Fixation of a Proximal Humerus Fracture With an Intramedullary Nail.

Proximal humerus fractures (PHFs) are common injuries seen mostly in elderly patients. Although nonoperative management is acceptable for most patients, surgical fixation is often advocated for younger patients or active, elderly patients with reconstructable fractures. A variety of fixation methods exist: percutaneous pinning, open reduction and internal fixation with locking plates, and intramedullary nailing are viable options. Modern intramedullary nails serve as excellent options for the fixation of many operative PHFs. In this Supplemental Digital Content 1 (see video, http://links.lww.c

Intramedullary screw fixation for midshaft fractures of the clavicle.

Open intramedullary fixation of 37 fresh midshaft clavicular fractures in 35 patients was performed using a 6.5 partially threaded cancellous screw. Mean age was 38 years (range 18-65). The screw was inserted from the medial fragment after retrograde drilling of that fragment. Average follow-up period was 21 months (range 9-36). Radiological evidence of union was apparent in all cases within six to eight weeks after surgery (mean 7.8). Two cases had intraoperative failure of fixation, nine complained of subcutaneous prominence of the screw head, five experienced decreased sensation over the si

Percutaneous intramedullary fixation of metacarpal shaft fractures.

Twenty-two displaced metacarpal shaft fractures in 19 patients were stabilized with multiple intramedullary K-wires. These were inserted percutaneously through a small window in the base of the metacarpal and were buried in the bone. No external splintage was routinely used postoperatively and all patients were encouraged to mobilize their fingers immediately: formal physiotherapy was not usually required. All the fractures that we were able to follow-up united, but the K-wires bent at the fracture site in two instances, producing 20 degrees angular deformities. The buried K-wires had to be re

[The closed method of intramedullary fixation of forearm fractures].

The authors demonstrate a group of 27 patients subjected to intramedullary osteosynthesis of the forearm by means of Kirschner wires, using closed surgery. They prefer the described method in patients suffering from multiple injuries who have devastated soft parts of the forearm and in unstable fractures in children. The authors draw attention to the fact that they have achieved very good results by the simple surgical method they described.

Closed intramedullary fixation of humeral shaft fractures.

During a seven-year prospective study, 158 humeral shaft fractures in 157 patients were treated with closed intramedullary nailing. Only humeral fractures not responsive to closed reduction and immobilization and those in noncomplaint patients were eligible. No immobilization was used postoperatively. Seven patients were lost to follow-up. A total of 148 fractures healed; there were three nonunions. The average time to clinical union was 7.2 weeks. There were no infections or malunions. Eight of 11 preoperative and three postoperative radial nerve palsies were lesions in continuity and healed

An assessment of outcomes with intramedullary fixation of fractured ribs.

BACKGROUND: Surgical management of fractured ribs with internal fixation is an increasingly accepted therapy. Concurrently, specific rib fixation prostheses are being developed which should improve results and minimise hardware and rib/splint construct failures. The Synthes titanium intramedullary splint lends itself to difficult to access areas such as posterior rib fractures and fractures under the scapula. We analyse a case series of patients in whom this rib fixation prosthesis has been used. METHODS: Fifteen patients received 35 intramedullary splints. Follow up at 3 and 6 months was per

[Intramedullary fixation of pediatric bone shaft fractures].

Intramedullary stabilisation (Prévot-nailing) of shaft fractures in childhood allows full weight bearing without cast. Early fracture healing is seen by respecting soft tissue around fracture and by using distal or proximal approaches. Good indications of femoral shaft osteosynthesis are recommended in oblique- and transverse-fractures. Intramedullary stabilization seems to be a good alternative methode instead of casting calf-shaft-fractures. An instable fracture of forearm may be treated intramedullary to avoid often observed displacement ad axim and rotation. Humoral fractures primary have

Hip fractures following intramedullary nailing fixation for femoral fractures.

INTRODUCTION: Proximal peri-implant femoral fractures occur following intramedullary nailing (IMN) fixation for trochanteric, femoral shaft, and distal femoral fractures. However, analyses of secondary hip fractures (SHFs) using large clinical samples are lacking. Therefore, we aimed to report the incidence and clinical outcomes of SHF after nailing fixation (IMN or cephalomedullary nailing [CMN]) for overall femoral fractures. In addition, we focused on IMN for femoral shaft fractures and investigated the risk factors for SHF. METHODS: This multicenter, retrospective, cohort study included 2

Peri-implant fracture: a rare complication after intramedullary fixation of trochanteric femoral fracture.

