A Thorn in the Flesh? Forensic Inpatients in General Psychiatry.

PURPOSE: To illuminate whether and how taking care of forensic inpatients is experienced as a burden among staff and managers in general psychiatry. DESIGN AND METHODS: Qualitative analytical strategies based on interviews and questionnaires. FINDINGS: The interplay between physical environment, bottlenecks, poor information exchange, lack of knowledge and competences, complex psychopathology, and a vague and therefore uncomfortable task of nursing leads to a focus on criminal offenses rather than mental disorders and an increased risk of brutalization and stigmatization in nursing practices

Commentary: the role of forensic psychiatry in the asylum process.

There has been a substantial growth of law school clinics and law firms doing pro bono work to aid individuals seeking asylum in the United States. At the same time, the federal government has been placing in detention more individuals who are here illegally. The number of immigration cases awaiting resolution has surpassed 200,000. The absence of a clear U.S. immigration policy contributes to this development. The conflicts in Iran and Afghanistan have resulted in an increase in the number of individuals seeking asylum. Psychiatric evaluations and psychological testing that provide organized

Stalking--a contemporary challenge for forensic and clinical psychiatry.

BACKGROUND: Stalking is causing pervasive and intense personal suffering and is an area of psychiatry that is currently overlooked. AIMS: To review demographic and clinical characteristics of stalkers as well as the psychological consequences for victims of stalking. METHOD: A Medline and PsycLit search was conducted on stalking, forensic psychiatry, personality disorders, de Clérambault syndrome and erotomania, with respect to the relevance of the articles selected for stalking. RESULTS: Stalkers are best thought of as a heterogeneous group whose behaviour can be motivated by different for

Ethical dilemmas in forensic psychiatry: two illustrative cases.

One approach to the analysis of ethical dilemmas in medical practice uses the "four principles plus scope" approach. These principles are: respect for autonomy, beneficence, non-maleficence and justice, along with concern for their scope of application. However, conflicts between the different principles are commonplace in psychiatric practice, especially in forensic psychiatry, where duties to patients often conflict with duties to third parties such as the public. This article seeks to highlight some of the specific ethical dilemmas encountered in forensic psychiatry: the excessive use of se

Forensic and medical legal issues in addiction psychiatry.

There is a broad range of forensic issues in addiction psychiatry. For many psychiatrists, what is most challenging about addressing these issues is their origin in the law, which is a system and a way of thinking that may feel foreign to the medical practitioner. To address forensic issues, addiction psychiatrists should learn and understand the specific legal questions that arise in each forensic context. They should become familiar with the relevant definitions, criteria, and legal requirements that apply in each specific area of their practice, rather than assume that clinical definitions

Biological factors and crime: implications for forensic psychiatry.

To summarize, we can say that (1) Criminal behavior, especially chronic criminal behavior, seems to be partly genetically predisposed; (2) An important task at this point is to attempt to determine the biological factors which predispose people to crime; and (3) We have related some tentative initial steps being taken in the study of the autonomic nervous system as one possible heritable, biological basis for the failure of normal social learning forces in inhibiting criminal behavior. Early in this paper we discussed the tenability of asserting criminal responsibility on individuals whose cri

The impact of COVID-19 on clinical forensic medicine and forensic psychiatry: A literature review.

Purpose:The COVID-19 pandemic prompted significant challenges in clinical forensic medicine and forensic psychiatry and emphasised the need for sustainable service delivery. Both fields faced difficulties in maintaining services due to COVID-19 precautions and limited referrals. This review examined the changes in forensic practices, aiming to understand their impact and learn from them to inform future strategies. Methods: A search was conducted across Ovid Medline, Ovid Embase, Ovid Emcare and PubMed, and webpages of governments and other organisations. Studies were included if they assessed

Using physiological biomarkers in forensic psychiatry: a scoping review.

UNLABELLED: Forensic psychiatry aims to reduce criminogenic risks and enhance societal safety. While effective at a macro level, crime recidivism rates among forensic patients remain substantial. This underlines the need for innovation, with growing interest in the use of physiological biomarkers. To identify the extent (size), range, (variety), and nature (characteristics) of evidence on the use of physiological biomarkers in forensic psychiatry, a systematic scoping review was conducted following JBI methodology and PRISMA-ScR guidance. Data on study characteristics and results were extracte

Evidence for Greater Forensic Education of all Psychiatry Residents.

