Root-Cause Analysis for Enhancing Public Health Emergency Preparedness: A Brief Report of a Salmonella Outbreak in the Alamosa, Colorado, Water Supply.

To demonstrate how public health systems can use root-cause analysis (RCA) to improve learning from critical incidents, the research team utilized a facilitated look-back meeting to examine the public health systems' response to a Salmonella outbreak in the water supply in Alamosa, Colorado. We worked with public health, emergency management agencies, and other stakeholders to identify response challenges related to public health emergency preparedness capabilities, root causes, and lessons learned. The results demonstrate that RCA can help identify systems issues that, if addressed, can impro

Management of the child with a learning disorder.

The label "learning disabilities" is all-embracing; it describes a syndrome, not a specific child with specific problems. Parent involvement in and collaboration with pediatricians and schools in navigating service systems, accessing appropriate services, and case management are particularly relevant for children with LD. Comprehensive service delivery in school settings requires the interface of other service sectors, such as primary care, mental health, and other community agencies that also recognize and address children's problems. To date, marked differences exist among the assessment, cla

Syndromic surveillance using minimum transfer of identifiable data: the example of the National Bioterrorism Syndromic Surveillance Demonstration Program.

Several health plans and other organizations are collaborating with the Centers for Disease Control and Prevention to develop a syndromic surveillance system with national coverage that includes more than 20 million people. A principal design feature of this system is reliance on daily reporting of counts of individuals with syndromes of interest in specified geographic regions rather than reporting of individual encounter-level information. On request from public health agencies, health plans and telephone triage services provide additional information regarding individuals who are part of ap

Expanding the Scope of Forensic and Other Services for Justice-Involved Veterans.

Military veterans are a unique population within the criminal justice system. Numerous criminal justice reforms for this population exist, but there is a dearth of research summarizing and critiquing past and ongoing efforts directed at veterans. This article examines the partnerships developed by the Department of Veterans Affairs with criminal justice agencies to address the needs of justice-involved veterans. Despite these efforts, there is more to be done to support veterans. We provide recommendations for agencies, including enhanced identification of veterans in criminal justice settings

Working with State Health Departments on Emerging Issues in Maternal and Child Health: Immediate Postpartum Long-Acting Reversible Contraceptives.

BACKGROUND: Immediate postpartum long-acting reversible contraceptives (LARC) are highly effective in preventing unintended pregnancy. State health departments are in the process of implementing a systems change approach to better apply policies supporting the use of immediate postpartum LARC. METHODS: Beginning in 2014, a group of national organizations, federal agencies, and six states have convened a LARC Learning Community to share strategies and best practices in immediate postpartum LARC policy development and implementation. Community activities consist of in-person meetings and a webi

Challenges and opportunities for effective adoption of HRH information systems in developing countries: national rollout of HRHIS and TIIS in Tanzania.

BACKGROUND: The establishment of a functional information system for human resource for health (HRH) was one of the major challenges for the Tanzanian health sector. In 2008, the Ministry of Health and Social Welfare developed the HRH Strategic Plan, in which establishment of computerized information systems were one of the strategic objectives. In response to this objective, the Ministry developed two information systems, namely the Human Resource for Health Information System (HRHIS) and the Training Institution Information System (TIIS), to capture information from both the public and priva

Evaluating the Role and Policy Implications of Using External Evidence in Survival Extrapolations: A Case Study of Axicabtagene Ciloleucel Therapy for Second-Line DLBCL.

BACKGROUND AND OBJECTIVE: Health technology assessment (HTA) of haemato-oncology therapies typically requires extrapolation of long-term survival beyond a trial's follow-up. Health technology assessment agencies must balance caution around uncertainty in early follow-up trial data whilst aiming to provide timely access. This study qualitatively and quantitatively assessed how eight HTA agencies considered maturing data and external evidence. METHODS: The eight HTA appraisals were based on ZUMA-7, a phase III trial for axicabtagene ciloleucel (axi-cel) for second-line diffuse large B-cell lymp

A framework for addressing Alzheimer's disease: Without a frame, the work has no aim.

Confronting Alzheimer's disease (AD) involves patients, healthcare professionals, supportive services, caregivers, and government agencies interacting along a continuum from initial awareness to diagnosis, treatment, support, and care. This complex scope presents a challenge for health system transformation supporting individuals at risk for, or diagnosed with, AD. The AD systems preparedness framework was developed to help health systems identify specific opportunities to implement and evaluate focused improvement programs. The framework is purposely flexible to permit local adaptation across

Survey of state health agencies' staff who practice the epidemiology of noninfectious diseases and conditions.

The primary causes of mortality in the United States are noninfectious diseases and conditions. Epidemiologic and intervention activities related to most of these diseases and conditions have increased in most State health agencies over the past decade. Because little was known of the practice of noninfectious disease epidemiology in State health agencies, a mail survey was undertaken in 1991. Persons working in State health agencies who responded to the survey had a graduate degree in epidemiology, biostatistics, or related fields and actively participated in the epidemiology of noninfectious

Challenges to Accessing Newer Antibiotics for Multidrug-resistant Bacterial Infections in Asia: A Literature Review of Multi-Stakeholder Perspectives.

