Pyoderma Gangrenosum: A Review for the Gastroenterologist.

Pyoderma gangrenosum (PG) is a rare ulcerative skin disease of unknown etiology often associated with systemic inflammatory conditions, most commonly inflammatory bowel disease (IBD). The most common presentation of PG is spontaneous rapid formation of an extremely painful ulcer on the extremities, associated with significant morbidity and mortality. Rarely, PG can also occur as a chronic peristomal complication or as an acute postoperative complication of a surgical wound. The clinical course is unpredictable; it may not correlate with IBD activity and may even precede a diagnosis of IBD. Pyo

Ipilimumab-induced toxicities and the gastroenterologist.

Ipilimumab has been shown to improve overall survival in patients with advanced melanoma. Ipilimumab acts through immune-modulation, and is recognized to cause potentially severe immune-related adverse events (irAEs) including dermatitis, colitis, thyroiditis, hypophysitis, and hepatitis. The acceptance of ipilimumab as a treatment for metastatic melanoma means patients will continue to be treated with this agent and gastroenterologists will be increasingly called upon to assist in managing severe autoimmune-related hepatitis and colitis. To date, the recommendations for managing irAEs seconda

A review of liver transplantation for gastroenterologists.

The 1980s will be remembered as the era of transplantation. During this period both liver and heart transplantation have their places as established therapeutic procedures for selected patients with end stage organ failure. As increasing numbers of patients are transplanted, others practicing physicians, especially gastroenterologists and cardiologists, will need to become familiar with all aspects in the care of patients that they have referred for transplantation. Only through close cooperation between transplant surgeons and this new breed of "transplant internists" will all suitable patien

Paediatric endoscopy performed by adult-service gastroenterologists.

BACKGROUND: Children often travel from district hospitals to teaching centres for endoscopic procedures by paediatric gastroenterologists. A 10-year district hospital experience of 'adult-service' gastroenterologists endoscoping children is reported with the aim of quantifying the workload, indications, sedation/anaesthesia practices, findings and safety of paediatric endoscopy performed by adult-service gastroenterologists. METHODS: Data on endoscopic procedures in patients younger than 16 years of age between 1997 and 2006 were obtained from hospital case-notes and computerized endoscopy/hi

Esophageal motility disorders: a gastroenterologists' perspective for radiologists.

Esophageal motility disorders are commonly encountered in the outpatient setting during the evaluation of difficulty swallowing. They typically present with symptoms of dysphagia to solids or liquids, non-cardiac chest pain, or regurgitation. Practitioners rely on both invasive and non-invasive testing to evaluate these complaints, often utilizing endoscopy, fluoroscopic evaluations, and functional esophageal motility testing to characterize symptoms into formal motility disorders, when able. Many of these tests complement each other and more than one is often needed to fully characterize a pa

Anal Squamous Intraepithelial Lesions: A Primer for Gastroenterologists.

PURPOSE OF REVIEW: The purpose of this review is to provide gastroenterologists a basic understanding of what anal squamous intraepithelial lesions (ASILs) are, how they develop, who are the high-risk populations, who and how to screen, and management of these lesions. RECENT FINDINGS: Human papilloma virus (HPV) infection is the most common cause of squamous cell carcinoma of the anus. Anal intraepithelial neoplasia is a precursor to the development of anal squamous cell carcinoma, and screening can lead to early detection and cure. Despite the availability of HPV vaccination for primary pre

Assessing for Eating Disorders: A Primer for Gastroenterologists.

Eating disorders involve irregularities in eating behavior that may cause gastrointestinal (GI) symptoms. Consequently, many patients with eating disorders seek gastroenterological healthcare at some point in their illness, with many seeking this care even before they seek treatment for and/or diagnosed with their eating disorder. As such, the gastroenterology provider is in a unique position to identify, manage, and facilitate treatment for an eating disorder early in the course of the illness. Although assessing eating disorders is already a difficult task, the identification of eating disor

Nutrition in Pancreatic Diseases: A Roadmap for the Gastroenterologist.

While common pancreatic diseases, such as acute pancreatitis (AP), chronic pancreatitis (CP), and pancreatic cancer (PC), may greatly impact the normal pancreatic physiology and contribute to malnutrition, the adequate nutritional approach when those conditions are present significantly influences patients' prognosis. In patients with AP, the goals of nutritional care are to prevent malnutrition, correct a negative nitrogen balance, reduce inflammation, and improve outcomes such as local and systemic complications and mortality. Malnutrition in patients with CP is common but often a late manif

Chronic Abdominal Pain: Gastroenterologist Approach.

