Stepped Care for Opioid Use Disorder Train the Trainer (SCOUTT) initiative: Expanding access to medication treatment for opioid use disorder within Veterans Health Administration facilities.

The US is confronted with a rise in opioid use disorder (OUD), opioid misuse, and opioid-associated harms. Medication treatment for opioid use disorder (MOUD)-including methadone, buprenorphine and naltrexone-is the gold standard treatment for OUD. MOUD reduces illicit opioid use, mortality, criminal activity, healthcare costs, and high-risk behaviors. The Veterans Health Administration (VHA) has invested in several national initiatives to encourage access to MOUD treatment. Despite these efforts, by 2017, just over a third of all Veterans diagnosed with OUD received MOUD. VHA OUD specialty ca

Comparative effectiveness of ceftolozane/tazobactam relative to ceftazidime/avibactam for the treatment of adult, hospitalized patients with pneumonia due to multidrug-resistant Pseudomonas aeruginosa in U.S. Veterans Health Administration facilities.

Ceftolozane/tazobactam (C/T) and ceftazidime/avibactam (C/A) are frequently used for the treatment of multidrug-resistant Pseudomonas aeruginosa (MDR-PSA) pneumonia. However, comparative "real-world" effectiveness data are limited. We performed a retrospective cohort study within the U.S. Veterans Health Administration (VHA; 2015-2019, 2022-2024), including adults with microbiologically confirmed MDR-PSA pneumonia receiving C/T or C/A within ≤5 days of index culture. Inverse probability of treatment weighting (IPTW) was applied to balance baseline covariates. The primary outcome was 30-day all

Early antenatal care (ANC) booking and associated factors among pregnant women attending ANC services at public health facilities in Bahir Dar City Administration, Northwestern Ethiopia, 2023: a cross-sectional study.

Antenatal care (ANC) is vital for improving maternal and fetal health by enabling the early detection and management of pregnancy-related complications. The World Health Organization recommends a minimum of eight ANC contacts to optimize maternal and neonatal outcomes and reduce perinatal mortality. Increased ANC contacts enhance the opportunity to identify and address potential complications, and studies show that eight or more ANC visits can reduce perinatal deaths by up to 8 per 1,000 births compared to fewer visits. Therefore, the main objective of this study is to assess early ANC booking

Effect of dolutegravir-based first-line antiretroviral therapy on weight and body mass index among adult people living with HIV on follow up at health facilities in Hawassa city administration, Southern Ethiopia: a retrospective cohort study.

BACKGROUND: The nature and burden of weight gain associated with antiretroviral treatment (ART) using a combination of Tenofovir disoproxil fumarate, lamivudine and dolutegravir (TLD) among people living with HIV (PLWH) has not been thoroughly investigated in Ethiopia. Therefore, this study aimed to evaluate changes in body weight and body mass index (BMI) in adults who initiated TLD or switched to TLD compared to those who received a non-nucleoside reverse transcriptase inhibitor (NNRTI)-based therapies. METHODS: A retrospective cohort study was conducted among adult PLWH who had been receiv

Challenges to and opportunities for the adoption and routine use of early warning indicators to monitor pediatric HIV drug resistance in Kenya.

BACKGROUND: Pediatric non-adherence to antiretroviral therapy (ART), loss to follow-up, and HIV drug resistance (HIVDR) are challenges to achieving UNAIDS' targets of 90% of those diagnosed HIV-positive receiving treatment, and 90% of those receiving treatment achieving viral suppression. In Kenya, the pediatric population represents 8% of total HIV infections and pediatric virological failure is estimated at 33%. The monitoring of early warning indicators (EWIs) for HIVDR can help to identify and correct gaps in ART program functioning to improve HIV care and treatment outcomes. However, EWIs

A Qualitative Analysis of Factors Contributing to Cervical Cancer Screening Implementation in Health Care Facilities in Uganda.

IntroductionCervical cancer prevalence and incidence are high in sub-Saharan Africa, especially in Uganda, where screening rates are low, despite government policy recommending screening for all women aged 25-49 every three years. Most prior research focuses on client-related barriers to screening, including women's fears about the screening procedure, lack of partner support, and community-level stigma. Fewer studies elaborate on inner context factors related to health care systems, facilities, and providers. The aim of this study was to use qualitative methods to explore inner context factor

An administrator's perspective on mental health in assisted living.

Administrators of 94 assisted living facilities in Michigan completed a 19-item survey about the extent of mental health problems of residents and how such problems were addressed. In 45 facilities more than half of the residents were reported to be cognitively impaired. The two most common problems were dementia and depression. The most common problematic behaviors were resistance to care and wandering. Two-thirds of the facilities conducted some type of mental health evaluation at admission, and two-thirds conducted evaluations as needed. The most common treatment barriers were patient refus

Implementing Gender-Affirming Care in Correctional Settings: A Review of Key Barriers and Action Steps for Change.

Transgender and gender-diverse (TGD) people are disproportionately impacted by incarceration, interpersonal violence, HIV and other sexually transmitted infections, substance use disorders, and suicidality. Little is known about successful approaches to improve health outcomes for TGD individuals impacted by incarceration. We review the barriers to providing gender-affirming clinical care in correctional systems in the United States, identify key knowledge gaps regarding the provision of gender-affirming care to incarcerated TGD populations, and highlight necessary steps to improve the health

Availability of Health-Related Programs in Private and Public Prisons.

Little is known about the resources available to protect inmates' health in private prisons compared to their public counterparts. This is the first national-level study that exclusively examined the availability of health-related programs in private and public prisons in the United States. We applied propensity score weighting and doubly robust estimation to compare private prisons to comparable public prisons. Data were self-reported by prison administrators as part of the 2005 Census of State and Federal Adult Correctional Facilities. We found that private prisons offered fewer substance de

Interrater reliability of mechanical tests for functional classification of transtibial prosthesis components distal to the socket.

Substantial evidence suggests that the design and associated mechanical function of lower-limb prostheses affects user health and mobility, supporting common standards of clinical practice for appropriate matching of prosthesis design and user needs. This matching process is dependent on accurate and reliable methods for the functional classification of prosthetic components. The American Orthotic & Prosthetic Association developed a set of tests for L-code characterization of prosthesis mechanical properties to facilitate functional classification of passive below-knee prosthetic components.