BACKGROUND: Asia faces significant barriers to accessing newer antibiotics despite a high burden of antimicrobial resistance. METHODS: We conducted a literature review to identify key gaps and challenges in access to newer antibiotics in Asian countries. Peer-reviewed and gray literature from January 2010 to October 2024 were searched for articles discussing access to Food and Drug Administration (FDA)-approved antibiotics post-2010 with an Asian perspective. A total of 134 articles were reviewed. RESULTS: Key challenges spanned 7 main stakeholders: pharmaceutical industries, drug regulatory

Prioritisation in speech and language therapy departments in Scotland and Malaysia.

This study investigates how prioritisation in health services have influenced speech and language therapy (SLT) services in Scotland in prioritising their caseload. Additionally, it evaluates how current prioritisation systems may contribute towards the development of the SLT service in Malaysia. Health, education and social agencies in Malaysia were contacted and a literature review was conducted. Information on prioritisation systems used in Malaysia was unavailable. Prioritisation systems from seven SLT departments within Scotland were investigated, of which three SLT managers took part in

Abortion in conflict zones: moral responsibilities of humanitarian agencies in providing safe abortion care.

Unsafe abortion remains a leading cause of maternal mortality globally, disproportionately affecting women in conflict zones where health systems are fractured, legal protections are weakened, and access to reproductive care is severely limited. Armed conflict increases the incidence of unwanted pregnancy through sexual violence, displacement, and disruption of contraceptive services, while also heightening barriers to timely, comprehensive abortion care. Despite growing international recognition of abortion as a life-saving intervention, humanitarian agencies often face legal, cultural, and o

Hurricane Katrina response and guidance for health-care providers, relief workers, and shelter operators.

Hurricane Katrina struck the coastal areas of Alabama, Florida, Louisiana, and Mississippi on August 29, 2005, causing substantial numbers of deaths among both humans and animals, infrastructure damage, and flooding. Affected areas continue to experience shortages of essential services, including electricity, potable water, food, and fuel; damage to health-care and public health systems; and disrupted communications. CDC/ATSDR, local and state health departments, other federal agencies, and other partners are supporting public health and medical-care functions for persons in affected areas and

[Guidelines for investigating clusters of health events].

Clusters of health events, such as chronic diseases, injuries, and birth defects, are often reported to health agencies. In many instances, the health agency will not be able to demonstrate an excess of the condition in question or establish an etiologic linkage to an exposure. Nevertheless, a systematic, integrated approach is needed for responding to reports of clusters. In addition to having epidemiologic and statistical expertise, health agencies should recognize the social dimensions of a cluster and should develop an approach for investigating clusters that best maintains critical commun

Legionellosis on the Rise: A Review of Guidelines for Prevention in the United States.

CONTEXT: Reported cases of legionellosis more than tripled between 2001 and 2012 in the United States. The disease results primarily from exposure to aerosolized water contaminated with Legionella. OBJECTIVE: To identify and describe policies and guidelines for the primary prevention of legionellosis in the US. DESIGN: An Internet search for Legionella prevention guidelines in the United States at the federal and state levels was conducted from March to June 2012. Local government agency guidelines and guidelines from professional organizations that were identified in the initial search were

Quality of Medicare Skilled Home Health for People Living With Dementia in the US: National Patterns and Implications.

BACKGROUND: People living with dementia frequently use Medicare skilled home health care and have unique usage patterns as compared to people without dementia, but little is known about variation in measured quality of home health care received by this population. METHODS: Using 2021 Medicare Fee-for-Service Claims data, we examined receipt of high-quality home health (i.e., care from an agency with a star rating > 3.5) as determined by two publicly available measures: the Quality of Patient Care Star Rating (based on standardized clinical status measures) and the Patient Survey Star Rating (

Bridging the leadership gap: agency training and support shapes non-supervisors' perceptions of their leaders.

BACKGROUND: Strong leadership is essential for government public health agencies to thrive, as it shapes critical factors like organizational culture, workforce engagement, and job satisfaction. Supervisors serve as key pathways to building and sustaining an effective public health workforce. To strengthen public health leadership, it is important to understand the training and support supervisors currently receive and how this influences the experiences of non-supervisory staff. METHODS: This analysis uses data from PH WINS 2024, a national survey of the state and local public health workfor

Human and animal health surveys among pastoralists.

Valid human and livestock health surveys, including longitudinal follow-up, are feasible among mobile pastoralists and provide fundamental information to agencies for interventions that are responsive to realities and effective in addressing the needs of pastoralists. However, pastoralists are often excluded from studies, surveillance systems and health programmes. The occurrence of preventable and treatable diseases such as perinatal tetanus, measles and tuberculosis are indicative of limited access to health providers and information. It is difficult for health services to include effective
