Painless Aortic Syndrome in a Patient with Syncope and Globus Sensation: A Case Report.

INTRODUCTION: Aortic dissection is a devastating clinical entity with a variety of presentations and requires prompt recognition and management. To our knowledge this is the first reported case of a patient who presented with a globus sensation and was diagnosed with an aortic dissection prior to clinical deterioration. CASE REPORT: The patient presented with an episode of near-syncope and globus sensation. Imaging studies revealed a type A aortic dissection with hemopericardium requiring emergent operative intervention. Unfortunately, the patient's course was complicated by significant hemor…

Globus sensation: value of static radiography combined with videofluoroscopy of the pharynx and oesophagus.

Pharyngo-oesophageal abnormalities are found in a high proportion of patients with globus sensation. This study compares the diagnostic value of static single- and double-contrast radiography of the pharynx and oesophagus with videofluoroscopy and with videofluoroscopy combined with static radiography in these patients. Pharyngeal and oesophageal morphology and motor function were studied in 130 consecutive patients with globus sensation (46 males, 84 females; mean age, 47 years) by means of static single and double-contrast radiography and by videofluoroscopy. Videofluoroscopy revealed signif…

Upper GIT Endoscopic Evaluation and Psychological State Assessment of Patients with Globus Sensation.

Globus sensation is a subjective feeling of a lump or foreign body in the throat without interfering swallowing of food. It is a persistent and distressing sensation in throat. It affects about 6% of population. But cause of globus is still unknown. Exact aetiology of globus is considered to be multifactorial. Some other studies also show association between globus and psychological distress including anxiety and depression. As there is no established pharmacological treatment, adequate investigations with negative result could reassure patients and improve their symptoms. In this prospective …

Hyperdynamic upper esophageal sphincter pressure: a manometric observation in patients reporting globus sensation.

OBJECTIVES: Although globus sensation is a common symptom, its pathogenesis is poorly defined. The aim of this study was to quantify the timing and magnitude of respiratory variation in upper esophageal sphincter (UES) pressure with high-resolution manometry (HRM) in patients with globus sensation, normal controls, and gastroesophageal reflux disease (GERD) patients without globus sensation. METHODS: HRM recordings spanning from the hypopharynx to the stomach were analyzed in 131 consecutive globus patients with normal (64) and abnormal (67) distal esophageal motility. Resting UES pressure wa…

Globus sensation and increased upper esophageal sphincter pressure with distal esophageal acid perfusion.

The aim of the present study was to determine whether acid perfusion into the distal esophagus causes a globus sensation and an increase in upper esophageal sphincter (UES) pressure. UES pressures were measured using a sleeve-type sensor in 20 healthy volunteers. A 0.1-N HCl solution, the same as gastric acid, was perfused into the distal esophagus at a rate of 20 mL/min. During perfusion, 4-channel pH monitoring was performed to determine whether the HCl reached the hypopharynx. The following parameters were measured: (1) changes in UES pressure before and during acid perfusion, (2) presence …

Upper esophageal sphincter tone and reactivity to stress in patients with a history of globus sensation.

Anxiety and abnormal upper esophageal sphincter function have been ascribed ill-defined roles in the etiology of globus sensation. In this study, we examined the psychological profile and effect of acute mental stress (dichotic listening task) on UES tone in seven patients reporting to the clinic with globus sensation and 13 healthy controls. Alterations in heart rate, blood pressure, frontalis EMG, and skin conductance confirmed the effectiveness of the stress test in patients and controls. During resting conditions, UES pressure (mean +/- SE) in patients (40.4 +/- 4.6 mm Hg) did not differ s…

Globus Sensation Due to a Mobile Foreign Body in a 41-year-old Female.

Leech infestation could be associated with wide range of complications including epistaxis, stridor, hemoptysis, globus sensation, hematemesis, and vaginal bleeding. In the present case report. , we introduce a case of leech infestation in a 41-year-old female presenting with chronic globus sensation, stridor, vomiting and dysphagia later diagnosed with leech infestation. Therefore, leech bites might be considered as a differential. Diagnosis of globus sensation in the endemic rural areas where in unhealthy water from natural sources is consumed without taking required hygienic precautions.

Globus sensation in obstructive sleep apnea patients; A cross-sectional study of 120 patients.

PURPOSE: Globus pharyngeus (GP) is a common complaint in many disciplines, especially otolaryngology. Pharyngeal symptoms and abnormalities, including GP, are frequent in obstructive sleep apnea (OSA) patients. This study aims to investigate globus sensation in non-operated OSA patients. METHODS: After translation and validation, the Laryngopharyngeal Measure of Perceived Sensation (LUMP) was administered to 120 untreated OSA patients. All patients underwent polysomnography and thorough physical examination. The association between LUMP scores and OSA measures was evaluated. RESULTS: LUMP sc…

Cannabis-Associated Aerodigestive Symptoms in an Adolescent: A Case Report of Globus Sensation, Dysphagia, and Hypersalivation.

Cannabis use remains common among adolescents, and evolving product potency and routes of administration raise concern for underrecognized effects on oropharyngeal and esophageal function. A 16-year-old male presented with globus sensation, dysphagia, and hypersalivation after 2 years of daily cannabis use. Extensive evaluation, including flexible laryngoscopy and barium esophagram, was unremarkable. Empiric treatment with proton pump inhibitors, antibiotics, and allergy medications offered minimal benefit. Symptoms partially improved after cannabis cessation and with glycopyrrolate, with furt…

Sliding hiatal hernia detected by transnasal esophagogastroscopy is associated with poorer treatment response in reflux-associated globus sensation.

Transnasal esophagogastroscopy (TNE) may provide clinically relevant information on gastroesophageal junction morphology in patients with globus sensation and reflux disease. This prospective single-arm interventional cohort study with a pre-post design evaluated the association between TNE findings and treatment response in patients with objectively confirmed reflux disease. Ninety-two patients with globus sensation underwent dual-probe pH monitoring; reflux disease was detected in 59 patients. These patients underwent TNE and completed the German version of the Glasgow Edinburgh Throat Scale…
