The physician workforce and financing of graduate medical education. American College of Physicians.

This paper addresses key issues concerning the physician workforce and the financing of graduate medical education. The American College of Physicians recommends the establishment of a national advisory organization to develop a coherent and coordinated national policy on the health professions workforce. Given the increasing oversupply of physicians, the College recommends that no new medical schools be created, that total enrollment in U.S. medical schools not increase, and that the number of international medical graduates entering residency training in the United States be restricted. What are the precise reasons for the disparity in referrals to fitness to practise between international and UK medical graduates?

PURPOSE OF THE STUDY: To discover the precise reasons for referring a doctor to the Medical Practitioners Tribunal Service, and whether there are any disparities between referrals of international medical graduates and UK graduates. Further to consider whether understanding the precise reasons would provide insight into the nature of referrals. STUDY DESIGN: We collected and analysed the data from the Medical Practitioners Tribunal Service website over a period of 12 months. RESULTS: There were 228 cases of which 142 (62%) were international medical graduates and 86 (38%) were UK graduates.

Comparison of performances of transfer students and first-year acceptees.

Graduates of the Medical College of Pennsylvania during 1970-1977 were studied to determine the impact of three types of preclinical training on prelicensure clinical performance. The 586 students who comprised these groups consisted of 471 students admitted to the first year, 53 students admitted with advanced standing from U.S. medical or graduate programs, and 62 students admitted with advanced standing from foreign medical schools. Academic performances among the groups differed as measured by scores on the examinations of the National Board of Medical Examiners. However, perceived clinica

The Growing Nationwide Radiologist Shortage: Current Opportunities and Ongoing Challenges for International Medical Graduate Radiologists.

The United States is facing shortages in a myriad of medical fields, including diagnostic radiology (DR). The increasing number of imaging studies, owing to advancing technology and an aging population, is outgrowing the capacity of radiologists. However, there have not been effective long-term solutions to manage this issue, which could have a looming effect on patient care and radiology residents' education. The alternate pathway introduced by the American Board of Radiology (ABR) allows international medical graduates (IMGs) who have completed DR residency outside the U.S. and Canada to be

Offshore Medical Schools Are Buying Clinical Clerkships in U.S. Hospitals: The Problem and Potential Solutions.

U.S. medical education faces a threat from for-profit Caribbean medical schools which purchase clinical rotation slots for their students at U.S. hospitals. These offshore schools are monetizing a system that was previously characterized as a duty-the duty of the current generation of physicians to educate their successors. Offshore schools purchase clinical rotation slots using funds largely derived from federally subsidized student loans. This leads to pressure on U.S. schools to pay for clinical clerkships and is forcing some of them to find new clinical training sites.For-profit Caribbean

Doctor Retention or Migration: From Ireland to the World? Comment on "Doctor Retention: A Cross-sectional Study of How Ireland Has Been Losing the Battle".

The recent study of prospective doctor migration and retention suggests that more than half of junior doctors intend to migrate from Ireland. While intent is not necessarily outcome, such intentions match similar survey results in Ireland and elsewhere. The rationale for migration is described as a function of difficult workplace circumstances (notably long hours and mismanagement). Lifestyle factors may however also be important for both migration and significant levels of return migration. These are related to family formation, and to an established culture of migration, that has contributed

[Foreign knowledge being tested: European physicians fighting the Moscow plague of 1771].

The transfer of Western medicine to Russia increased significantly in the Eighteenth century. Foreign doctors were employed, their writings translated, their education standards copied. But who regarded that knowledge as superior and why? Taking the Moscow Plague of 1771 as a case study, this article examines the crucial role foreign and Russian medical practitioners played during the epidemic. It argues that especially those ideas and practices that were useful for social control filtered into politics and public discourse, but failed to convince the majority of the population.

A program for successful integration of international medical graduates (IMGs) into U.S. surgical residency training.

OBJECTIVE: U.S. surgery residency programs have traditionally attracted international medical graduates (IMGs). However, the qualifications and performance of IMGs are variable and difficult to predict. Poor performance negatively affects patient care, the residency program, and the IMGs. We sought to identify causes of poor performance and to develop a program to identify those with chances to succeed. DESIGN: Longitudinal study. Retrospective analysis. Description of a new program. SETTING: University of Washington, a tertiary care teaching hospital. PARTICIPANTS: Performance of former IM

The need for national medical licensing examination in Saudi Arabia.