Booth and his colleagues have made an important contribution to the emerging evidence base that shows education in forensic psychiatry topics can improve attitudes toward the field. Given the desinstitutionalization of those with severe mental illness from state psychiatric facilities and the incarceration of many individuals with severe mental illness in correctional facilities, the need to train many more psychiatrists with competence in correctional settings is clear. Simply training more forensic psychiatrists will not both meet the psychiatric needs of incarcerated patients and fulfill th

Borderline personality disorder and related traits in forensic psychiatry.

Persons with borderline personality disorder (BPD) and related traits appear in many forensic psychiatry settings. Their clinical hallmarks affecting judgment, insight, impulsivity, motivations, and regulation of emotions, as well as their frequently chaotic lives (internal and external), inaccurate perceptions, rationalizations, and comorbid syndromes can have a marked effect on many civil, criminal, and institutional (eg, corrections) issues. Individuals with BPD are overrepresented in civil, criminal, and child custody forensic situations. The character psychopathology of these individuals

Forensic Psychiatry versus the Varieties of Delusion-Like Belief.

The available categorical constructs within the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, do not allow forensic psychiatrists to distinguish easily between the varieties of delusion-like belief. This dilemma is especially challenging when seemingly delusional beliefs are shared online. Although the term "extreme overvalued belief" has been proposed to aid with such distinctions, its definition has conceptual problems, including diagnostic overlap with shared delusions, "extremism" that refers to behavior rather than belief, and the potential to be applied with prose

Psychiatric side effects of mefloquine: applications to forensic psychiatry.

Mefloquine (previously marketed in the United States as Lariam®) is an antimalarial medication with potent psychotropic potential. Severe psychiatric side effects due to mefloquine intoxication are well documented, including anxiety, panic attacks, paranoia, persecutory delusions, dissociative psychosis, and anterograde amnesia. Exposure to the drug has been associated with acts of violence and suicide. In this article, we discuss the history of mefloquine use and describe plausible mechanisms of its psychotropic action. Mefloquine intoxication has not yet been successfully advanced in legal p

Exploring the impact of trauma-adapted yoga in forensic psychiatry.

The specialized field of forensic psychiatry deals with the care of criminal offenders who suffer from severe mental disorders. As this field is positioned at the intersection of illness, crime, and security, it poses complex challenges. While high-quality clinical studies in forensic psychiatry settings are limited, recent investigations have suggested yoga as a complementary clinical tool within correctional environments. This report of a quasi-experimental study examines the impact of a 10-week trauma-adapted yoga intervention on mental health, antisocial and aggressive behaviors, pain perc

Delusion in general and forensic psychiatry--historical and contemporary aspects.

Delusion has always been a central topic for psychiatric research with regard to etiology and pathogenesis and to diagnosis, treatment, and forensic relevance. Throughout the history of psychiatry as a scientific discipline, there has been dissent on the issue of whether chronic delusion is a nosological entity of its own or just a specific type of another mental disorder, e.g. schizophrenia, mania, or personality disorder, and there already is a considerable literature on this. This article seeks to elucidate the central lines of thought that have governed the scientific debate on delusions a

A Thirty-Year Forensic Case Report of Chronic Psychosis: Systemic Implications for Italian Forensic Psychiatry.

Background: Italy's forensic psychiatric system has undergone major reforms over recent decades, shifting from custodial institutions to a right-based, community-oriented model. While internationally praised for its commitment to dignity, recovery, and social reintegration, this framework presupposes a degree of clinical recoverability that is not always achievable. Individuals with chronic, treatment-resistant disorders-particularly those who remain socially dangerous-pose complex challenges that strain the system. Methods: We report a thirty-year longitudinal case of G.D.M.: a man initially

AIM: To research whether the Brøset-Violence-Checklist (BVC), an instrument for short-term prediction of violence of psychiatric inpatients, can be used in forensic psychiatry and how the use of the BVC is experienced. METHOD: Twice every 24 hours at more or less fixed times, a BVC score was registered for all patients who stayed in a crisis department within a Forensic Psychiatric Cen

A critical ethnographic perspective on risk and dangerousness in forensic psychiatry.

In the Canadian forensic psychiatric context, the concepts of risk and dangerousness interact, intersect, and morph into the notion of significant threat to the safety of the public. Stemming from the results of a critical ethnography of the Ontario Review Board, this article unpacks the central role of forensic psychiatric nursing, as an example of a 'psych' discipline (e.g., psychiatry and psychology), in a system that is built to produce risky persons and to legitimize their detention and supervision. By using excerpt of interviews conducted with nurses, ethnographic observations of Review
