From national strategy to local implementation: a policy analysis of eye health integration and financing in Kenya's devolved health system.

BACKGROUND: In 2010, Kenya's Constitution created a devolved governance structure, with county governments assuming responsibility for health service delivery after 2013. Eye health carries a burden, with blindness prevalence at 0.37% and vision impairment affecting 11.0-37.5% across regions. Policy and financing mechanisms shaping service availability remain poorly understood. Kenya has advanced through seven strategic plans, the latest being the National Eye Health Strategic Plan (NEHSP, 2020-2025). While plans provide direction, they have operated without a policy framework, limiting financ

Long-Term Outcomes in Patients on Life-Long Antibiotics: A Five-Year Cohort Study.

BACKGROUND: Little is known about the impacts at an individual level of long-term antibiotic consumption. We explored health outcomes of long-term antibiotic therapy prescribed to a cohort of patients to suppress infections deemed incurable. METHODS: We conducted a 5-year longitudinal study of patients on long-term antibiotics at Monash Health, a metropolitan tertiary-level hospital network in Australia. Adults prescribed antibiotics for >12 months to suppress chronic infection or prevent recurrent infection were included. A retrospective review of medical records and a descriptive analysis w

Assessing health outcomes after extraction of third molars: the postoperative symptom severity (PoSSe) scale.

A postoperative symptom severity (PoSSe) scale was devised from questions commonly used in the clinical assessment of patients who have had third molars extracted, and divided into subscales corresponding to seven main adverse effects that had been identified in a previous study. Ninety-seven patients completed the 15-item questionnaire at one week, and 71 patients at four weeks, after extraction of impacted third molars. After statistical testing, four items were discarded and the final PoSSe scale achieved a high level of internal reliability. Patients' PoSSe scores were highly correlated wi

Considering Health Literacy, Health Decision Making, and Health Communication in the Social Networks of Vulnerable New Mothers in Hawai'i: A Pilot Feasibility Study.

Health literacy is understudied in the context of social networks. Our pilot study goal was to consider this research gap among vulnerable, low-income mothers of minority ethnic background in the state of Hawai'i, USA. Recruitment followed a modified snowball sampling approach. First, we identified and interviewed seven mothers ("egos") in a state-sponsored home visiting program. We then sought to interview individuals whom each mother said was part of her health decision-making network ("first-level alters") and all individuals whom the first-level alters said were part of their health decisi

Development of a personal competency evaluation indicator system for tuberculosis health educators: a scoping review and Delphi study.

OBJECTIVE: To systematically synthesize evidence on the personal competency of tuberculosis (TB) health educators and to develop a scientifically robust and comprehensive evaluation indicator system. METHODS: A two-phase mixed-methods design was employed. Phase 1 involved a scoping review of seven databases (PubMed/MEDLINE, Web of Science, Scopus, Embase, CINAHL, ERIC, and the Cochrane Library) to identify competency-related literature on TB health educators, with no restrictions on publication date, language, or geographic setting. Extracted competency elements were synthesized into a prelim

Relationship between an Oral Health Risk Assessment Using a Salivary Multi-Test System and Woman's Subjective Oral Health Symptoms and Sleep Disorder.

Saliva is used as a diagnosis and monitoring tool for various diseases because it can maintain the balance of the oral ecosystem and reflect the physiological and pathological state of the body. Because women suffer more fatigue than men because of physiological, psychological, and social factors, individual management strategies are needed to evaluate mental health and oral diseases. Therefore, this study examined the oral health risk level from seven saliva factors using a saliva multi-test system for adult women to confirm the possibility of screening for sleep disorders. The saliva of 83 a

Development and content validation of an interprofessional collaboration model for child sexual abuse prevention in primary health care.

BACKGROUND: Child sexual abuse (CSA) remains a significant public health issue requiring effective prevention strategies within primary health care (PHC). Interprofessional collaboration (IPC) is recognized as a key approach; however, context-specific IPC models for CSA prevention are still limited, particularly in low- and middle-income settings. OBJECTIVE: This study aimed to develop and conduct content validation of an interprofessional collaboration model for CSA prevention in primary health care settings. METHODS: The model was developed through a structured narrative literature review

Implementation of Home based management of malaria in children reduces the work load for peripheral health facilities in a rural district of Burkina Faso.

BACKGROUND: Home Management of Malaria (HMM) is one of the key strategies to reduce the burden of malaria for vulnerable population in endemic countries. It is based on the evidence that well-trained communities health workers can provide prompt and adequate care to patients close to their homes. The strategy has been shown to reduce malaria mortality and severe morbidity and has been adopted by the World Health Organization as a cornerstone of malaria control in Africa. However, the potential fall-out of this community-based strategy on the work burden at the peripheral health facilities leve

The comprehensive assessment of health status in survivors of childhood cancer: application to high-risk acute lymphoblastic leukaemia.

The health status of 69 survivors of high-risk acute lymphoblastic leukaemia (ALL) is assessed using a multi-attribute classification system. Seven attributes are included: sensation, mobility, emotion, cognition, self-care, pain and fertility. Three to five levels of functioning are defined for each attribute. Comprehensive health states are described as a specific combination of seven attribute levels. The system captures combinations of sequelae. The system provides a compact but comprehensive tool for long term follow up of survivors of childhood cancer. The results underscore the cognitiv

Ideal cardiovascular health influences cardiovascular disease risk associated with high lipoprotein(a) levels and genotype: The EPIC-Norfolk prospective population study.

BACKGROUND AND AIMS: Lipoprotein(a) (Lp[a]) is a strong genetic risk factor for cardiovascular disease (CVD). The American Heart Association has prioritised seven cardiovascular health metrics to reduce the burden of CVD: body mass index, healthy diet, physical activity, smoking status, blood pressure, diabetes and cholesterol levels (together also known as ideal cardiovascular health). Our objective was to determine if individuals with high Lp(a) levels could derive cardiovascular benefits if characterized by ideal cardiovascular health. METHODS: A total of 14,051 participants of the EPIC-No

Oral bacteria and clinical variables in dependent individuals at a special facility.

The oral health of dependent residents at special facilities has often been reported as being poor, but it is difficult to relate oral health to general health and define the need for oral health care. Microbiological analyses of the oral flora have been suggested as a suitable method for evaluating oral health in this group of patients. A study was performed at a nursing facility where 33 individuals participated. The aim was to describe their oral flora in relation to other health variables and to classify the residents on different risk levels. An oral examination of the residents was made

Controlled trial using computerized feedback to improve physicians' diagnostic judgments.

The goal of this study was to test an innovative method to improve physicians' diagnostic judgments by integrating the use of a computer program (employing cognitive feedback to teach a clinical rule that predicts the probability of streptococcal pharyngitis), a traditional lecture, and periodic disease-prevalence reports. In a controlled trial using pre- and postintervention measures involving 885 patients, the authors compared the effects of the integrated method on the diagnostic judgments of seven experienced physicians at a university health service (from 1982 to 1985) with the effects of

Racial Health Equity Plans in the 30 Largest US Cities.

BACKGROUND: Racial inequities in life expectancy vary significantly across US cities, with city-level gaps ranging from zero to more than 10 years. Given that these inequities are rooted in racism and maintained through social structures and policies, population-level solutions are needed. Local health departments (LHD) are well-situated to lead these types of changes. METHODS: We conducted an environmental scan and document review of formal health plans of the LHDs with jurisdictions covering the 30 most populous US cities. We assessed the inclusion of equity priorities and specific and meas
