INTRODUCTION: The number needed to treat (NNT) for all-cause medication discontinuation in large, industry-sponsored, non-randomized, observational studies conducted across world geographies was compared with NNTs from CATIE, an 18-month, NIMH-sponsored, randomized study.
METHODS: NNTs (with 95% confidence intervals) were calculated using data from 3 large Lilly-sponsored, non-randomized, observational studies (EU-SOHO, IC-SOHO, and US-SCAP, n=20 957). Group differences at medication initiation were adjusted by Cox regression modeling. These NNTs were compared with published NNTs for CATIE (p

A man in his early 20s with kidney biopsy-confirmed focal segmental glomerulosclerosis (FSGS) was admitted with one month of nausea and vomiting, intermittent episodes of confusion, shortness of breath, and dysuria. He reported that many people from his native village in Central America, where he harvested sugarcane as a child, have died from kidney disease, including his father and cousin. He believed the source of disease to be agrochemicals found in the village's water supply. Although FSGS would be a rare manifestation, the patient's risk factors strongly suggested chronic kidney disease o

Work on the geography of health has been undertaken in association with the Centro de Investigaciones Ecologicas del Sureste, Chiapas State, Mexico. An example is given from a study of relationships between housing and Chagas' disease in the village of Agua Azul in Chiapas. Prospects for future work in medical geography might involve further studies of Chagas' disease and onchocerciasis, and of the health of refugees and of populations involved in the exploitation of petroleum reserves.

We apply and extend Philo (2000)׳s arguments about Foucault׳s Birth of the Clinic as an inspiration for health geography and the study of governance of gays. Philo identified three spaces through which he argued disease is framed: disease tabled, embodied, and institutionalized. These focus attention on the spatialities through which the medical gaze is power-laden. We adopt this framework empirically through an historical geography of an urban public health system engaged with sexually transmitted infections (STIs) in the "homosexual" population of Seattle, Washington in the 1970s. It reveals

Spinal cord injury (SCI) remains a major cause of disability worldwide, with distinct epidemiologic patterns between traumatic (TSCI) and nontraumatic SCI (NTSCI). Recent demographic shifts, particularly population aging, have led to a growing proportion of NTSCI, largely driven by degenerative and neoplastic conditions. Regional differences are substantial, with low-income and middle-income countries still facing a high burden of trauma-related injuries. Early diagnosis and timely surgical intervention, are critical for improving outcomes. In parallel, long-term care models and prevention str

Human immunodeficiency virus (HIV) infection alters gut microbiota composition and function, but the impact of geography and antiretroviral therapy remains unclear. Here we determined gut microbiome alterations linked to HIV infection and antiretroviral treatment in 327 individuals with HIV and 260 control participants in cohorts from Uganda, Botswana and the USA via faecal metagenomics. We found that while HIV-associated taxonomic differences were mostly site specific, changes in microbial functional pathways were broadly consistent across the cohorts and exacerbated in individuals with acqui

This article traces the travels of the Scottish physician Archibald Smith through the Peruvian Andes between the 1820s and 1860s. Despite his prominent role in the nineteenth-century Peruvian medical scene, almost nothing has been written on Archibald Smith. By exploring Smith's medical activities, publications, and debates, this article intends to uncover unexplored areas of Peruvian medical history, such as the animosity between local and foreign physicians during the post-Independence war era and the important role played by medical geography as a scientific discipline for redefining ethnic

Data concerning the seasonality and geographical location of tsutsugamushi disease (TD) in Japan from 1955 to 2014 were compiled. After disappearance of "classical" TD in the mid-1960s, there was a long near-vacuum. Then, in 1975, a TD epidemic with October-December seasonality emerged in southern prefectures, and in 1979, that with May-June seasonality in northern prefectures. The current TD epidemic could be classified according to seasonality and geography into three types: a large October-December epidemic with a negligibly small May-June epidemic in the southern part of Japan, a large May

