BACKGROUND AND PURPOSE: Laparoscopic Heller cardiomyotomy for the treatment of achalasia can be performed safely. The application of this minimally invasive approach has not been reported in Taiwan. This study assessed the results obtained using this new method in Taiwanese patients.
METHODS: From January 1998 to December 2000, we recruited 10 patients (3 men, 7 women; average age 37.3 yr) with achalasia who underwent laparoscopic cardiomyotomy and fundoplication. Before and after surgery, the severity of three symptoms (dysphagia, regurgitation, chest pain) was evaluated by symptom scores (0

Surgical myotomy is the mainstay of treatment for oesophageal achalasia. Minimally invasive surgical techniques, if feasible, reduce patient morbidity and mortality. In this study we review our experience of thoracoscopic Heller's myotomy. Thoracoscopic myotomy was undertaken in 9 patients (male = 3; female = 6, mean age = 37). All patients presented with dysphagia of 1 to 8 yr duration. Diagnosis was based on barium swallow and manometry. Two patients had previous dilatations and 1 had a transabdominal myotomy. All patients had a 5 port thoracoscopic technique. Thoracoscopic Heller's myotomy

BACKGROUND: Achalasia is a rare esophageal motility disorder with two effective surgical interventions: Heller myotomy with fundoplication (Heller) and peroral endoscopic myotomy (POEM). Although both procedures alleviate dysphagia in the short term, there are limited data on long-term outcomes.
METHODS: We sent Eckardt, Dakkak Dysphagia, and GERD-HRQL questionnaires during four time periods between 2020 and 2025 to patients who had Heller or POEM between 2013 and 2016. We compared the final responses to the two interventions. Next, we analyzed electronic medical records to determine long-ter

BACKGROUND: Robotic surgery benefits include binocular vision, increased dexterity, and adjustable hand control speed. We hypothesized perforation rate of robotic Heller myotomy (RH) was lower than that of laparoscopic Heller myotomy (LH).
STUDY DESIGN: A retrospective cohort study of a prospectively maintained database of 135 patients diagnosed with achalasia or esophagogastric junction outlet obstruction who underwent Heller myotomy by a single surgeon was identified for inclusion. After exclusions, 107 patients were analyzed. The primary outcome was the intraoperative perforation rate. Sec

BACKGROUND: National guidelines suggest routine intraoperative esophagogastroduodenoscopy (EGD) during laparoscopic Heller myotomy (LHM) to assess for mucosal perforation and myotomy adequacy, but the utility of this is unknown. This study aimed to evaluate the effect of intraoperative EGD on outcomes after LHM.
METHODS: Patients who underwent LHM in a single center were retrospectively identified. Outcomes were compared between patients who did and did not undergo intraoperative EGD.
RESULTS: Sixty-one patients were reviewed: 46 (75%) underwent intraoperative EGD and 15 (25%) did not. Mucos

Esophageal achalasia is a primary esophageal motility disorder defined by the lack of esophageal peristalsis, and by a lower esophageal sphincter that fails to relax in response to swallowing. Patients' symptoms include dysphagia, regurgitation, aspiration, heartburn, and chest pain. Achalasia is a chronic condition without cure, and treatment options are aimed at providing symptomatic relief, improving esophageal emptying, and preventing the development of megaesophagus. Presently, a laparoscopic Heller myotomy with a partial fundoplication is considered the best treatment modality. A properl

Blown-out myotomy (BOM) is an underrecognized but increasingly acknowledged long-term structural complication following both laparoscopic Heller myotomy (LHM) and peroral endoscopic myotomy (POEM) for achalasia. Characterized by a localized pseudo-diverticulum or eccentric dilation at the prior myotomy site, BOM often manifests with recurrent dysphagia, regurgitation, and food stasis, and may mimic treatment failure or other anatomic abnormalities. Radiologically, BOM is defined as a > 50% increase in distal esophageal diameter, best visualized on barium esophagogram, with corresponding endosc

