Fatal Distant Venous Epidural Hematoma Following Posterior Cranial Fossa Surgery in a Twelve-Year old Child.

Cranial epidural hematoma is a serious event requiring immediate intervention. This can be due to sudden traction tearing the vessels between the dura and the skull. During posterior fossa surgery, brain collapse may emerge due to the sudden reduction of prolonged elevated intracranial pressure; it could cause dura-skull detachment to create epidural hematoma even far from the surgical site. Hence, we should be aware of this complication when approaching posterior fossa tumors as it frequently leads to severe neurologic impairment or death. Here, we report a 12-year old previously healthy chil

[Significance of morphological changes in the middle meningeal arteries in the development of epidural hematomas after mild cerebro-cranial injury].

Among 58 patients with epidural hematomas in 8 cases they developed following a light cerebro-cranial injury with no fractured cranial bones. In 26 patients who died of diverse causes a histological examination of median meningeal arteries disclosed the presence of focal elastofibrosis, atrophy of the central layer, deposition of lime, cleavage of the elastic membrane, atherosclerosis, thrombogenesis, acute inflammatory process, formation of non-traumatic sacular and arterio-venous aneurysm. These alterations, especially the sacular and arterio-venous aneurysms, may be causative of blood effus

Conservative management of a large temporal epidural hematoma associated with an incidental middle cranial fossa arachnoid cyst: A case report.

BACKGROUND: Epidural hematoma (EDH) is a potentially life-threatening complication of traumatic brain injury typically associated with skull fractures and injury to meningeal vessels. Arachnoid cysts are benign cerebrospinal fluid-filled lesions most frequently located in the middle cranial fossa and are usually discovered incidentally during neuroimaging. The coexistence of an EDH with an underlying arachnoid cyst is uncommon and may influence both the radiological appearance and clinical course. CASE DESCRIPTION: A 27-year-old male football player presented with headache and dizziness after

Postdural Puncture Headache, Subdural Hematoma, and Sixth Cranial Nerve Palsy Lumbar Following Spinal Drain, Resolved with Epidural Blood Patch: Case Report.

BACKGROUND: Postdural puncture headache (PDPH) is a known potential complication of lumbar drain placement. Data regarding incidence of PDPH after lumbar drain placement varies, with a range of 0% to 47%. Furthermore, spinal drain placement has been associated with a significantly increased risk of debilitating neurologic complications. CASE REPORT: In this article, we describe the case of a patient who developed PDPH, subdural hematoma, and abducens nerve palsy after lumbar drain. The patient was treated successfully with epidural blood patch. To our knowledge, this is the first case report

Chronic epidural hematoma of the vertex.

Epidural hematomas of the cranial vertex can be rarely found in patients victims of head trauma. The diagnosis of the vertex hematoma may be delayed by the odd location of the bleeding site and the absence of a clear localization symptomatology. The current method of choice for investigation of head trauma patients, the computed tomography (CT) scans, may also give misleading diagnostic clues. Epidural hematomas of the vertex can be also encountered in a chronic form, and re-bleeding is possibly the underlying mechanism for the long term permanence of the hematoma. We report a case of a patien

Re-formation of acute parietal epidural hematoma following rapid spontaneous resolution in a multitraumatic child: a case report.

Acute epidural hematomas resulting from traumatic brain injury remain among the most common causes of mortality and disability. In the literature, there are cases about the resolution and recurrence of spinal epidural hematomas. This case is characterized by the rapid disappearance and re-formation of an acute cranial epidural hematoma (EDH) associated with no overlying skull fracture. Various authors have reported resolution of EDHs managed conservatively, but rapid resolution and recurrence of cranial EDH was not reported before.

Spontaneous resolution of an acute epidural hematoma with normal intracranial pressure: case report and literature review.

Traumatic epidural hematomas are critical emergencies in neurosurgery, and patients symptomatic from acute epidural hematomas are typically treated with rapid surgical decompression. However, some patients, if asymptomatic, may be treated with close clinical observation and serial imaging. Although rare, rapid spontaneous resolution of epidural hematomas in the pediatric population has even been reported, with only seven cases in the literature. Numerous theories have been proposed to explain the pathophysiology behind these cases, including egress of epidural collections through cranial disco

Abducens nerve palsy associated with a clival epidural hematoma.

