Glucokinase deficit and birthweight: does maternal hyperglycemia always meet fetal needs?

AIMS: Many authors do not recommend hypoglycemic treatment during pregnancy in women affected by monogenic diabetes due to heterozygous glucokinase (GCK) mutations (MODY 2) in case of affected fetus, because maternal hyperglycemia would be necessary to achieve a normal birthweight. We aimed to evaluate differences in birthweight between MODY 2 affected children according to the parent who carried the mutation. METHODS: We retrospectively studied 48 MODY 2 affected children, whose mothers did not receive hypoglycemic treatment during pregnancy, divided into two groups according to the presence

Dorzagliatin: A Breakthrough Glucokinase Activator Coming on Board to Treat Diabetes Mellitus.

Dorzagliatin, an innovative dual-acting allosteric oral glucokinase activator that targets glucose homeostasis and insulin resistance, has gained approval for treating type 1 diabetes mellitus (T1DM) and type 2 diabetes mellitus (T2DM). The effectiveness of existing antidiabetic treatments in enhancing beta cell (β-cell) activity is restricted. Currently, there are no satisfactory medications available to address the fundamental deficiency in glucose sensing for glucokinase-maturity-onset diabetes of the young (GCK-MODY), which is caused by mutations in the glucokinase gene; researchers have e

Discovery of liver-directed glucokinase activator having anti-hyperglycemic effect without hypoglycemia.

Glucokinase activators (GKAs) are among the emerging drug candidates for the treatment of type 2 diabetes (T2D). Despite effective blood glucose lowering in clinical trials, many pan-GKAs "acting both in pancreas and liver" have been discontinued from clinical development mainly because of their potential to cause hypoglycemia. Pan-GKAs over sensitize pancreatic GK, resulting in insulin secretion even at sub-normoglycemic level which might be a possible explanation for hypoglycemia. An alternative approach to minimize the risk of hypoglycemia is to use liver-directed GKAs, which are reported t

Discovery of piragliatin--first glucokinase activator studied in type 2 diabetic patients.

Glucokinase (GK) activation as a potential strategy to treat type 2 diabetes (T2D) is well recognized. Compound 1, a glucokinase activator (GKA) lead that we have previously disclosed, caused reversible hepatic lipidosis in repeat-dose toxicology studies. We hypothesized that the hepatic lipidosis was due to the structure-based toxicity and later established that it was due to the formation of a thiourea metabolite, 2. Subsequent SAR studies of 1 led to the identification of a pyrazine-based lead analogue 3, lacking the thiazole moiety. In vivo metabolite identification studies, followed by th

Resetting the glucose sensor: dorzagliatin and the evolution of glucokinase activation in diabetes therapy.

The progressive decline in functional β-cell mass represents a fundamental, unmet challenge in type 2 diabetes (T2DM) management. Dorzagliatin (HMS5552), a fourth-generation, dual-acting, and glucose-dependent glucokinase activator (GKA), offers a distinct mechanistic approach that aims to move beyond symptomatic glycemic control toward potential glucose sensor modulation. This review critically synthesizes the development of dorzagliatin, designed to overcome the hypoglycemia and tachyphylaxis of earlier GKAs. While its unique allosteric mechanism enables synchronized restoration of insulin s

Evaluation of efficacy and safety of glucokinase activators-a systematic review and meta-analysis.

AIMS: Glucokinase activators (GKAs) promote the activity of glucokinase (GK) and is under development for the treatment of diabetes. The efficacy and safety of GKAs require evaluation. METHODS: This meta-analysis included randomized controlled trials (RCTs) with a duration of at least 12 weeks conducted in patients with diabetes. The primary objective of this meta-analysis was the difference of hemoglobin A1c (HbA1c) change from baseline to study end between GKA groups and placebo groups. Risk of hypoglycemia and laboratory indicators were also evaluated. Weighted mean differences (WMDs) and

Chronotherapy with a glucokinase activator profoundly improves metabolism in obese Zucker rats.

