Functional residual capacity in anesthetized children: normal values and values in children with cardiac anomalies.

To assess the increase in functional residual capacity (FRC) with growth, FRC was measured after induction of anesthesia in two groups of children. One group consisted of 74 children, 0.1-11.2 yr of age, without signs of cardiorespiratory disease (referred to here as "normal" children), and the other of 21 children, 0.2-6.9 yr of age, with cardiac malformations. Anesthesia was maintained with halothane in the normal children and with fentanyl, droperidol, and nitrous oxide in the children with cardiac anomalies. All patients were paralyzed, their tracheas intubated, and their lungs mechanicall

Lung function tests in neonates and infants with chronic lung disease of infancy: functional residual capacity.

This is the second paper in a review series that will summarize available data and discuss the potential role of lung function testing in infants and young children with acute neonatal respiratory disorders and chronic lung disease of infancy. The current paper addresses the expansive subject of measurements of lung volume using plethysmography and gas dilution/washout techniques. Following orientation of the reader to the subject area, we focus our comments on areas of inquiry proposed in the introductory paper to this series. The quality of the published literature is reviewed critically, an

Functional residual capacity and lung clearance index in infants treated for esophageal atresia and tracheoesophageal fistula.

BACKGROUND: Newborn babies with esophageal atresia/tracheoesophageal fistula (EA/TEF) are prone to respiratory tract disorders. Functional residual capacity (FRC) and lung clearance index (LCI) are commonly considered useful and sensitive tools to investigate lung function and early detecting airways diseases. The aim of the present study is to report the first series of EA/TEF infants prospectively evaluated for FRC and LCI. METHODS: Prospective observational cohort study of all patients treated for EA/TEF. Lung volume and ventilation inhomogeneity were measured by helium gas dilution techni

Functional residual capacity and static compliance during the first year in preterm infants treated with surfactant.

Individual lung development during the first year of life was studied in surfactant treated preterm infants with respiratory distress syndrome (RDS) and healthy controls, as well as in a group who subsequently developed chronic lung disease of the newborn (CLDN). Lung development was assessed from functional residual capacity (FRC) and compliance of the respiratory system (Crs). Twenty-one infants with RDS after preterm birth received surfactant treatment. Six of them developed CLDN. Eighteen preterm infants without RDS served as a control group. Lung function measurements were performed at te

Effects of Obesity and Sex on Supine Functional Residual Capacity and Expiratory Flow Limitation in Children.

INTRODUCTION: Childhood obesity is associated with a higher risk of sleep-disordered breathing, but our understanding of the underlying physiologic mechanisms is limited. This study investigated the effects of obesity on lung volumes, expiratory flow limitation (EFL), and respiratory symptoms in the supine position. METHODS: Ninety-four children (8-12 years old, n = 58 with obesity and n = 36 without obesity) underwent lung volume and EFL measurements in the seated and supine positions in this repeated-measures cross-sectional study. Children without obesity underwent an additional condition

Prevalence, predictors, and prognostic implications of residual impairment of functional capacity after transcatheter aortic valve implantation.

BACKGROUND: Patients with degenerative aortic stenosis (AS) referred for transcatheter aortic valve implantation (TAVI) typically have advanced cardiac and vascular adverse remodeling and multiple comorbidities and, therefore, might not recover a normal functional capacity after valve replacement. We sought to investigate the prevalence, the predictors, and the prognostic impact of residual impairment of functional capacity after TAVI. METHODS AND RESULTS: Out of 790 patients undergoing TAVI with impaired functional capacity (NYHA II-IV) at baseline, NYHA functional class improved in 592 (86.

Positive end-expiratory pressure enhances development of a functional residual capacity in preterm rabbits ventilated from birth.

