EUS-guided gastroenterostomy in management of benign gastric outlet obstruction.

BACKGROUND AND STUDY AIMS: Endoscopic ultrasound-guided gastroenterostomy (EUS-GE) in malignant gastric outlet obstruction (GOO) appears to be promising; however, its role in benign GOO is unclear. The aim of this study was to ascertain the clinical efficacy and safety of EUS-GE in benign GOO. PATIENTS AND METHODS: This was an international retrospective series involving 5 tertiary centers. Consecutive patients who underwent EUS-GE between 1/2013 - 10/2016 for benign GOO were included. The primary endpoint was the rate of clinical success defined as ability to tolerate oral intake without vom

Endoscopic ultrasound-guided gastroenterostomy: Are we ready for prime time?

Interventional endoscopic ultrasonography (EUS) is currently becoming the less invasive therapeutic approach for the drainage of pancreatic fluid collections, of acute cholecystitis in patients unfit for surgery and for biliary drainage after failed endoscopic retrograde cholangiopancreatography. In addition, EUS-guided gastroenterostomy (EUS-GE) has recently emerged as a feasible procedure to treat patients with gastric outlet obstruction, as an alternative to surgery or to standard endoscopy when endoscopic stent placement is not possible. Prior animal studies have shown that the procedure i

A new technique of gastroenterostomy for palliative treatment of pancreatic head carcinoma.

BACKGROUND/AIMS: Most of the patients with pancreatic cancer can be only palliatively treated. Although 30% of the patients submitted to palliative biliary bypass will require further treatment for duodenal obstruction, prophylactic gastric bypass still remains a controversial issue in the management of unresectable pancreatic head carcinoma. The main disadvantage of current techniques of gastrojejunostomy is postoperative vomiting due to impaired gastric motility and circulus vitiosus through the nonobstructed duodenum. A new technique of prophylactic gastrojejunostomy is described herein as

Novel designed balloon specialized for EUS-guided gastroenterostomy: A pilot study.

BACKGROUND AND OBJECTIVES: We recently developed a balloon-assisted device for EUS-guided gastroenterostomy (EUS-GE) to enhance the safety and convenience of the procedure. This pilot study was conducted to evaluate the safety and feasibility of this device. METHODS: A retrospective analysis of data of patients who underwent EUS-GE using this balloon-assisted device at our institution from March 2024 to July 2024 was conducted. The primary end point was the procedure time, and the secondary end points were the volume of water injection, technical success rate, clinical success rate, and adver

Fluoroscopically guided percutaneous gastrostomy and gastroenterostomy: analysis of 158 consecutive cases.

We reviewed our experience with 158 consecutive patients who underwent either percutaneous gastrostomy or percutaneous gastroenterostomy during a 2-year period. The catheters used included Foley catheters (36), Cope-type gastric catheters (86), or Carey-Alzate-Coons gastrojejunostomy catheters (36). Gastrojejunostomy tubes were placed in patients with gastroesophageal reflux or aspiration, gastric atony, or partial gastric obstruction. Ninety percent of the tubes were placed for feeding purposes. The technical success rate was 100%. Thirty-day follow-up was obtained in 89%. Thirty-day mortalit

Endoscopic ultrasound-guided gastroenterostomy, with focus on technique and practical tips.

Gastric outlet obstruction (GOO) is a condition characterized by a mechanical obstruction of the stomach or duodenum, caused by either benign or malignant disease. Traditionally, surgical gastrojejunostomy (SGJ) has been the standard treatment for malignant GOO and endoscopic stenting (ES) offers a less invasive option, but it often requires repeat interventions. Recently, endoscopic ultrasound (EUS)-guided gastroenterostomy (EUS-GE), an innovative technique, has been applied as an alternative to SGJ and ES for GOO patients. Direct EUS-GE, device-associated EUS-GE, and EUS-guided double balloo

Role of Endoscopic Ultrasound-guided Gastroenterostomy for Benign Gastric Outlet Obstruction.

