[Neovascular glaucoma in the diabetic patient].

Neovascular glaucoma is the final complication of diabetic proliferative retinopathies. Its incidence had significantly reduced during these last ten years owing to the improvement of the management of diabetic patients and systematic panretinal photocoagulation for the treatment of these proliferative retinopathies. Contrary to the other causes of neovascular glaucomas, its evolution is usually capricious, bilateral and its delay of appearance may be quite variable. Vitrectomy--when non associated to peroperative endoretinal photocoagulation--and cataract extraction--mainly intracapsular cata

Clinical outcomes of the PAUL Glaucoma implant for neovascular glaucoma.

PURPOSE: In neovascular glaucoma (NVG), surgical interventions such as glaucoma drainage devices (GDD), may become necessary, especially when other therapies prove ineffective. The PAUL® Glaucoma Implant (PGI), with its refined, smaller drainage tube, presents a promising solution for lowering intraocular pressure (IOP) in such complex cases. This study aims to evaluate the PGI's effectiveness and safety in managing IOP in patients with NVG. METHODS: This study reviewed medical records of patients who underwent PGI surgery for NVG at the University Hospital Bonn between May 2021 and January 2

Angiogenesis in glaucoma filtration surgery and neovascular glaucoma: A review.

Angiogenesis may pose a clinical challenge in glaucoma, for example, during the wound healing phase after glaucoma filtration surgery and in the severe secondary glaucoma called neovascular glaucoma (NVG). Upregulation of vascular endothelial growth factor (VEGF), a key mediator of angiogenesis, occurs in eyes that have undergone glaucoma filtration surgery, as well as those with NVG. This has led investigation of the ability of anti-vascular endothelial growth factor therapy to improve outcomes, and we examine the findings with respect to the safety and efficacy of anti-vascular endothelial g

Ciliary sulcus Ahmed Glaucoma Valve tube placement in neovascular glaucoma.

A 61-year-old woman presented with severe ocular pain 3 months after cataract extraction with intraocular lens placement. After performing a thorough ophthalmic examination, it was determined that neovascular glaucoma was present. The patient underwent implantation of an Ahmed valve (New World Medical, Inc., Rancho Cucamonga, CA) with tube placement into the ciliary sulcus and received 1.25 mg of bevacizumab intracamerally. The patient's iris neovascularization resolved and the patient maintained stable intraocular pressures without any major complications 6 months after surgery. Tube placemen

Radiation-induced neovascular glaucoma: A devastating disease.

Neovascular glaucoma (NVG) is a potentially blinding form of secondary glaucoma, with radiation being one of the rare causes. This report is aimed to discuss a case of NVG caused secondary to radiotherapy (RT) given for a nasal malignancy. A 50-year-old male presented with enophthalmos, dry eye, and NVG 3 years after receiving RT for chondrosarcoma of nasal and paranasal cavities. He was given topical antiglaucoma medications, retinal laser, and intravitreal bevacizumab injection and thus prevented the eye from becoming a painful blind eye. The radiation oncologist and ear, nose, and throat sp

Neovascular glaucoma: pathophysiology, diagnosis, and contemporary management.

BACKGROUND: Neovascular glaucoma (NVG) is a vision-threatening secondary glaucoma driven by pathologic neovascularization of the iris and anterior chamber angle and is most commonly secondary to ischemic retinal diseases. Despite advances in retinal imaging and anti-vascular endothelial growth factor (anti-VEGF) therapy, NVG remains difficult to manage and is associated with high rates of vision loss, ocular morbidity, and surgical failure. PURPOSE: This narrative review synthesizes the contemporary understanding of NVG pathophysiology, summarizes diagnostic approaches with emphasis on anteri

Medical and surgical management of neovascular glaucoma.

PURPOSE OF REVIEW: Neovascular glaucoma (NVG) is a severe secondary glaucoma precipitated by ocular ischemia and abnormal neovascularization, resulting in elevated intraocular pressure (IOP) and vision loss if not promptly addressed. This study evaluates recent advances in both medical and surgical management of NVG, focusing on strategies that integrate anti-vascular endothelial growth factor (VEGF) therapy, retinal ablation, and evolving surgical techniques. RECENT FINDINGS: Anti-VEGF agents remain central to NVG treatment, with newer agents and combination regimens showing sustained neovas

Etiology, pathogenesis, and diagnosis of neovascular glaucoma.

Neovascular glaucoma is defined as iris and/or anterior chamber angle neovascularization associated with increased intraocular pressure. It is a secondary glaucoma that is most frequently caused by severe retinal ischemia. The most common diseases responsible for the development of neovascular glaucoma are diabetic retinopathy, ischemic central retinal vein occlusion, and ocular ischemic syndrome. Uncommon causes include ocular radiation, ocular tumors, uveitis and other miscellaneous conditions. Vascular endothelial growth factor is an important and likely predominant agent involved in the pa

Neovascular glaucoma and carotid artery obstructive disease.

Carotid artery obstructive disease, although infrequently diagnosed as a primary or contributing cause of neovascular glaucoma, can produce distinctive characteristics. Decreased perfusion of the ciliary body may decrease aqueous humor production. As a result, such eyes with neovascular glaucoma may occasionally be normotensive or even hypotensive. Fluorescein angiography may show an increased arm-to-retina time and leakage from the major retinal arterioles. Panretinal photocoagulation may not eliminate the anterior segment neovascularization because of anterior segment ischemia. Endarterectom

Neovascular glaucoma in systemic occlusive vascular disease.

A 58-year-old woman was seen with a red inflamed right eye with rubeosis iridis and neovascular glaucoma with closure of the angle. There was no evidence of the diabetes and no evidence of occlusion of the central retinal vein. Physical examination was normal except for an absent carotid pulse on the right side, a carotid bruit on the left side, and absence of the pulse in all extremities. Further evaluation revealed severe arteriosclerosis involving many major branches of the aorta, including both carotid arteries. This patient had neovascular glaucoma as the only presenting sign of severely

[A case report of neovascular glaucoma due to carotid artery occlusive disease--diagnosis of neovascular glaucoma and histological characteristics].

PURPOSE: The methods of diagnosing neovascular glaucoma were evaluated in a case with carotid artery occlusive disease. A trabeculectomy specimen taken from this patient was examined histologically to find the characteristic picture of this slowly progressive neovascular glaucoma. MATERIALS AND METHODS: The left eye of a 48-year old man with neovascular glaucoma due to carotid artery occlusive disease had been previously diagnosed as uveitis with ocular hypertension. His left eye was examined by fluorescein fundus and gonioangiography. Trabeculectomy was performed because of uncontrolled intr

Neovascular glaucoma after central retinal vein occlusion in pre-existing glaucoma.

BACKGROUND: To determine the outcome of central retinal vein occlusion (CRVO) in pre-existing glaucoma and the predisposing factors of developing neovascular glaucoma (NVG). METHODS: We retrospectively assessed a pre-existing glaucoma CRVO group and a non-glaucoma CRVO group to elucidate the demographics, clinical course and ocular parameters of these two cohorts. Among the pre-existing glaucoma cases, the predisposing factors for the development of NVG were monitored, including the retinal capillary non-perfusion status, intraocular pressure (IOP) and best-corrected visual acuity (BCVA) at p