BACKGROUND: Achalasia refers to a primary oesophageal motility disorder characterised by the absence of peristalsis and incomplete or complete lack of relaxation of the lower oesophageal sphincter. The cardinal symptom is dysphagia. The therapeutic goal is surgical or interventional repair of the oesophageal outflow tract at the level of the oesophagogastric junction.
INDICATION: We present the case of a 24-year-old patient with dysphagia accompanied by regurgitations, odynophagia as well as an unintentional weight loss over two years.
METHODS: The video describes the preoperative imaging as

AIM: The appropriate management of achalasia in children remains debatable. The present study aimed to evaluate the outcome of laparoscopic Heller myotomy by assessing short- and mid-term issues.
METHODS: Children submitted to surgery from 2002 to 2010 were included. Surgery was proposed after failure of endoscopic dilatations. All patients underwent esophagocardiomyotomy (Heller) plus anterior fundoplication (Dor) by laparoscopy. Demographics, presentation symptoms and perioperative details were analyzed. The outcomes were assessed both by medical records and the application of a 14 items (s

OBJECTIVES: In the early 1990s, minimally invasive videoscopy was applied to numerous operations. After undertaking more than 50 "open" Heller myotomies, our experience with videoscopic Heller myotomy began in 1992. We sought to determine whether the outcome following videoscopic Heller myotomy is influenced by surgeon experience.
METHODS: Seventy-eight patients with severe dysphagia secondary to achalasia underwent videoscopic Heller myotomy between 1992 and 1998. Intraoperative endoscopy was utilized to ensure adequate myotomy in all patients. Patients were stratified into 3 groups: the fir

OBJECTIVE: The goal of this study was to review the authors' results with laparoscopic cardiomyotomy and partial fundoplication for achalasia.
SUMMARY BACKGROUND DATA: Pneumatic dilatation and botulinum toxin (BOTOX) injection of the lower esophageal sphincter largely have replaced cardiomyotomy for treatment of achalasia. After a brief experience with a thoracoscopic approach, the authors elected to perform cardiomyotomy laparoscopically, in combination with a partial fundoplication (anterior or posterior).
PATIENTS AND METHODS: Forty patients were treated between July 1992 and November 199

A 67-year-old female presented with progressive dysphagia to solids and liquids. Imaging was concerning for end-stage achalasia with multiple failed attempts at pneumatic dilation. An outside hospital recommended oesophagectomy, but she came to our clinic for a second opinion. Computed tomography revealed tortuous megaoesophagus measuring up to 7 cm. Barium swallow revealed aperistalsis. Oesophageal manometry revealed an elevated integrated relaxation pressure, 100% failed peristalsis and panoesophageal pressurization in >20% of swallows, consistent with type II achalasia. We recommended a rob

BACKGROUND: Achalasia is an esophageal motility disorder that presents as dysphagia and severely affects quality of life. An esophageal myotomy has been the golden standard for treatment. Peroral endoscopic myotomy (POEM) as a first-line therapy has an acceptable outcome. However, after the clinical failure of POEM, appropriate second-line therapy is rather controversial. Here, we present the first published case in English of a patient who was successfully treated using laparoscopic Heller myotomy (LHM) with Dor fundoplication following an unsuccessful POEM.
CASE PRESENTATION: A 64-year-old

BACKGROUND: Peroral endoscopic myotomy (POEM) has previously been shown to be equally if not more expensive than laparoscopic Heller myotomy (LHM). We compare perioperative outcomes and charges between POEM and LHM at a single institution.
METHODS: Outcomes and charge data of 33 patients who underwent LHM and 126 patients who underwent POEM were analyzed. Patients who did not present electively were excluded.
RESULTS: There were no demographic differences between groups. Patients who underwent POEM had a significantly shorter mean operative time and median length of stay (both p < 0.001). Pa

BACKGROUND: Peroral endoscopic myotomy (POEM) and laparoscopic Heller myotomy (LHM) are the main endoscopic and surgical options for patients with achalasia and other motility disorders that disrupt esophagogastric outflow. This study aimed to assess and compare the rates of decision regret (DR) for patients who underwent LHM and POEM.
METHODS: A prospectively maintained single-institution database was queried for all patients who underwent LHM and POEM between 2011 and 2024. The primary outcome was DR. The secondary outcome was reasons for regret. A multivariate analysis (MVA) was performed
