Enteroviruses in gastrointestinal diseases.

Gastrointestinal diseases including diarrhoea constitute a major cause of morbidity and mortality in infants and young children especially in developing countries. Worldwide deaths among all ages due to diarrhoea during 2015 were estimated to be about 1.31 million, diarrhoeal deaths in children below 5 years of age being 499 000. Rotavirus accounted for about 200 000 deaths. Although diarrhoeal deaths decreased significantly during the last two decades, they still represent the third largest cause of infantile deaths. Several bacterial, viral, parasitic, fungal and non-infectious diarrhoea cau

Association Between Oral Microbiota and Gastrointestinal/Extra-Gastrointestinal Diseases.

Dental clinicians and researchers have recently recommended oral microbial examinations to more accurately diagnose and treat oral diseases, including periodontitis and dental caries. Theoretical and experimental evidence suggests that oral microbiota may also be associated with non-oral diseases, such as gastrointestinal and extra-gastrointestinal diseases. This review highlights studies demonstrating microbial alterations in the oral cavity associated with malignant tumors including gastric, colorectal, esophageal, and lung cancers, implying that these alterations may serve as early indicato

Diamine Oxidase and Gastrointestinal Diseases.

Histamine plays an essential role in gastrointestinal physiology and immune control, while its balance in the intestinal environment is largely maintained through degradation by diamine oxidase (DAO), a copper-dependent enzyme mainly produced by differentiated enterocytes. Impaired DAO activity reduces the intestinal barrier to luminal histamine, leading to increased local and systemic exposure and contributing to altered motility, permeability, visceral sensitivity, and immune activation. Accumulating research suggests that disturbances in diamine oxidase regulation contribute to the developm

Obesity and Upper Gastrointestinal Diseases.

Obesity increases gastroesophageal reflux disease through several factors. As a result, Barrett's esophagus, esophageal adenocarcinoma, and gastroesophageal junctional gastric cancer are increasing. Existing studies usually defined obesity by body mass index and analyzed the correlation. Recently, more studies have shown that central obesity is a more important variable in upper gastrointestinal diseases related to gastroesophageal reflux. Studies have reported that weight loss is effective in reducing gastroesophageal reflux symptoms. Obesity also affects functional gastrointestinal diseases.

Biologics in eosinophilic gastrointestinal diseases.

Eosinophilic gastrointestinal diseases are a constellation of conditions categorized by the location of eosinophilic infiltration in the gastrointestinal tract. Symptoms vary based on location of eosinophils and age of the patient. There are no approved medications at the current time with individuals using off-label steroids or dietary therapy. Translational research has identified potential pathways to target in the treatment of eosinophilic esophagitis (EoE), gastritis (EoG), and enteritis (EoN), including type 2 pathways, mast cells, and eosinophils. Preliminary studies found cendakimab (a

Eating Disorders and Gastrointestinal Diseases.

Eating disorders (ED) are frequently associated with a wide range of psychiatric or somatic comorbidities. The most relevant ED are anorexia nervosa (AN), bulimia nervosa (BN), and binge eating disorders (BED). Patients with ED exhibit both upper and lower gastrointestinal (GI) symptoms. Evidence of alterations throughout the GI tract in ED will be analyzed given the role of the GI tract in food intake and its regulation. It remains a matter of debate whether GI disorders are inherent manifestations of ED or the results of malnutrition occurring from ED. Moreover, recent clinical studies have

Antibacterial peptides and gastrointestinal diseases.

Defensins and cathelicidins are small cationic peptides produced by neutrophils and epithelial cells. They are highly expressed during infection. The role of constitutive and inducible antibacterial peptides has been extensively studied over the recent years; especially in the gastrointestinal (GI) tract, where the balance between the luminal bacteria and antibacterial peptides is crucial in the maintenance of a healthy GI tract. There are reports showing that the expressions of defensins and cathelicidins in the gut are dysregulated in various disease states. They could participate in the dev

[Biological treatment of gastrointestinal diseases].

Biological methods are widely used in the treatment of intestinal inflammation (II). The TNFalpha inhibitor infliximab can quickly correct recurrence, provide long-term remission and reduce hormone need in many patients with hormone-resistant and hormone-dependent forms of Crohn's disease (CD) and ulcerative colitis (UC). However, almost half of II patients fail anticytokine therapy. Some clinical trials of biological medicines were stopped because of toxicity and complications. One of promising approaches to II treatment is now transplantation of mesenchymal stromal cells (MSC). The trial wit

[Chronic diseases after gastrointestinal surgery].

Gastrointestinal surgery may not only lead to early postoperative complications but also chronic consequences. These have therapeutic implications for affected patients. The kind and extent of surgical intervention determines the spectrum of postsurgical phenomena which may occur. These chronic consequences are due to changes in gastrointestinal anatomy, the synchronization of digestive processes, or the ability to digest and absorb food. In case of transplantation surgery, adverse effects of immunosuppression have to be considered. Sometimes, chronic consequences of surgical procedures are di

Nutrient antioxidants in gastrointestinal diseases.

Oxidative stress appears to play a role in the pathogenesis of a number of gastrointestinal disease states, including pancreatitis; gastric and duodenal ulcer disease; IBD; gastric, esophageal, and colon cancers; and hepatic injury secondary to alcohol, metal storage disorders, hepatitis, and ischemia/reperfusion injury. The nutritional antioxidants are attractive potential therapeutic and chemopreventive agents because they are inexpensive and have a relatively low toxicity profile. A word of caution should be noted: Some antioxidants, such as vitamin C, can be prooxidant under certain condit

[Skin symptoms in gastrointestinal diseases].

Diseases of the skin and the gastrointestinal tract may occur together. It is important to examine the skin of everyone showing a gastrointestinal problem. Gastrointestinal signs and symptoms in dermatologic diseases may occur with dysphagia, abdominal pain, gastrointestinal bleeding and diarrhea with or without malabsorption. In general the cause is found in a genetic disorder, or it is infectious, drug-induced, inflammatory or related to a malignant disorder. Polyposis are hamartomatous tumors or result as an inflammatory reaction. All these syndromes may present with cutaneous lesions. As m

Food allergy and gastrointestinal diseases.

Several gastrointestinal diseases and symptoms have been attributed to food intolerance, but in only a few cases has the participation of immune mechanisms been confirmed. Acute or chronic gastroenteritis is commonly due to allergic reactions to food proteins, inflammatory diseases and infections, or the presence of immunological defects may induce secondarily a food allergic disorder. This review will focus on the immunopathogenetical importance of the mucosal and systemic defenses, and the absorption of food proteins by the gut.

[Ultrasonic diagnosis of gastrointestinal diseases].

The authors describe their experiences with the ultrasound tomography (compound scan and real-time scan) on the basis of 2,360 own examinations. This examination method was particularly suitable in cholelithiases, in concrements of the choledochus, in diseases of the pancreas above all in different forms of pancreatitis and pancreatic tumours and in abdominal lymphomas. The authors emphasize the advantages of the non-invasive method, however, they recommend to valuate the ultrasonographic findings always taking into consideration all other results of clinical examinations.

[Infusion therapy in gastrointestinal diseases].

In terms of a short review indications and applications of infusion therapy in gastroenterology (concerning vomiting, fistulas, diarrhea, ileus and peritonitis) are discussed. It is pointed out that in cases of water and electrolyte deficiency a rigid regimen is not reasonable. If possible a balance should be obtained. Parenteral nutrition is applied in obstructions of the upper gastrointestinal tract and in maldigestion and malabsorption.