The factors regulating lung aeration and the initiation of pulmonary gas exchange at birth are largely unknown, particularly in infants born very preterm. As hydrostatic pressure gradients may play a role, we have examined the effect of a positive end-expiratory pressure (PEEP) on the spatial and temporal pattern of lung aeration in preterm rabbit pups mechanically ventilated from birth using simultaneous phase-contrast X-ray imaging and plethysmography. Preterm rabbit pups were delivered by caesarean section at 28 days of gestational age, anesthetized, intubated, and placed within a water-fil

Effect of positive end expiratory pressure on functional residual capacity and compliance in surfactant-treated preterm infants.

UNLABELLED: Positive end expiratory pressure is routinely used when ventilating preterm infants. Elevation of PEEP increases lung volume, as does surfactant treatment. The purpose of this study was to investigate the effect of various levels of PEEP within the range of 0.2 to 0.4 kPa on lung volume, compliance and gas exchange. We measured functional residual capacity, compliance of the respiratory system and arterial blood gases in 20 infants (median birth weight 1240 g, range 660-1690 g; median gestational age 28 weeks, range 24-32 weeks; postnatal age 3-4 days). The infants were studied at

The influence of sleeping position on functional residual capacity and effective pulmonary blood flow in healthy neonates.

Variation in body position has been shown to affect respiratory function in adults and neonates with and without respiratory illness. At present it remains unclear why respiratory function should be affected by different body positions. We hypothesized that the effect of body weight on the relatively compliant chest wall of the newborn infant in the prone position would cause a reduction in functional residual capacity (FRC) and a compensatory improvement in ventilation/perfusion matching as measured by effective pulmonary blood flow. To evaluate this, a paired crossover study was performed on

Effect of increased functional residual capacity on the active range of thoracic axial rotation in healthy young men.

[Purpose] To understand the effect of increased functional residual capacity (FRC) on the active range of thoracic axial rotation (AROTAR) in healthy young men. [Subjects and Methods] Thirty-nine right-handed healthy male volunteers (age=20.1 ± 1.6 years) participated in this study. A standard plastic goniometer was used to measure the AROTAR in the supine position with both glenohumeral joints fully flexed. AROTAR was recorded at the resting expiration level (REL) and for 4 different FRC levels: 500, 1,000, 1,500, and 2,000 ml air inhaled at REL. FRC volumes and laterality were analyzed using

The effect of body position, sedation, and thoracic bandaging on functional residual capacity in healthy deep-chested dogs.

The objective of this study was to determine the effect of body position, chest wrap, and sedation on functional residual capacity (FRC) in 6 healthy dogs. Functional residual capacity was determined by helium dilution (re-breathing) whilst in different clinically relevant conditions. These conditions included the standing (sternal) and lateral positions in unsedated dogs and then again both standing and lateral following chest bandaging, and sedation with acepromazine, IV and butorphanol, IV. The mean FRC at each measurement point was determined, as was the change in FRC (delta FRC) from one

A novel physiological investigation of the functional residual capacity by the bias flow nitrogen washout technique in infants.

The dynamic functional residual capacity (FRC(dyn)), the lung volume most routinely measured in infants, is an unreliable volume landmark. In addition to the FRC(dyn), we measured the (passive) static FRC (FRC(st)) by inducing a brief post-hyperventilation apnea (PHA) in 33 healthy infants aged 7.4-127.2 weeks. A commercial system for nitrogen (N2) washout to measure FRC, and a custom made system to monitor and record flow and airway opening pressure signals in real-time were used in unison. Infants were manually hyperventilated to induce a PHA. After the last passive expiration, FRC(st) was e

Validation of a nitrogen washout system to measure functional residual capacity in premature infants with hyaline membrane disease.

A multiple-breath nitrogen washout system designed to measure lung volume in mechanically ventilated infants was validated by assessing three performance criteria: 1) accuracy of lung volume measurements in the presence of an endotracheal tube leak was assessed by comparing the measurements of functional residual capacity (FRC) in a mechanical lung model with and without airway leak; 2) in vivo accuracy was assessed in rabbits by comparing FRC measurements obtained by this system with measurements obtained by helium dilution; and 3) in vivo precision was assessed by analyzing measurements of F

Relationship between functional residual capacity and oxygen desaturation during short central apneic events during sleep in "late preterm" infants.

