Three patients with scrotal swelling for 2 days, a year, and many years, respectively, were shown ultimately to have hematoceles even though their clinical history and physical findings were atypical. Contact B-scans of these lesions all showed a basically anechoic mass traversed by thick irregular septations. The septations in one acute case decreased in size and number over 10 days. A 67 year-old male consulted our department for examination a painless left scrotal mass accompanied by a continuously high serum carbohydrate antigen 19-9 level (CA19-9, 563.2 IU/ml). Three infants presented with bluish discoloration and swelling of the scrotum with no history of scrotal trauma. Sonography revealed unilateral hematoceles in 2 cases and bilateral hematoceles in the third. All 3 infants had intact testes. BACKGROUND: Scrotal pain and swelling is a common complaint encountered in emergency medicine. The differential diagnosis includes testicular, scrotal, and intra-abdominal pathology.
CASE REPORT: We present a case of an 80-year-old man on warfarin therapy presenting with acute atraumatic scrotal pain and swelling initially diagnosed as a hydrocele. The diagnosis was subsequently determined to be a communicating hematocele secondary to occult splenic rupture.

A 59-year-old man was admitted with giant tumor of testis and massive hemorrhage into accompanying hydrocele. His clinical status had become worse quickly. The emergency resection of tumor with hematocele was performed. The diagnosis was made upon histological examination of the postoperative tissue and was confirmed by immunohistochemistry. Spermatocytic seminoma was recognized. The surgical specimen comprised a tumor (26 cm x 28 cm x 17 cm) in size, weighed 2150 g. Tumor with hematocele achieved about 40 cm in diameter.

An ectopic tubal pregnancy that undergoes repeated minor ruptures instead of a single episode of rapid bleeding frequently develops into a pelvic hematocele.