Benign gastric outlet obstruction (GOO) often results from intrinsic conditions like peptic strictures, caustic-induced stricture, and surgical anastomoses, and extrinsic conditions like pancreatitis, hematoma, and superior mesenteric artery syndrome. While traditional management involved surgery or endoscopic balloon dilation, endoscopic ultrasound-guided gastroenterostomy (EUS-GE) has emerged as a minimally invasive alternative using lumen-apposing metal stents (LAMS). We aimed to summarize the currently available literature on EUS-GE in treating benign GOO. EUS-GE demonstrates high technica

Transgastric endoscopic gastroenterostomy using a partially covered occluder: a canine feasibility study.

BACKGROUND AND STUDY AIMS: The use of natural orifice transluminal endoscopic surgery (NOTES) for gastroenterostomy has been previously reported, but it remains technically challenging and additional assistance is often needed. The aim of this study was to develop and evaluate a novel method for the creation of a gastroenterostomy using NOTES with an occluder. METHODS: Transgastric endoscopic gastroenterostomy was performed in 12 healthy female dogs using a therapeutic upper gastrointestinal endoscope and a partially covered occluder. The occluder was removed with a snare 1 week later. The pat

Endoscopic or surgical gastroenterostomy for malignant gastric outlet obstruction: a randomised trial.

BACKGROUND: Although surgical gastrojejunostomy (SGJ) is the standard method for palliation of gastric outlet obstruction (GOO), an endoscopic method-endoscopic ultrasound-guided gastroenterostomy (EUS-GE)-has been proposed as a novel, less invasive approach. OBJECTIVE: We compared both methods to determine whether clinical outcomes for EUS-GE are superior to surgery. DESIGN: We conducted a multicentre, randomised superiority trial of patients with malignant GOO to receive either EUS-GE or SGJ. Primary endpoint was composite measure, consisting of Gastric Outlet Obstruction Scoring System (G

Percutaneous endoscopic gastroenterostomy and jejunal extension for gastric stasis in pancreatic carcinoma.

Gastric stasis in pancreatic cancer without mechanical obstruction is distressing and difficult to manage. We describe two patients who were treated by percutaneous endoscopic gastroenterostomy (PEG) combined with a jejunal extension. Both patients had pancreatic cancer and gastric stasis, with recurrent vomiting and no evidence of distal duodenal obstruction. They were unresponsive to high-dose prokinetic agents. In both cases a Bower-PEG feeding tube with jejunal extension was inserted endoscopically, with clinical improvement. The technique has the advantages of permitting enteral feeding a

Standardizing Success and Troubleshooting in EUS-Guided Gastroenterostomy: An International Technical Review (With Videos).

EUS-guided gastroenterostomy (EUS-GE) is rapidly emerging as a pivotal procedure in the management of gastric outlet obstruction due to its advantages over historical comparators such as surgical gastroenterostomy and endoscopic placement of gastroduodenal stents. The ability to create a stable surgical-range connection between 2 lumens, distant from the tumor, with the minimally invasive nature of an endoscopic procedure, offers high clinical efficacy, acceptable safety, and low recurrence rates. However, widespread adoption is impeded by the steep learning curve and lack of standardized meth

Retrievable puncture anchor traction method for endoscopic ultrasound-guided gastroenterostomy: A porcine study.

BACKGROUND: Endoscopic ultrasound-guided gastroenterostomy (EUS-GE) is an alternative method for the surgical treatment of gastric outlet obstruction, but it is regarded as a challenging technique for endoscopists as the bowel is highly mobile and can tent away. Thus, the technique requires superb skill. In order to improve EUS-GE, we have developed a retrievable puncture anchor traction (RPAT) device for EUS-GE to address the issue of bowel tenting. AIM: To evaluate the feasibility of RPAT-assisted EUS-GE using an animal model. METHODS: Six Bama mini pigs each weighing between 15 and 20 kg
