Anatomical Basis of the Intermediate Dorsal Pedal Neurocutaneous Perforator Pedicled Propeller Flap: A Cadaveric Dissection.

Soft tissue defects of the forefoot represent a challenging surgical modality to reconstructive microsurgeons. This study describes the anatomical basis and design of the perforator-based intermediate dorsal pedal neurocutaneous vascular flap. Thirty fresh human lower limb specimens were injected with red latex and used for dissection of the dorsal vascular and neural anatomy of the foot. The direction and distribution of the intermediate dorsal cutaneous nerve and the vascular anatomy of the third dorsal artery of the plantar arch, along with the intermediate dorsal neurocutaneous nutrient ve

Influence of tibialis posterior muscle activation on foot anatomy under axial loading: A biomechanical CT human cadaveric study.

BACKGROUND: Collapse of the medial longitudinal arch and subluxation of the subtalar joint are common occurrences in adult flatfoot deformity. Controversy exists about the role of the tibialis posterior (TP) tendon as first and/or essential lesion. Subtle changes in the foot configuration can occur under weight bearing. PURPOSE: This human cadaveric study is designed to investigate the effect that isolated actuation of the TP tendon has on the medial longitudinal arch and the hindfoot configuration under simulated weight bearing. METHODS: A radiolucent frame was developed to apply axial load

Development of the foot in prenatal life in relation to idiopathic club foot.

The development of the human foot from the eighth to the twenty-first intrauterine week was studied in 147 specimens. The results documented a stage of "physiologic club foot" characterized by a medial deviation and plantigrade orientation of talar neck and head during the ninth week. At the same time, the calcaneus assumes a position of varus with the forefoot in adduction. By the eleventh week, the foot assumes a normal position. All findings during the stage of physiologic club foot closely resemble those of the intrinsic type of club foot.

The Effect of Foot Arch Morphology and Plantar Pressure Distribution on Craniovertebral Angle and Acoustic Voice Parameters Across Different Postures.

OBJECTIVES: Human voice is a holistic phenomenon shaped by whole-body biomechanics and the integrity of the kinetic chain. This study investigated the effects of foot arch morphology on the craniovertebral angle and acoustic voice parameters across four postural conditions, in the context of forefoot plantar pressure distribution. METHODS: Seventeen female university students were classified into a neutral arch group (n = 7) and a pes planus group (n = 10) based on three-dimensional optical foot scanning. In each of four postural conditions defined as neutral, kyphotic, upright, and anterior

A computational human model for exploring the role of the feet in balance.

Many studies concerning human balance use computational models that represent the body as a single, double, or triple inverted pendulum while ignoring the feet. Clinical research, however, has begun to more closely examine specific contributions of the feet in balance, leading to a disparity between the state of clinical research and the models used for simulation. Here, we expand the single inverted pendulum model by adding four additional rigid links to represent the feet. Model parameters, equations of motion, actuation based on human musculature, and control based on proprioception are dis

Some new aspects in the pathology of clubfoot.

As part of a research project that is still in progress, 12 human embryos, fetuses, and one stillborn infant with clubfeet, were given injections of barium sulfate solution with China ink for correlated-contrast radiography and anatomic dissections of cleared specimens including histologic sections. The following observations suggested a vascular factor in the etiology of congenital idiopathic clubfoot: (1) the muscular, tendinous, fascial and other soft elements were not conspicuously abnormal; (2) radiologically and anatomically, all deformed feet had vascular abnormalities consisting of an

Intraosseous and extraosseous blood supply to the medial cuneiform: implications for dorsal opening wedge plantarflexion osteotomy.

BACKGROUND: Osteotomies of the medial cuneiform are commonly used to correct forefoot deformity. Bone healing occurs despite periosteal stripping of the dorsal and medial surfaces of this widely articulated bone followed by osteotomy in the midsection of the bone. The objective of this study was to characterize the blood supply of the medial cuneiform. METHODS: Thirty matched pairs of adult cadaver legs, 60 legs total, were amputated below the knee, and arterial casts were created with India ink and latex. Soft tissues were debrided, allowing visualization of the extraosseous blood vessels. I

Effect of first metatarsal shortening and dorsiflexion osteotomies on forefoot plantar pressure in a cadaver model.

BACKGROUND: Metatarsalgia of the second ray is a common problem associated with disorders of the first metatarsal. It also occurs after the operative treatment of those disorders. Plantar pressure changes from alteration of the static and dynamic structure of the forefoot may be associated with this condition. This study evaluated changes in plantar forefoot pressure especially under the second metatarsal head after three operative procedures on the first ray. METHODS: Each of 12 cadaver foot specimens was cyclically loaded on the servohydraulic MTS Mini Bionix test frame (MTS Systems Corp.,

Eumycetoma caused by Diaporthe phaseolorum (Phomopsis phaseoli): a case report and a mini-review of Diaporthe/Phomopsis spp invasive infections in humans.