BACKGROUND: Abdominal pain is a common symptom of gastroenterology examination. Chronic abdominal pain is present for >3 months. SUMMARY: Gastroesophageal reflux disease (GERD) is one of the most common gastrointestinal diseases encountered by both gastroenterologists and general practitioners. GERD is usually a chronic disease presented with a set of symptoms including heartburn and/or regurgitation, and less commonly epigastric pain. Epigastric pain syndrome is characterized by the following symptoms: epigastric pain and/or burning. It does not necessarily occur after meal ingestion, may oc

Rectal Cancer in 2018: A Primer for the Gastroenterologist.

The rectum has distinctive anatomic and physiologic features, which increase the risk of local spread and recurrence among rectal cancers as compared to colon cancers. Essential to the management of rectal cancers is accurate endoscopic localization as well as preoperative imaging assessment of local and distant disease. Successful oncologic care is multidisciplinary including input from Gastroenterologists, Surgeons, Medical and Radiation Oncologists, Radiologists, and Pathologists. Extensive planning of curative intent is mandatory as failures of upfront treatment present great long‐term dif

A perspective on metabolic surgery from a gastroenterologist.

Type 2 diabetes (T2D) and obesity are important public health problems. The global prevalence of diabetes mellitus is 8.8%. Interventional diabetology and obesitology have been recently advocated as treatment options for T2D and obesity. The roles of metabolic surgery such as Roux-en-Y gastric bypass, sleeve gastrectomy, gastric banding, and biliopancreatic diversion are focused. Different types of metabolic surgeries have different glucose-lowering and weight loss effects. Endoscopic treatments include the intra-gastric balloon (to restrict the gastric volume) and duodenal-jejunal bypass line

The Gastroenterologist's Role in Management of Perianal Fistula.

Perianal fistula occurs frequently in the Crohn disease population. Therapy for fistulas has changed through the years from primarily surgical management to multidisciplinary management among gastroenterologists, radiologists, and surgeons. Gastroenterologists play a role in assisting with diagnosis through endoscopic ultrasound and assessment of luminal disease activity, providing medical therapy including biologic therapy and antibiotics, and coordinating the multidisciplinary care with surgical and radiologic colleagues.

Cystic fibrosis from the gastroenterologist's perspective.

Cystic fibrosis is a life-limiting, recessive disease caused by mutations in the cystic fibrosis transmembrane conductance regulator (CFTR) gene. Increased survival outcomes and the multisystem nature of the disease, including the involvement of hepatobiliary and gastrointestinal tracts, now require the need for more extensive knowledge and expertise in cystic fibrosis among gastroenterologists. Manifestations are either a direct consequence of the primary defect in cystic fibrosis or a secondary complication of the disease or therapy. Adult patients with cystic fibrosis also have an increased

Gastroenterologist's Guide to Assess for Alcohol Use.

PURPOSE: Alcohol-associated liver disease (ALD) carries significant health care burden as it is the leading indication for hospitalization and liver transplantation among patients with chronic liver disease. Key challenge with ALD remains a delay in its diagnosis during the asymptomatic stage warranting the need for improved screening for heavy alcohol use. Further, given the impact of alcohol use in steatotic liver disease, accurate assessment of alcohol use is paramount. The aim of this study is to review the challenges associated with alcohol assessment, importance of subjective assessment,

Parkinson Disease and the Gut: A Primer for Gastroenterologists.

Parkinson disease (PD) is a chronic, progressive movement disorder and the fastest growing neurological condition worldwide, affecting over 6 million individuals. In 2017, the economic burden of PD in the United States alone reached $52 billion. Gastrointestinal symptoms and dysfunction such as constipation, gastroparesis, and dysphagia are common in PD, are difficult to manage, and negatively affect quality of life. In addition, constipation often precedes motor symptoms by decades, perhaps suggesting that a disrupted bidirectional microbiota-gut-brain axis is present early in PD. Data from m

Demystifying Meckel's diverticulum - a guide for the gastroenterologist.

PURPOSE OF REVIEW: Meckel's diverticulum (MD) is a common congenital ileal diverticulum. Whilst mostly asymptomatic, 4-9% develop complications, such as small bowel obstruction, diverticulitis or bleeding. In 1933, Charles Mayo wrote that MD is 'frequently suspected, often looked for and seldom found', and it continues to pose a diagnostic challenge today. With advancements in small bowel imaging and endoscopy, this review outlines the gastroenterologist's approach to MD. RECENT FINDINGS: There are a number of strategies for diagnosing MD. Meckel's scan has a sensitivity of 80-92% in children