Circadian rhythms play a critical role in regulating metabolism, including daily cycles of feeding/fasting. Glucokinase (GCK) is central for whole-body glucose homeostasis and oscillates according to a circadian clock. GCK activators (GKAs) effectively reduce hyperglycemia, but their use is also associated with hypoglycemia, hyperlipidemia, and hepatic steatosis. Given the circadian rhythmicity and natural postprandial activation of GCK, we hypothesized that GKA treatment would benefit from being timed specifically during feeding periods. Acute treatment of obese Zucker rats with the GKA AZD16

Pregnancy outcome of Japanese patients with glucokinase-maturity-onset diabetes of the young.

AIMS/INTRODUCTION: Glucokinase-maturity-onset diabetes of the young (GCK-MODY; also known as MODY2) is a benign hyperglycemic condition, which generally does not require medical interventions. The only known exception is increased birthweight and related perinatal complications in unaffected offspring of affected women. As previous data were obtained mostly from white Europeans, the present study analyzed the pregnancy outcomes of Japanese women with GCK-MODY to better formulate the management plan for this population. MATERIALS AND METHODS: The study participants were 34 GCK-MODY families wh

Identification of a new class of glucokinase activators through structure-based design.

Glucose flux through glucokinase (GK) controls insulin release from the pancreas in response to high glucose concentrations. Glucose flux through GK also contributes to reducing hepatic glucose output. Because many individuals with type 2 diabetes appear to have an inadequacy or defect in one or both of these processes, compounds that can activate GK may serve as effective treatments for type 2 diabetes. Herein we report the identification and initial optimization of a novel series of allosteric glucokinase activators (GKAs). We discovered an initial thiazolylamino pyridine-based hit that was

[Glucokinase gene mutation as a causative factor of permanent neonatal diabetes mellitus].

INTRODUCTION: The most frequent type of diabetes in childhood is type 1 diabetes. Thanks to the development of genetic testing, the rare monogenic forms of that disease have been defined. One of them is neonatal diabetes identified within the first 6 months of life and often associated with the mutation in KCNJ11, ABCC8 or insulin gene. A less frequent mutation in the glucokinase gene can cause both permanent neonatal diabetes as well as mild diabetes MODY 2. CASE REPORT: A 33-day-old boy admitted to hospital because of hyperglycemia from the first day of life. Treatment with intravenous infu

Targeting glucokinase activation for the treatment of type 2 diabetes--a status review.

Glucokinase (GK) plays a key role in glucose homeostasis. Developments over the past decade, such as the determination of the function of GK regulatory protein, the discovery of GK mutations related to maturity onset of diabetes of the young, permanent neonatal diabetes mellitus and persistent hyperinsulinemia hypoglycemia of infancy, and the discovery of novel GK activators (GKAs) and their X-ray co-crystal structures with GK, have significantly enhanced our understanding of GK structure and function. This review discusses key publications on GKAs that report full characterization, key compou

Hexokinase isoenzymes in normal and cirrhotic human liver: suppression of glucokinase in cirrhosis.

The activities of hexokinase isoenzymes I-IV (EC 2.7.1.1) and of N-acetylglucosamine kinase (EC 2.7.1.59) were determined in normal human liver and in alcoholic liver disease and primary biliary cirrhosis after FPLC fractionation of high-speed supernatants on Mono-Q with a linear NaCl gradient. In control human liver the hexokinase activities were: I, 3.6; II, 0.7; III, 3.5, IV, 4.8 (mUnits/mg supernatant protein). The activity of N-acetylglucosamine kinase was 8 mU/mg of protein. In alcoholic liver disease and primary biliary cirrhosis, the activity of hexokinase IV (glucokinase) was suppress

Unimpaired effect of insulin on glucokinase gene expression in hepatocytes challenged with amylin.

Amylin appears to interfere with the action of insulin in muscle and possibly in liver. We have attempted to detect a direct antagonism between amylin and insulin in cultured rat hepatocytes. The stimulation of glucokinase gene expression was used as a marker of insulin action. Amylin proved ineffective in suppressing subsequent accumulation of glucokinase mRNA in response to maximal or submaximal doses of insulin. When applied to cells already induced by prior incubation with insulin alone, amylin failed to reverse induction, in contrast to the effectiveness of glucagon under the same conditi

Glucokinase Inactivation Reduces Lysophosphatidylcholine Accumulation to Alleviate High-Fat Diet-Induced Hepatic Steatosis.

