do persist up to adult life, they hardly have any pathological significance. It therefore appears proper to see foot deformities of children from the prognosis point of view, that is, to differentiate between a benign, pain-free course of development with no functional restriction even under load, and pathological deformities which systematically require cons

Foot deformities may reflect a generalized disorder, especially a neurologic problem; thus, the child should have a brief general examination. Many infantile foot deformities, such as calcaneovalgus, are postural and self-correcting. Metatarsus varus is not referred for treatment until age 2 months and then only if the deformity is moderate or severe. Fixed forefoot equinus and heel varus characterize a clubfoot, which requires immediate treatment. Corrective shoes are not advised as the primary treatment for metatarsus varus or clubfoot but often are prescribed to maintain the corrected posit

Conditions of ankle instability, peroneal tendon tears, and stress fractures of the lateral metatarsals are commonly encountered in a clinical foot and ankle practice. Evaluation of the supporting foot structure is critical to prevent failure of index procedures. The prominence of the subtle cavus foot is now a recognized entity and must be properly diagnosed and addressed surgically.

Foot deformity in infants is the most common congenital musculoskeletal condition. A precise diagnosis can sometimes be impossible to establish clinically. Radiologic imaging plays a major role in the evaluation of musculoskeletal abnormalities. However conventional imaging techniques, such as plain radiographs of the foot, are of very little help in this age group because of the lack of ossification of the tarsal bones. US presents a significant advantage because it permits the visualization of cartilaginous structures. This leads to the detailed assessment of foot deformities in infants. Fur

Although the neurological defects associated with cerebral palsy are not progressive, secondary musculoskeletal disorders due to growth and gravity are variable. In the clinical analysis of spastic foot deformities different mechanisms that produce a variety of deformities have to be analyzed. The goals of surgical treatment are correction of the deformity, reestablishment of stability of the foot and preservation of functionally important ranges of motion and muscle strength. The most common spastic foot deformities are equinus, planovalgus, equinovarus and calcaneus. For treatment soft tissu

In children, the lower extremities have extremely wide variations of normal, including calcaneovalgus foot, metatarsus adductus and flatfoot. The first two conditions often resolve or improve spontaneously over time and may be considered abnormal if they persist into adulthood. Serial casting, splints and surgery are rarely necessary. Corrective shoes, orthotic inserts and some forms of splinting have not proved to be effective. The primary care physician must establish an early and correct diagnosis, perform a general screening examination to rule out accompanying musculoskeletal deformities,

During the period of development foot deformities can occur, not only during the growth and development, but also in the later age. The most frequent foot deformity is flatfoot, congenital club foot and hallux valgus. Prior to the decision on surgical treatment of the deformity, whenever possible the patient should be referred for physical therapy that may yield acceptable results in specific treatment phases. The basis of the treatment involves kinesitherapy, application of certain agents (thermotherapy, electrotherapy, ultrasound) and orthosis for maintaining corrections. If such therapy doe

Any surgical treatment of spastic foot deformities must be preceded by an exact preoperative analysis of every aspect of the deformity and its functional consequences. The goals of surgical treatment are correction of the deformity, reestablishment of the stability of the foot and preservation of functionally important ranges of motion and muscle strength. These goals can usually be achieved only by a package of several simultaneous procedures. The foot must never be treated in isolation. Proximal joints have to be considered as well as the opposite side, because a plantigrade position of the

Clinical observation suggests that deformities of the foot and ankle are common in children with cerebral palsy. Two hundred children with cerebral palsy attending the Central Remedial Clinic were examined and photographed. The children had no previous surgical intervention and were between 1.5 and 19 years of age. Deformities were assessed and related to the type of cerebral palsy and mobility status.

Of 104 patients with corrective surgery for foot deformities subsequent to a cerebrovascular accident from 1980 until 1983, 53 patients returned for clinical examination and 22 patients were evaluated by questionnaire and telephone interview. The average follow-up period was 6.4 years. The operative techniques were tenotomy of the toe flexors for hammer-toe deformity, lengthening of the aponeurosis of the gastrocnemius for equinus deformity, and transfer of the anterior tibial tendon or the posterior tibial tendon or the long toe flexors for varus deformity. In 74% of patients, correction was

Hallux valgus leads to lateral deviation of the big toe's longitudinal axis. In consequence, the forces working on the tip of the toe during unrolling of the foot are shifted in medial direction. As a result, subungual clavi, unguis incarnatus, unguis convolutus, or laterally turning onychogryposis like a cork-screw develop.

Major foot deformities were analyzed using standardized radiographic drawings of the foot in weight-bearing. Specific criteria and a classification of foot deformities by radiographic analysis are presented, utilizing "lining systems" related to the main bones of the hindfoot. The radiographic appearance of the foot is described and analyzed, as are the principal deformities such as varus and valgus heel and forefoot. The prefixes "talipes" and "pes" have been used to signify congenital and acquired deformities, respectively.Specific foot deformities, including talipes equinovarus (clubfoot), p

The aim of this study were to assess what type of foot deformities are found in rheumatoid arthritis (RA) patients, to detect frequency of deformities, and to evaluate deformities affecting Foot Function Index (FFI) and patient functional capacity. Anteroposterior and lateral weight-bearing radiographs of 156 feet of 78 patients who had RA for > or =2 years and of 76 feet of 38 healthy controls were studied. We measured hallux valgus angle, intermetatarsal angle between first and second (M1/2) and intermetatarsal angle between first and fifth (M1/5) on anteroposterior radiographs, and calcanea