Apneic episodes are frequent in the preterm neonate and particularly in active sleep (AS), when functional residual capacity (FRC) can be decreased. Furthermore, FRC may be inversely correlated with the speed of blood-O(2)-desaturation. We evaluated the potential involvement of FRC in the mechanisms responsible for blood-O(2)-desaturation during short central apneic events (>3 s) in "late-preterm" infants and analyzed the specific influence of sleep state. Apneic events were scored in 29 neonates (postmenstrual age: 36.1 +/- 1.2 wk) during AS and quiet sleep (QS). FRC was measured during well-

Changes in functional residual capacity and lung mechanics during surgical repair of congenital heart diseases: effects of preoperative pulmonary hemodynamics.

BACKGROUND: To characterize the impact of lung volume changes in the lung function impairment after the surgical repair of congenital heart diseases, combined measurements of functional residual capacity, lung clearance index, and respiratory mechanics were performed in children with hypoperfused lungs (tetralogy of Fallot [TOF]) or with pulmonary hyperperfusion (ventricular septal defect [VSD]). METHODS: Lung volume and clearance were assessed by using a sulfur hexafluoride washout technique, and the mechanical properties of the respiratory system were assessed using a low-frequency oscillat

Automated sulfur hexafluoride washout functional residual capacity measurement system for any mode of mechanical ventilation as well as spontaneous respiration.

A new sulfur hexafluoride (SF6) washout functional residual capacity (FRC) measurement system has been developed which will work with any mode of mechanical ventilation, as well as with spontaneous respiration. This system was evaluated in three different human studies. In the first two studies, the accuracy of the system was compared with He dilution and body plethysmography in 12 spontaneously breathing normal volunteers and in 12 spontaneously breathing chronic obstructive pulmonary disease (COPD) patients. In the third study, the reproducibility and efficacy of using the system in the ICU

Functional residual capacity and compliance of the respiratory system after surfactant treatment in premature infants with severe respiratory distress syndrome.

UNLABELLED: To understand the mechanisms behind improved oxygenation after intratracheal surfactant instillation, the immediate and late effects on lung volume and compliance of the respiratory system (CRS) were analysed. Infants received modified porcine surfactant (Curosurf) or modified bovine surfactant (Alveofact). Measurements of functional residual capacity (FRC) and CRS were successfully performed in 90 ventilated preterm infants (birth weight 1264 +/- 435 g; gestational age 28.2 +/- 2.5 weeks) with severe respiratory distress syndrome. FRC and CRS were measured during mechanical ventil

Mechanical Power Indexed to Estimated Functional Residual Capacity as a Predictor of Mortality in Acute Respiratory Distress Syndrome: Post Hoc Analysis.

BACKGROUND AND AIMS: Mechanical power (MP) reflects the energy delivered by the ventilator and has been associated with mortality in acute respiratory distress syndrome (ARDS). We evaluated whether MP indexed to estimated functional residual capacity (MP/FRC) was associated with intensive care unit (ICU) mortality in ARDS. PATIENTS AND METHODS: This post hoc analysis included 158 adult patients with ARDS who required invasive mechanical ventilation between 2016 and 2022. Functional residual capacity was physiologically estimated from standard ventilatory variables. Mechanical power, MP/FRC, d

The effect of positive endexpiratory pressure, peak inspiratory pressure, and inspiratory time on functional residual capacity in mechanically ventilated preterm infants.

UNLABELLED: In mechanical ventilation of preterm infants, positive endexpiratory pressure (PEEP) is widely used to prevent alveolar collapse, maintain functional residual capacity (FRC) and improve oxygenation. Prolongation of inspiratory time (ti) and increase of peak inspiratory pressure (PIP) are also used for this purpose. We investigated the effect of variations of PEEP, PIP and ti on FRC in ten infants with hyaline membrane disease and onset of bronchopulmonary dysplasia (BPD, n = 7), pulmonary hypertension (n = 1), pulmonary hypoplasia (n = 1) or severe BPD (n = 1) (gestational age 24-3