INTRODUCTION: Trochanteric femoral fractures are among the most common operatively treated fractures. Intramedullary fixation has become the treatment of choice in many centers around the world. Nevertheless, the knowledge of rare complications of these fractures is limited. In this study, the incidence and treatment strategies for peri-implant fractures (PIF) were assessed. MATERIALS AND METHODS: A single-center retrospective cohort study was done on 987 consecutive operatively treated trochanteric fractures. PFNA cephalomedullary nail was used as a fixation method. All patients were followe

Supination Adduction Ankle Fracture Treated With Fibular Intramedullary Screw Fixation.

This abstract describes an open reduction and internal fixation technique used for a supination adduction ankle injury. This technique demonstrates medial plafond articular disimpaction with backfilling of the bony void with cancellous allograft. The medial malleolus was then fixed with a mini fragment plate in antiglide fashion with subchondral rafting screws. The lateral malleolus fracture was addressed with use of a retrograde screw rather than a fibular plate. This technique emphasizes the importance of both recognizing and addressing the anteromedial plafond impaction of supination adduct

Antegrade Intramedullary Fixation for Clavicular Shaft Fracture: A Technical Trick.

The standard open reduction and internal fixation technique with a plate and screws for a simple mid shaft clavicular fracture necessitates a relatively large incision and can also lead to variable amount of keloid scar formation. Historically, other techniques of more minimally invasive retrograde intramedullary fixation with the entry point posterolaterally on the shoulder have shown their own disadvantages and complications. We present a surgical technique of antegrade intramedullary fixation for mid shaft clavicular fractures and an illustrative case series.

Treatment of Jones fracture nonunion with isolated intramedullary screw fixation.

Although the treatment of acute Jones fractures is well described in published studies, the Jones fracture nonunion is more controversial with regard to treatment. Although nonoperative treatment is an option, surgery is the usual course. We conducted a retrospective case series of 7 patients (1 man and 6 women; age range 39 to 54 years), who were initially treated nonoperatively for acute Jones fractures. They went on to develop nonunions and were treated with intramedullary screw fixation. All patients had healed radiographically by 11 weeks postoperatively. One screw (14.29%) required remov

Proximal humeral fracture fixation: locking plate construct ± intramedullary fibular allograft.

BACKGROUND: Locking plate constructs for proximal humeral fractures can fail due to varus collapse, especially in osteoporotic bone with medial cortex comminution. Augmentation, using a fibular allograft as an intramedullary bone peg, may strengthen fixation preventing varus collapse. This study investigated the ability of the augmented locking plate construct to withstand repetitive varus stresses relative to the nonaugmented construct. MATERIALS AND METHODS: Proximal humeral fractures with medial comminution were simulated by performing wedge-shaped osteotomies at the surgical neck in cadav

Intramedullary screw fixation of ballistic metacarpal and phalanx fractures.

INTRODUCTION: Ballistic hand fractures present a complex problem for surgeons. Traditionally, these have been treated with K-wires, necessitating prolonged immobilization. While fixation of hand fractures using intramedullary (IM) screws has been shown to have excellent outcomes in blunt trauma, their use in ballistic fractures has not been well studied. Here, we report our early outcomes of fixation of ballistic hand fractures with IM screws. METHODS: Our retrospective study reviewed adult patients from March 2023 to August 2024 treated for ballistic hand fractures with IM screws. Demographi

Elastic Locking Intramedullary Nails Fixation in Trimalleolar Fractures.

OBJECTIVE: Since the 1960s, although open reduction and internal fixation for ankle fractures has been widely used, it is associated with complications such as wound dehiscence, infection, prominent hardware, and failure. Closed reduction and internal fixation, on the other hand, offers greater biomechanical strength, requires minimal incisions, and features low-profile hardware. Our study compares the efficacy of elastic locking intramedullary nails (ELIN) fixation featuring minimally invasive microenvironmental protection microstress shielding versus rigid internal fixation (RIF) for trimall

A new technique for intramedullary screw fixation of sternal fractures.

INTRODUCTION: Sternal fractures are uncommon but may result in significant morbidity when associated with respiratory compromise or severe pain. Conventional methods such as plate fixation are often invasive and technically challenging. METHODS: We retrospectively reviewed eight cases of transverse sternal fractures treated using an intramedullary fixation technique with cannulated cancellous screws (CCS). Preoperative computed tomography with 3D reconstruction was used for surgical planning. Reduction was achieved percutaneously or through a limited incision, followed by guidewire insertion
