Diagnosis and Management of Vertebral Compression Fractures.

Vertebral compression fractures (VCFs) are the most common complication of osteoporosis, affecting more than 700,000 Americans annually. Fracture risk increases with age, with four in 10 white women older than 50 years experiencing a hip, spine, or vertebral fracture in their lifetime. VCFs can lead to chronic pain, disfigurement, height loss, impaired activities of daily living, increased risk of pressure sores, pneumonia, and psychological distress. Patients with an acute VCF may report abrupt onset of back pain with position changes, coughing, sneezing, or lifting. Physical examination find

Surgical patterns in osteoporotic vertebral compression fractures.

PURPOSE: To report morphological patterns of osteoporotic vertebral compression fractures (OVCFs) presenting for surgery. To describe surgical options based on fracture pattern. To evaluate clinical and radiological outcome. METHODS: Forty consecutively operated OVCFs nonunion patients were retrospectively studied. We define four patterns of OVCFs that needed surgical intervention. Group 1 mini open vertebroplasty (N = 10) no neurologic deficits and kyphotic deformity, but with intravertebral instability and significant radiological spinal canal compromise. Group 2 with neurologic deficits (N

Evaluation and management of vertebral compression fractures.

Compression fractures affect many individuals worldwide. An estimated 1.5 million vertebral compression fractures occur every year in the US. They are common in elderly populations, and 25% of postmenopausal women are affected by a compression fracture during their lifetime. Although these fractures rarely require hospital admission, they have the potential to cause significant disability and morbidity, often causing incapacitating back pain for many months. This review provides information on the pathogenesis and pathophysiology of compression fractures, as well as clinical manifestations and

Osteoporotic vertebral compression fractures and vertebral augmentation.

Vertebral fractures account for ~27% of all osteoporotic fractures in both men and women. The economic burden is substantial and growing: osteoporosis is expected to affect 14 million people by the year 2020. There is substantial morbidity associated with osteoporotic vertebral compression fractures (VCFs) including decreased quality of life, reduced pulmonary function, and increased mortality. Relatively recent additions to the treatment armamentarium include vertebral augmentation using vertebroplasty and kyphoplasty. Numerous retrospective and case studies demonstrate short-term efficacy an

Compression plating of pediatric femoral shaft fractures.

The authors conducted a retrospective review of 60 children under the age of 16 years with femoral shaft fractures treated with compression plate fixation. All patients were followed to hardware removal. The 100% union rate and low complication rate after compression plate fixation of pediatric femur fractures compared favorably to the literature of other treatment options. Compression plate fixation is a safe and effective treatment in children with both isolated femoral shaft fractures and those associated with multiple injuries.

Metastatic compression fractures--vertebroplasty for pain control.

Quality of life issues remain at the forefront for individuals with life-threatening disease, such as metastatic cancer. The pain of metastatic bone cancer can severely hamper an individual's quality of life. Percutaneous vertebroplasty offers a minimally invasive way to reinforce bony elements, provide substantial improvement in pain control, allow for mobilization, and overall improve quality of life in these patients.

Percutaneous treatment of osteoporotic spinal compression fractures.

Osteoporotic spinal compression fractures are a significant health risk for elderly females, as well as other patient groups. These fractures are associated with an increased risk of mortality in comparison with age-matched controls. Recent techniques of minimally invasive surgical treatment show promising early results with respect to pain reduction and restoration of function, with low procedural complication rates.

Magnetic resonance imaging of vertebral compression fractures.

In this study, magnetic resonance imaging (MRI) was used to analyze the signal intensity and vascularity of compression fractures of vertebrae in 74 patients. The possibility of nonunion was assessed according to the specific image findings and clinical presentation. All patients had chronic back pain for more than 3 months and compression fractures of the vertebrae initially demonstrated by plain radiography. Pre-enhanced T1 and T2*-weighted images (*multiplaner gradient recall sequence) and postenhanced MRI were obtained. Images were divided into three categories according to the signal inte

Self-compression plates for orbital rim fractures.

