Characterization of multinucleated giant cells in synovium and subchondral bone in knee osteoarthritis and rheumatoid arthritis.

BACKGROUND: Multinucleated giant cells have been noticed in diverse arthritic conditions since their first description in rheumatoid synovium. However, their role in the pathogenesis of osteoarthritis (OA) or rheumatoid arthritis (RA) still remains broadly unknown. We aimed to study the presence and characteristics of multinucleated giant cells (MGC) both in synovium and in subchondral bone tissues of patients with OA or RA. METHODS: Knee synovial and subchondral bone samples were from age-matched patients undergoing total joint replacement for OA or RA, or non-arthritic post mortem (PM) cont

The host tissue reaction to Madurella mycetomatis: new classification.

In this prospective study, three types of tissue reaction to mycetoma grains are described. Type I reaction is characterized by the adherence of neutrophils to the surface of the grain leading to its disintegration. In Type II reaction, the fragmented grain and the dead neutrophils are cleared by macrophages and multinucleated giant cells, while in Type III reaction there is a discrete well developed epithelioid granuloma with Langhans's giant cells. The three types of reaction are usually seen in different combinations in the same lesion.

Mycobacterium fortuitum granulomatous synovitis caused by a dog bite.

Mycobacterium fortuitum was isolated from the synovial tissues of the dorsum of the hand after a dog bite. Caseating granulomas with Langhans' type giant cells were demonstrated microscopically. Healing followed prolonged treatment with antituberculosis drugs. It is suggested that the organism was forced into the patient's tissues by the dog's bite.

Clinical and laboratory features of PCR-confirmed periocular tuberculosis in China.

Experts lack knowledge of periocular tuberculosis (TB) in China. Nested polymerase chain reaction (PCR) shows advantages in diagnosis of extrapulmonary TB. Our study aims to explore the clinical and laboratory features of PCR-confirmed periocular TB. We retrospectively reviewed medical records of presumptive periocular TB and performed nested PCR test to confirm diagnosis. Nine cases were recruited. Clinical symptoms were chronic and insidious. Eight cases achieved favorable visual acuity, while one underwent enucleation due to fungal-TB panophthalmitis. Sensitivity of caseous necrosis, acid-f

[A new case of Mycobacterium bovis pulmonary tuberculosis in the dromedary (Camelus dromedarius) in Mauritania].

A case of pulmonary tuberculosis is described in a dromedary from Nouakchott (Mauritania). Gross lesions affected pulmonary parenchyma, diaphragmatic pleura, pericardium and regional lymph nodes: caseo-calcified nodules, miliary tubercles and haemorrhagic "pendeloques". Microscopically lesions were characterised by granulomatous tissue, epithelioid cells, necrotic material in the centre. No Langhans giant cells were seen. Mycobacterium bovis was isolated from these samples.

[A case of esophageal and intestinal tuberculosis that occurred during treatment of rheumatoid arthritis with etanercept].

An 88-year-old woman with rheumatoid arthritis who had started etanercept treatment in July 2011 was referred to our hospital in February 2012 for right-sided pleural effusion. Chest computed tomography showed right pleural effusion, partial swelling of a calcified mediastinal lymph node, and mid-esophageal thickening of the mucosal wall. Gastroendoscopy showed mid-esophageal ulceration. Histological examination of biopsy specimens from this ulceration revealed noncaseating granulomas with Langhans giant cells. Ziehl-Neelsen staining of this section was positive for acid-fast bacilli. Polymera

Atypical large plasma cells in lymph node granulomas in cat-scratch disease.

A histological variant of plasma cells found in the granulomas of cat-scratch disease (CSD) lymphadenitis is reported. Though the lesion shows the typical features of suppurative granulomatous lymphadenitis, many atypical giant cells which have abundant basophilic cytoplasm and bizarre nuclei with occasional multinucleated forms are noted among epithelioid histiocytes. The diagnosis of CSD lymphadenitis was confirmed by comparing clinical; histopathological, and histochemical (Warthin-Starry silver impregnation stain) studies on lymph node sections from five cases with features typical of the

Cytopathology of intraocular lens implantation.

