Exoskeleton-assisted Gait Training in Spinal Disease With Gait Disturbance.

OBJECTIVE: Gait impairment reduces a patient's quality of life. Exoskeletons and wearable robotics enable patients with gait disturbance to stand up and walk. An exoskeleton was developed for use in patients with stroke and spinal cord injuries. This study aimed to evaluate the effectiveness of overground exoskeleton-assisted gait training (OEGT) in spine diseases with gait disturbance. METHODS: This was a single-group preliminary study. Five participants with gait disorders because of root dysfunction accompanying spinal stenosis were included in this study. All participants underwent surgic

A body-worn gait analysis system for evaluating hemiplegic gait.

This paper describes a system for measuring the temporal parameters of hemiplegic gait. This system uses shoe insoles with sensors, acting as switches, placed under the heel, head of the first metatarsal, head of the fifth metatarsal and the big toe. This system is able to monitor gait for up to 10 min and can be used by the patient over any surface. Parameters for evaluating hemiplegic gait are defined, including scuffing during swing and the degree of inversion during stance.

The language of gait: interpreting emotional states through gait videos.

INTRODUCTION: Gait integrates sensorimotor and affective processes, serving both locomotor function and emotional expression. This embodiment framework has clinical relevance in neurodegenerative disorders and suggests potential for emotion-based modulation of gait. OBJECTIVES: This study developed and tested a range of emotion-specific gait videos to assess whether healthy individuals could correctly recognize different emotions, in order to identify standardized stimuli for future research on emotion recognition and embodied simulation in neurological patients. METHODS: We created a video

Gait profile score and gait variable scores in spina bifida.

Quantitative data assessment on the basis of three-dimensional gait analysis has been routinely used in the evaluation of pathological gait of children with cerebral palsy. However, a similar quantitative methodology has not been applied for spina bifida patients in whom atypical gait patterns are thought to correlate with various levels of neurological paralysis. The purpose of this study is to investigate the differences among gait patterns in spina bifida between different levels of neurological lesions using quantitative methods: Gait profile score (GPS) and gait variable scores (GVS), sco

Overground gait training using virtual reality aimed at gait symmetry.

This study investigated if training in a virtual reality (VR) environment that provides visual and audio biofeedback on foot placement can induce changes to spatial and temporal parameters of gait during overground walking. Eighteen healthy young adults walked for 23 min back and forth on an instrumented walkway in three different conditions: (i) real environment (RE), (ii) virtual environment (VE) with no biofeedback, and (iii) VE with biofeedback. Visual and audio biofeedback while stepping on virtual footprint targets appearing along a straight path encouraged participants to walk with an a

Circumscribed supplementary motor area injury with gait apraxia including freezing of gait and shuffling gait: a case report.

Clinical findings in cases of injury circumscribed with SMA is no consensus. We report the case of a 60-year-old male with circumscribed SMA injury who showed freezing of gait, and shuffling gait. Twenty-one days after onset, the patient showed difficulties with the left leg swing in gait initiation (freezing of gait). In steady-state gait, the stride of the left leg swing was short (shuffling gait). Thirty-four days after onset, this phenomenon was not observed during gait. Circumscribed SMA injury can cause gait apraxia, including freezing and shuffling gait, such as in extensive SMA injury

Predicting prosthetic gait and the effects of induced stiff-knee gait.

Prosthetic gait differs considerably from the unimpaired gait. Studying alterations in the gait patterns could help to understand different adaptation mechanisms adopted by these populations. This study investigated the effects of induced stiff-knee gait (SKG) on prosthetic and healthy gait patterns and the capabilities of predictive simulation. Self-selected speed gait of two participants was measured: one healthy subject and one knee disarticulation subject using a variable-damping microprocessor controlled knee prosthesis. Both performed unperturbed gait and gait with restricted knee flexio

Neurological gait assessment.

Gait disorders are a common feature of neurological disease. The gait examination is an essential part of the neurological clinical assessment, providing valuable clues to a myriad of causes. Understanding how to examine gait is not only essential for neurological diagnosis but also for treatment and prognosis. Here, we review aspects of the clinical history and examination of neurological gait to help guide gait disorder assessment. We focus particularly on how to differentiate between common gait abnormalities and highlight the characteristic features of the more prevalent neurological gait

Psychogenic gait disorders.

Psychogenic disorders of posture and gait are common and are the major manifestation in 8 to 10% of patients with psychogenic movement disorders. The colorful history of these disorders is reviewed. Anxiety and depression are the commonest psychological accompaniments of functional gait disorder in contemporary practice. The particular case of the cautious gait and its flip side, "fear of falling," are considered in more detail. Common presentations for somatoform disorders and malingering are also described. It is often possible to make this diagnosis based on recognition features and gait ob

Gait Speed and Decline in Gait Speed as Predictors of Incident Dementia.

BACKGROUND: Slow gait is common in dementia, but it remains unknown whether the slowing happens many years prior to dementia onset. We therefore examined the relationship between slow baseline gait speed (GS), change in GS, and the hazard of incident dementia in a community dwelling of elderly people. METHODS: A total of 3,663 participants dementia-free at baseline (mean age, 73.5 years) were followed up for 9 years from a prospective cohort (Three-City study, France) for incident dementia (all-cause, Alzheimer's disease, vascular dementia, and other causes). GS over 6 m was assessed 4 times

Instrumented Gait Analysis: A Tool in the Treatment of Spastic Gait Dysfunction.

Spasticity is a term commonly used to describe a collection of muscle overactivity patterns associated with the upper motor neuron syndrome including actual spasticity, clonus, dystonia, co-contraction, associated reactions, and flexor or extensor spasms. Gait dysfunction in the upper motor neuron syndrome can be due to a combination of paresis, impaired coordination and balance, and muscle overactivity and contracture. Treatment options include physiotherapy, assistive devices, orthotic devices, oral and intrathecal medications, intramuscular chemodenervation, neurolysis, and/or neuro-orthopa

The role of gait analysis in treating gait abnormalities in cerebral palsy.

Individuals with cerebral palsy (CP) cannot take a normal activity like walking for granted. CP is the most common pediatric neurologic disorder, with an incidence of 3.6 per 1000 live births. The current trend in the treatment of individuals with CP is to perform a thorough evaluation including a complete patient history from birth to present, a comprehensive physical examination, appropriate radiographs, consultation with other medical specialists, and analysis of gait.

The Gait Deviation Index: a new comprehensive index of gait pathology.

This article describes a new multivariate measure of overall gait pathology called the Gait Deviation Index (GDI). The first step in developing the GDI was to use kinematic data from a large number of walking strides to derive a set of mutually independent joint rotation patterns that efficiently describe gait. These patterns are called gait features. Linear combinations of the first 15 gait features produced a 98% faithful reconstruction of both the data from which they were derived and 1000 validation strides not used in the derivation. The GDI was then defined as a scaled distance between t

Temporal and spatial parameters of gait in children. II: Pathological gait.

Temporal and spatial parameters of gait were measured in 72 children with anomalous walking patterns as a result of neuromuscular impairment, 21 with childhood hemiplegia, 27 with congenital paraplegia and 24 with miscellaneous neuromotor disorders. Measurements were compared with those for a group of normal children. In all three groups, step length, average maximum foot-velocity and walking speed were reduced, double support time was increased, while cadence deviated from normal in both directions. Hemiplegic children varied considerably in their measured side-to-side gait asymmetries, sugge