The development and principles of self-compression plating, as well as a series of patients treated with this technique, are described. The use of such plates has specific advantages over other techniques for disorders encountered by the oculoplastic surgeon. The indications for plating were displaced trimalar fracture, displaced trimalar fracture with orbital rim fragment, and comminuted orbital rim fracture. Results were good; no complications have been observed to date. Comparison of compression plating to other methods for repair of orbital fractures is made. Self-compression plating with

Compression Screw Fixation for Pediatric Olecranon Fractures.

BACKGROUND: Pediatric olecranon fractures can be treated with several methods of fixation. Though postoperative outcomes of various fixation techniques, including cannulated intramedullary screws, have been described in adults, functional and radiographic outcomes of screw fixation in pediatric patients are unclear. In this study, we assessed clinical, radiographic, functional, and patient-reported outcomes of pediatric olecranon fractures treated with compression screw fixation. METHODS: We retrospectively identified 37 patients aged 16 years or younger with a total of 40 olecranon fractures

Diagnosis and treatment of vertebral compression fractures.

A healthy spine is an integral part of an individual's overall well-being. The spinal column's essential role in physiological and neurological function can be compromised when disease or trauma causes a vertebra to compress under the body's weight, producing a vertebral compression fracture. This is a common ailment among adults older than 65 years of age, especially for those with low bone mass or osteoporosis. This article describes vertebral compression fractures, with a special emphasis on medical imaging.

Headless compression screw fixation of scaphoid fractures.

Scaphoid fractures carry significant long-term morbidity and short-term socioeconomic difficulty in the young and active patient population in which they most commonly occur. While cast immobilization results in high rates of radiographic union in nondisplaced scaphoid fractures, internal fixation with headless compression screws has been recommended in cases of displaced fractures. Internal fixation has led to high rates of union in both nondisplaced and displaced fractures with the added benefits of earlier mobilization and return to work and sports. Multiple manufacturers are now offering "

Medical consequences of osteoporotic vertebral compression fractures.

Osteoporotic vertebral compression fractures are an increasingly common source of morbidity and mortality in the aging population. Previously, these fractures were assumed to be benign, self-limited entities with few, if any, significant sequelae. More recently, however, individual cohorts and population-wide analyses demonstrate high rates of chronic pain, functional decline, physiologic disorder, psychosocial dysfunction, and early mortality among patients with osteoporotic vertebral body compression fractures.

Compression fracture in postpartum osteoporosis.

Osteoporosis is mainly a problem in postmenopausal women. However, we had a case of postpartum compression fracture associated with osteoporosis. A 42-year-old multiparous woman had undergone tertiary cesarean section without complications 1 month before. She was breast-feeding her baby and had no other bone-related complication history. She did not exercise on a regular basis. She experienced back pain abruptly and was diagnosed with a recent compression fracture of T12, L1, and L3. We evaluated this rare case of postpartum osteoporosis and compression fracture.

Vertebroplasty for vertebral compression fracture.

The patients with intensive pain caused by the vertebra body fracture were treated by application of bone cement under local or general anesthesia, by means of diascopy through unilateral transpedicular approach. The intensity of pain was followed-up on the VAS scale before surgery, 24 hours after surgery and three months after surgery. The cement was applied in 55 patients (43 women, 12 men). The average age of the tests was 68.43 years (38 to 82 years). Fifty five procedures were implemented on 85 vertebrae, i.e. 32 metastatic fractures and 23 osteoporotic fractures. The procedures were appl

Acute compression fracture of the scaphoid.

A previously unreported radiological sign of a sclerotic margin to an acute displaced scaphoid fracture due to bone compression is reported. This should to be recognized because of the implied bone loss and hence risks of both mal-union and non-union, and because of the implication that volar compression fractures of the scaphoid may occur.