Diaporthe phaseolorum (syn. Phomopsis phaseoli) is a frequent fungal parasite of plants, present on all continents around the world. It has rarely been involved in human diseases. We report a case of eumycetoma with osteomyelitis of the forefoot caused by this fungus and diagnosed by molecular biology. The patient had positive HTLV-1 serology and was a farmer from French Guiana who walked barefoot. He was successfully treated with long-term oral itraconazole (400 mg/day). A review of the literature underlines the essential roles of plants and host immunosuppression in this infection and the fa

Foot segments motion during uneven terrain walking and influence on lower limb biomechanics.

Human locomotion has been extensively studied on flat ground; however, everyday walking often occurs on uneven terrain, which poses additional biomechanical challenges. The role of foot segmental mobility under these conditions remains underexplored. This study investigated how irregular surfaces influence foot kinematics and lower-limb biomechanics. Twelve able-bodied young adults performed eight barefoot tasks at self-selected speed, including level walking, obstacle crossing and walking on uneven ground. Multi-segment foot kinematics was quantified using the Oxford Foot Model, while hip, kn

Sterkfontein member 2 foot bones of the oldest South African hominid.

Four articulating hominid foot bones have been recovered from Sterkfontein Member 2, near Johannesburg, South Africa. They have human features in the hindfoot and strikingly apelike traits in the forefoot. While the foot is manifestly adapted for bipedalism, its most remarkable characteristic is that the great toe (hallux) is appreciably medially diverged (varus) and strongly mobile, as in apes. Possibly as old as 3.5 million years, the foot provides the first evidence that bipedal hominids were in southern Africa more than 3.0 million years ago. The bones probably belonged to an early member

Measuring the Forefoot-to-Rearfoot Relationship in the Sagittal Plane-A Novel Measurement for Evaluating Pseudoequinus in the Lower Extremity.

Evaluation of the static angle of the forefoot-to-rearfoot relationship in the sagittal plane is essential in determining the degree of pseudoequinus in the lower extremity. The concept of an increased sagittal plane forefoot-to-rearfoot relationship is generally referred to as anterior equinus, which may result in a pseudoequinus syndrome. As its name implies, pseudoequinus is not a true lack of ankle joint dorsiflexion but rather a condition wherein the ankle joint functions as if it were restricted. Currently, the literature lacks specifications on how to obtain this measurement. Qualitativ

Long-term results of the modified Hoffman procedure in the rheumatoid forefoot. Surgical technique.

BACKGROUND: Rheumatoid arthritis commonly affects the forefoot, causing metatarsalgia, hallux valgus, and deformities of the lesser toes. Various types of surgical correction have been described, including resection of the lesser-toe metatarsal heads coupled with arthrodesis of the great toe, resection arthroplasty of the proximal phalanx or metatarsal head, and metatarsal osteotomy. We report the results at an average of five and a half years following thirty-seven consecutive forefoot arthroplasties performed in twenty patients by one surgeon using a technique involving resection of all five

Giant epidermoid cyst on the forefoot.

BACKGROUND: Epidermoid cysts are the most common cysts of the skin, usually appearing in the hairy regions. However, epidermoid cysts on the non-hair-bearing areas, such as the palms and the soles, are rare and seldom reach a size of more than 5 cm in diameter. OBJECTIVE: To present a patient with an epidermoid cyst of unusual location and size. METHODS: A 23-year-old woman with a painless tumorlike mass on her right forefoot was examined and the mass was excised. RESULTS: The 6.5 cm x 5.5 cm x 5.5 cm mass was totally excised. Histologic diagnosis was reported as an epidermoid cyst. CONCLU

A Passive Three Degree of Freedom Transtibial Prosthesis With Adjustable Coronal Compliance and Independent Toe Joint.

The effects of including lateral compliance or a toe joint in transtibial prostheses have been studied independently, showing the potential to improve the gait biomechanics in terms of stability, walking speed, and metabolic cost. However, both of these features are not commonly found in commercial prostheses despite their importance in human gait. In this work, we present a multi-axis passive transtibial prosthesis with three degrees of freedom (DOF). The prosthesis includes a compliant and adjustable coronal articulation using beam springs, an independent 3D-printed flexible forefoot as a to

Associations between changes in loading pattern, deformity, and internal stresses at the foot with hammer toe during walking; a finite element approach.

Over the past decade, Finite Element (FE) modelling has been used as a method to understand the internal stresses within the diabetic foot. Foot deformities such as hammer toe have been associated with increased risk of foot ulcers in diabetic patients. Hence the aim of this study is to investigate the influence of hammer toe deformity on internal stresses during walking. A 3D finite element model of the human foot was constructed based on capturing Magnetic Resonance Imaging (MRI) of a diabetic neuropathic volunteer exhibiting hammer toe. 3D gait measurements and a multi-body musculoskeletal

Streptococcus uberis and Staphylococcus aureus forefoot and blood stream co-infection in a haemodialysis patient: a case report.

BACKGROUND: Streptococcus uberis, the most frequent cause of mastitis in lactating cows, is considered non-pathogenic for humans. Only a few case reports have described human infections with this microorganism, which is notoriously difficult to identify. CASE PRESENTATION: We report the case of a 75-year-old male haemodialysis patient, who developed a severe foot infection with osteomyelitis and bacteraemia. Both Streptococcus uberis and Staphylococcus aureus were identified in wound secretion and blood samples using mass spectrometry. The presence of Streptococcus uberis was confirmed by sup