BACKGROUND: Medical education in Saudi Arabia is facing multiple challenges, including the rapid increase in the number of medical schools over a short period of time, the influx of foreign medical graduates to work in Saudi Arabia, the award of scholarships to hundreds of students to study medicine in various countries, and the absence of published national guidelines for minimal acceptable competencies of a medical graduate. DISCUSSION: We are arguing for the need for a Saudi national medical licensing examination that consists of two parts: Part I (Written) which tests the basic science an

Students' response to disaster: a lesson for health care professional schools.

The response of medical students, young physicians, and other health professionals to the February 2010 earthquake and tsunami in Chile provides important lessons about health care delivery during disasters and about the development of professionalism. Tertiary and secondary care of victims of these disasters was possible because local and national resources were available and field hospitals provided by Chile's armed forces and foreign countries replaced damaged hospitals. However, primary care of persons living on the outskirts of towns and in small villages and coves that were destroyed and

Establishing a national clearinghouse on international medical education programs: an idea whose time has finally come.

In 2006, a special committee appointed by the Federation of State Medical Boards (FSMB) issued a report that evaluated undergraduate medical education in the United States and abroad. With accreditation systems that can provide reasonable and adequate assurance for the quality of medical education in this country, the committee focused its attention on international medical school programs. Because international medical graduates (IMG) comprise a quarter of the physician workforce, U.S. medical licensing boards continue to seek useful and appropriate information on the medical schools of their

United States citizens studying medicine abroad. Their backgrounds and test performance.

To acquire information about the characteristics of U.S. citizens who had recently studied medicine abroad, the Educational Commission for Foreign Medical Graduates (ECFMG) and the Association of American Medical Colleges merged independently collected data on a study group of 10,460 U.S. citizens who attended 359 medical schools in 75 foreign countries and who took their first ECFMG examination between 1978 and 1982. The study group was markedly heterogeneous: 21 percent were not U.S. citizens at birth, 32 percent did not have English as a native language, and 12 percent had two or more years

Health Care Workforce Immigration to the UK After Its European Union Membership Referendum.

IMPORTANCE: The UK relies heavily on internationally trained physicians and nurses. The 2016 UK European Union (EU) membership referendum (Brexit referendum) introduced uncertainty regarding future migration policy, with unclear implications for health care workforce supply and National Health Service (NHS) staffing. OBJECTIVE: To evaluate changes in immigration of internationally trained physicians and nurses to the UK after the Brexit referendum and to assess associated changes in NHS vacancies. DESIGN, SETTING, AND PARTICIPANTS: A controlled interrupted time-series study was conducted to

An opportunity to be grateful for? Exploring discourses about international medical graduates from India and Pakistan to the UK between 1960 and 1980.

INTRODUCTION: Following India and Pakistan gaining independence from British colonial rule, many doctors from these countries migrated to the UK and supported its fledgling National Health Service (NHS). Although this contribution is now widely celebrated, these doctors often faced hardship and hostility at the time and continue to face discrimination and racism in UK medical education. This study sought to examine discursive framings about Indian and Pakistani International Medical Graduates (IPIMGs) in the early period of their migration to the UK, between 1960 and 1980. METHODS: We assembl

Loss of the guide wire: a case report.

A case of a lost guide wire extending from the vena cava to the back of the neck after central venous catheterization is presented. A trainee inserted a central venous catheter via the left subclavian vein in a 40-year-old male patient after surgery, but did not notice that a guide wire was completely inserted in the vein. After 6 months, the lost guide wire was seen extending from the saphenous vein through the vena cava, right atrium, right ventricle, pulmonary artery and lung tissue to the back of neck. Although percutaneous catheterization of central veins is a routine technique, it is a p

Experiences of Overseas Trained Physical Therapists Working in Saudi Arabia: An Observational Study.

Background: Health professionals, including physical therapists (PTs), are known to migrate for better jobs, as well as for personal and professional development. However, this involves risks of maladjustment, discrimination, and exploitation. We conducted this study to investigate the experiences of overseas trained PTs in Saudi Arabia (SA) and their perceptions about physical therapy practice and problems regarding the profession in the country. Methods: A questionnaire and accompanying explanation of the study was sent to 175 members of the Saudi Physical Therapy Association (SPTA) working

Who considers alternative employment pathways among International Medical Graduates in Canada?

This study aims to examine which International Medical Graduates (IMGs) consider alternative employment pathways in Canada. IMGs are physicians whose country of practice differs from the country where they obtained their primary medical qualification. Due to the limitations and uncertainties of this career pathway, many IMGs opt for alternative career pathways (ACPs), which include non-physician roles that enable IMGs to apply their transferable knowledge, skills, and experience. This study aimed to analyze which IMGs choose alternative careers for job market integration in Canada. We carried