The cytopathology of intraocular lens (IOL) following implantation is reviewed. A newly placed implant attracts macrophages and these settle on its surfaces to form optically clear membranes composed of so-called fibroblast-like cells and a film of proteinaceous material. The membranes apparently become tougher and more firmly adherent with time. The fibroblast-like cells have phagocytic abilities and may form fibrous structures on the surface of implants in complicated situations. Few multinucleated giant cells may be seen in successful cases, but great numbers of giant cells are indicators o

[A case of Wegener's granulomatosis with a giant solitary pulmonary mass].

A 44-year-old woman was admitted to hospital with a history of high fever, cough, and weight loss. Her chest X-ray film showed a giant solitary pulmonary mass in the left lower lobe. After admission, she developed bilateral eyeache and left exophthalmos. CT film of the head showed a massive lesion in the sinuses. Biopsy specimens obtained from both massive lesions showed either epithelioid granulomas with Langhans' giant cells or necrotising vasculitis. Antineutrophil cytoplasmic antibodies, which have high sensitivity and specificity for active WG, were found. The diagnosis of WG was thus mad

Cytological diagnosis of vertebral tuberculosis with fine-needle aspiration biopsy.

Fine-needle aspiration biopsy monitored with computed tomography was used for the diagnosis of vertebral tuberculosis in thirty-eight patients; the diagnosis was confirmed by culture or by a Ziehl-Neelsen stain of the smear in thirty-four of the patients. Cytological examination revealed collections of epithelioid cells, scattered multinucleated Langhans giant cells, and necrosis. There were no complications related to the biopsy, and an operative biopsy was avoided.

The pathogenesis of giant cell arteritis: morphological aspects.

The light-microscopic, electron-microscopic and immunocytochemical characteristics of giant cell arteritis (GCA) have been investigated in a number of studies on temporal arteries. Arterial atrophy and calcification of the internal elastic membrane appear to be prerequisites for the evolution of the inflammatory process. Foreign body giant cells form close to calcifications, apparently without connection with other inflammatory cells and probably by the fusion of modified vascular smooth muscle cells. The foreign body giant cells attack the calcifications. Lymphocytes accumulate around them an

Gastric T-cell lymphoma presenting with epithelioid granulomas mimicking tuberculosis in regional lymph nodes.

In patients with malignant lymphomas, a sarcoid reaction is occasionally observed. However, lymphoma-related granulomas with caseous necrosis are rare. We describe such a case of T-cell gastric lymphoma that was difficult to diagnose. A 50-year-old man was referred to our hospital because of abnormal gastric endoscopic findings: hypertrophic folds with narrowing of the gastric lumen and multiple ulcers in the body. Gastric biopsy specimens showed non-specific inflammation. An open biopsy of the enlarged gastric regional lymph nodes was performed. The sections revealed effacement of the normal

Coexpression of NRAMP1, iNOS, and nitrotyrosine in bovine tuberculosis.

In murine models the inducible nitric oxide synthase (iNOS) and the natural resistance associated macrophage protein (NRAMP1) play major roles in host defense against mycobacteria. iNOS regulates nitric oxide (NO) production, which is noxious for ingested mycobacteria, and NRAMP1 displays pleiotropic antimicrobial effects, including upregulation of iNOS expression. Little is known about the role of these molecules in bovine tuberculosis (TB). In this work we demonstrate by Western blot a high expression of NRAMP1 in peripheral blood mononuclear cells (PBMCs), alveolar macrophages (obtained by

Elastophagocytosis in borderline tuberculoid leprosy: A rare case with diagnostic challenge and histopathological enigma: A case report.

Leprosy, or Hansen's disease, is a chronic granulomatous infection caused by Mycobacterium leprae, with a broad spectrum of clinical and histopathological manifestations. Histological examination plays a pivotal role in accurately diagnosing its various subtypes. Elastophagocytosis (phagocytosis of elastic fibers by giant cells) is typically associated with annular elastolytic giant cell granuloma and other granulomatous conditions but has rarely been reported in leprosy. We report the case of an 85-year-old male who presented with a solitary, slowly enlarging annular plaque on the trunk. The

Isolated tubercular dacryoadenitis in a non-immunocompromised healthy adult.

A middle-aged woman presented with a slowly enlarging mass in the right superotemporal orbit and ptosis. The patient had an elevated erythrocyte sedimentation rate, and incisional biopsy showed granulomatous inflammation with Langhan's giant cells suggestive of a tubercular etiology. Tuberculin skin test was negative. The patient responded well to anti-tuberculous therapy. Tubercular dacryoadenitis is a rare presentation of a lacrimal gland mass.