OBJECTIVE: To study association between the geographic location (latitude) of studies evaluating protective effect of BCG vaccine and it's efficacy / effectiveness against tuberculosis.
METHODS: A comprehensive literature search was carried out to identify relevant studies. Data extraction from these studies included place of study (geographic latitude), study design and reported point estimate of protective effect of BCG vaccine against tuberculosis. Information on latitude was obtained from Oxford School Atlas for World Geography. A spearman rank correlation coefficient was estimated to stu

Oral vaccines have several advantages compared with parenteral administration: they can be relatively cheap to produce in high quantities, easier to administer, and induce intestinal mucosal immunity that can protect against infection. These characteristics have led to successful use of oral vaccines against rotavirus, polio, and cholera. Unfortunately, oral vaccines for all three diseases have demonstrated lower performance in the highest-burden settings where they are most needed. Rotavirus vaccines are estimated to have >85% effectiveness against hospitalization in children <12 months in co

Chilblains are an inflammatory skin condition that develops following exposure to the cold. They consist of pruritic and/or painful erythematous-to-violaceous acral lesions and are the result of an abnormal vascular response to exposure to the cold. This benign condition is common in southern Brazil, particularly in Rio Grande do Sul where climatic conditions, together with cultural and economic aspects, facilitate the development of these sores.

This article explores the medical conceptualization of the causes of diseases in nineteenth-century Colombia. It traces the history of some of the pathologies that were of major concern among nineteenth-century doctors: periodic fevers (yellow fever and malaria), continuous fevers (typhoid fever), and leprosy (Greek elephantiasis). By comparing the transforming conceptualizations of these diseases, this article shows that their changing pattern, the idea of climatic determinism of diseases (neo-Hippocratism and medical geography), the weak standing of the medical community in Colombian society

Disparities by sex, race, socioeconomic status, and geography exist in diagnosis, treatment, and outcomes for people with lower extremity peripheral artery disease (PAD). PAD prevalence is similar in men and women, but women have more atypical symptoms and undergo lower extremity revascularization at older ages compared to men. People who are Black have an approximately 2-fold higher prevalence of PAD, compared to people who are White and have more atypical symptoms, greater mobility loss, less optimal medical care, and higher amputation rates. Although fewer data are available for other races

This paper summarizes the geography of sleeping sickness disease (or Human African Trypanosomiasis, HAT) over the last 100 years in West Africa, with the objective of identifying today's priority areas for the sleeping sickness surveillance. The history and geography of the disease are based on a bibliographic review of old reports and recent publications on recent results obtained from medical surveys conducted in West Africa up to 2007. This allowed us to situate the historical geography of HAT from the beginning of the 20th century to nowadays. For instance, active HAT foci seem to have mov

Bronchiectasis is a disease associated with chronic progressive and irreversible dilatation of the bronchi and is characterised by chronic infection and associated inflammation. The prevalence of bronchiectasis is age-related and there is some geographical variation in incidence, prevalence and clinical features. Most bronchiectasis is reported to be idiopathic however post-infectious aetiologies dominate across Asia especially secondary to tuberculosis. Most focus to date has been on the study of airway bacteria, both as colonisers and causes of exacerbations. Modern molecular technologies in

INTRODUCTION: In past decades, leishmaniasis burden has been low across Egypt; however, changing environment and land use has placed several parts of the country at risk. As a consequence, leishmaniasis has become a particularly difficult health problem, both for local inhabitants and for multinational military personnel.
METHODS: To evaluate coarse-resolution aspects of the ecology of leishmaniasis transmission, collection records for sandflies and Leishmania species were obtained from diverse sources. To characterize environmental variation across the country, we used multitemporal Land Sur

The concurrence of malaria and hemoglobinopathies, observed in malaria endemic regions, reflects the phenomenon of natural selection. Since the life cycle of the malaria parasite has an erythrocytic stage, abnormalities in the red blood cells (RBCs) hinder the parasite's survival in the human host. Hemoglobin (Hb) variants affect the life span of RBCs and thus lower the chance of infection by the parasite. While a change in just one of the Hb genes offers some protection against malaria, change in both alleles results in β-thalassemia major (β-TM). A striking geographical heterogeneity of β-th