A clival epidural hematoma is a rare lesion that usually develops after a hyperflexion or hyperextension injury of the neck, often in a child. A 5-year-old girl presented after a motor vehicle accident with multiple cranial neuropathies, including bilateral abducens nerve pareses and right facial, glossopharyngeal, and hypoglossal cranial nerve palsies. Neuroimaging identified a clival epidural hematoma. The child was observed and the hematoma resolved. The abducens nerve palsies resolved during the ensuing 14 months.

Vertex epidural hematoma: Diagnosis, therapeutic consideration and outcome.

OBJECTIVE: Vertex epidural hematoma (VEDH) is a relatively uncommon type of intracranial hematoma. Because of its unique location and the potential of massive intraoperative bleeding, diagnosis and surgical intervention of VEDH may be challenging. MATERIALS AND METHODS: A retrospective analysis of 32 patients with VEDH was undertaken to investigate the prognostic factor and therapeutic strategy of VEDH. Special attention was paid to the relationship between fracture pattern, surgical method, intraoperative blood loss and outcome. RESULTS: Patients treated surgically had a higher percentage o

[Special features of epidural hematoma in neonates and infants (author's transl)].

Epidural hematomas in infancy, meaning up until the closing of the cranial sutures, have special clinical manifestations and courses; four cases will be demonstrated here. Following an often mild cranio-cerebral trauma a characteristical subperiosteal hematoma lacking primary consciousness disturbance and free interval can develop. The considerable loss of blood coming from the epidural hemorrhage, of mainly venous origin, leads to an often extreme anemia and shock syndrome. Due to the combination of shock and increasing brain compression a fulminant course with sudden coma, respiratory failur

Post-traumatic hematoma of the "transdural" virtual space. A possible cause of incorrect surgical treatment of epidural hematomas.

The space existing between the external and internal dural layer has been anatomically described as a virtual space; no clear clinical significance has been given to it to date. We hereby describe a case of a child with what was expected to be a purely epidural hematoma, at surgery, was found to be composed of two equally coexistent components, one in the epidural space and the second one between the two dural layers. The recognition of a possible involvement of the transdural space in the case of post-traumatic epidural hematomas is relevant for a correct conclusion of the surgical treatment

Delayed post-traumatic large subgaleal hematoma caused by diastasis of rhomboid skull suture on the transverse sinus.

PURPOSE: This study aimed to consider an appropriate treatment for large subgaleal hematoma with skull fracture and epidural hematoma (EDH). CASE REPORT: A 6-year-old boy presented at our hospital with head trauma, and computed tomography (CT) showed a thin EDH in the right temporo-occipital area and cranial diastasis in the right lambdoidal suture. However, no neurological deficits were identified in the patient. One week after trauma, he visited our hospital again with a massive fluctuant watery mass extending from the forehead to the right temporoparietal areas, and laboratory data reveale

Isolated pediatric retroclival epidural hematoma following a minor blunt head trauma: a case report with review of the literature.

Post-traumatic clival hematomas are a rare entity and almost exclusive to the pediatric population. Those of retroclival location are exceptionally rare. This entity was first described in 1941 by Coleman and Thomson, and since then, less than 30 cases have been reported in the literature. Clinically, these hematomas are usually silent and slow, but clinical state worsening may be sudden and rapidly fatal by the onset of obstructive hydrocephalus in the absence of prompt treatment. Here, the authors report a new case of pediatric post traumatic retroclival epidural hematoma following a minor b

Traumatic Middle Meningeal Artery Aneurysm: A Rare Cause of Recurrent Acute Epidural Hematoma. A Case Report.

Traumatic middle meningeal artery aneurysm (TMMA) is a rare condition and a known cause of several different bleeding patterns after head injury. Once detected, they need to be treated as an emergency due to their potential for morbidity and mortality. Generally, recurrence does not occur in surgery for acute epidural hematoma if adequate hemostasis is achieved. Here, we report a case of atypical postoperative recurrence of an acute epidural hematoma, possibly due to the development and rupture of a TMMA. A 41-year-old man with left acute epidural hematoma after a head injury was referred to o