Partial inactivation of glucokinase (GCK) is typically characterized by mild hyperglycemia and a favorable lipid profile compared to type 2 diabetes. Previous studies have shown that GCK activity influences serum lipid profiles in a diet-dependent manner; however, its role in hepatic lipid metabolism in the context of metabolic dysfunction-associated steatotic liver disease (MASLD) remains unclear. To address this, we utilized a newly established heterozygous GCK mutation knock-in mouse model (GCKMut) fed either a normal diet (ND) or a high-fat diet (HFD). Under ND conditions, GCKMut mice deve

The glucokinase mutation p.T206P is common among MODY patients of Jewish Ashkenazi descent.

BACKGROUND: Maturity-onset diabetes of the young (MODY) is characterized by an autosomal dominant mode of inheritance; a primary defect in insulin secretion with non-ketotic hyperglycemia, age of onset under 25 yr; and lack of autoantibodies. Heterozygous mutations in glucokinase (GCK) are associated with mild fasting hyperglycemia and gestational diabetes mellitus while homozygous or compound heterozygous GCK mutations result in permanent neonatal diabetes mellitus. Given that both the Israeli-Arabic and the various Israeli-Jewish communities tend to maintain ethnic seclusion, we speculated t

Glucose-regulated glucose uptake by transplanted muscle cells expressing glucokinase counteracts diabetic hyperglycemia.

Type 1 diabetic patients depend on insulin replacement therapy. However, chronic hyperglycemia due to failure to maintain proper glycemic control leads to microvascular, macrovascular, and neurological complications. Increased glucose disposal by tissues engineered to overexpress key regulatory genes in glucose transport or phosphorylation can reduce diabetic hyperglycemia. Here we report that differentiated myoblast cells expressing the glucose-phosphorylating enzyme glucokinase (GK) showed a glucose-dependent increase in glucose uptake and utilization in vitro. Transplantation of GK-expressi

Recent clinical advances of glucokinase activators in the treatment of diabetes mellitus type 2.

Glucokinase (GK) is an isozyme of hexokinase that catalyzes the phosphorylation of glucose. GK is present in many organs of the human body, including the liver and pancreas. GK plays an important role in promoting the synthesis of hepatic glycogen and balancing postprandial blood glucose. Mutations in the GK gene can result in the inclusion of maturity-onset diabetes of the young (MODY2) and permanent neonatal Diabetes mellitus (PNDM). Glucokinase activators (GKAs) are a class of type 2 diabetes drugs designed for this target. In various animal models of type 2 diabetes, GKAs have been shown t

The potential role of glucokinase activator SHP289-04 in anti-diabetes and hepatic protection.

Glucokinase was glucose sensor in hepatocytes and islet beta cells. It not only promoted glucose metabolism, but also advanced glucose-stimulated insulin secretion. Glucokinase activators had been reported as one kind of new potential drugs for treatment of type 2 diabetes. Compound 6-(5-(3-fluorobenzyloxy)-2-ethoxybenzamido) pyridine-3-carboxylic acid (SHP289-04) was found to increase glucokinase activity effectively by our lab. In order to demonstrate its effect of anti-diabetes, in vitro, human liver carcinoma cell line HepG2 was used to glucose consumption test, and pancreatic cell line NI

Loss of glucokinase function relieves carbon catabolite repression in basidiomycetous yeast Pseudozyma antarctica.

The basidiomycetous yeast Pseudozyma antarctica produces the biodegradable plastic-degrading enzyme PaE. The PaE overexpression strain, constructed by replacing the promoter of the PaE gene with the xylanase promoter, exhibited high-level production of PaE in the presence of xylose. However, this production is suppressed by carbon catabolite repression (CCR) in the presence of glucose. Although CCR is a major issue involving the decreased expression of hydrolytic genes from their promoters in ascomycetes, little information is available in basidiomycetous yeasts. In the present study, we demon
