IMPORTANCE: Acute subdural hematoma (aSDH) and chronic subdural hematoma (cSDH) have traditionally been considered distinct clinical entities. However, a subset of patients with aSDH may subsequently develop cSDH, representing a poorly characterized clinical phenomenon with unclear incidence, outcomes, and risk factors.
OBJECTIVE: To quantify the incidence and outcomes of cSDH-related readmissions following aSDH, identify independent predictors of transformation, and develop and validate a prediction score for risk stratification.
DESIGN: Retrospective cohort study SETTING: Nationwide Readmi

INTRODUCTION: The pathogenesis of chronic subdural hematoma (CSDH) has not been completely understood. However, different mechanisms can result in space-occupying subdural fluid collections, one pathway can be the transformation of an original trauma-induced acute subdural hematoma (ASDH) into a CSDH.
MATERIALS AND METHODS: All patients with unilateral CSDH, requiring burr hole trephination between 2018 and 2023 were included. The population was distributed into an acute-to-chronic group (group A, n = 41) and into a conventional group (group B, n = 282). Clinical and radiographic parameters w

OBJECTIVE: Subdural rhabdomyosarcoma is very rare, and even more unusual is the association between these sarcomas and chronic subdural hematoma. In this report, we present a case of subdural rhabdomyosarcoma that developed in a chronic refractory subdural hematoma.
CLINICAL PRESENTATION: A 40-day-old boy was admitted with coma and intractable seizure after a head trauma caused by a fall. A subdural hematoma was detected, which later became chronic and refractory to treatment.
INTERVENTION: The patient repeatedly underwent different surgical interventions over 2 years. Finally, extensive bil

BACKGROUND AND PURPOSE: Chronic subdural hematoma (cSDH) is a challenging pathology with high recurrence rate after surgical treatment and may seriously affect the patient's quality of life. Membrane formation with angiogenesis plays an important role in the evolution of the disease, providing a promising target for endovascular therapy. Our goal is to categorize angiographic patterns of chronic subdural hematoma for standardized reporting purposes.
METHODS: In our retrospective analysis of prospective data collection, we analyzed angiographic properties of all high recurrence risk patients w

OBJECTIVES: Chronic subdural hematoma (CSDH) is a common neurosurgical problem. Most studies of pathogenesis and treatment involve humans. Advances in understanding of human diseases may be made using animal models. We reviewed all animal models of CSDH and report here their results, conclusions and limitations in order to set a baseline upon which further advanced experimental work related to this disease can be made.
METHODS: PubMed, Medline, Embase and ISI Web of Knowledge were searched with no time limits using the keyword 'chronic subdural hematoma' and MeSH term 'hematoma, subdural, chr

Two adult patients with unilateral chronic subdural hematomas are reported because of the presence of dilatation of the lateral ventricle opposite the hematoma. In both cases, dilatation was limited to the contralateral atrium and the occipital and temporal horns. The selective dilatation, we believe, is explained on the basis of the anatomy of the skull base and the falx. This phenomenon may contribute to an increase of the supratentorial pressure, originally elevated by the hematoma. This characteristic finding on computed tomographic scans should be regarded as critical in cases of chronic

An 86-year-old woman presented with generalized chorea in the face and extremities, which gradually progressed for two weeks. Cranial CT revealed a chronic subdural hematoma (CSDH) that covered the left parietal lobe. Discontinuation of amantadine did not improve the chorea. The hematoma was evacuated and the chorea completely subsided in a week. The pathogenesis leading to chorea in CSDH remains unclear. A unilateral hematoma presenting with generalized chorea similar to the present patient and two others with unilateral CSDH causing ipsilateral hemichorea have been reported. The rarity of th

BACKGROUND: In Tunisia, with the remarkable progress in health, life expectancy has significantly increased these last decades. Indeed, in 2004, 9.3% of the population was aged over 60, and this rate is predicted to reach 17% in 2029 and about 29% by 2050. In the elderly, chronic subdural hematoma (HSDC) may be caused by even minimal trauma or occur spontaneously. The manifestations of this type of accumulation depend on the degree of cerebral compression and the localisation of the mass. They could be delirium, dementia, convulsions or psychiatric disorders (frontal location).

A 75-year-old male presented with bilateral parkinsonism due to chronic subdural hematoma. The hematoma was removed through a small craniotomy. The parkinsonism rapidly improved following operation, suggesting a strong relationship between the hematoma and parkinsonism. We recommend surgical intervention in such cases.

The authors report a case of so-called "infected subdural hematoma" as a complication of chronic subdural hematoma. The patient was a 55-year-old man who had sustained a small laceration of the forehead in a traffic accident on March 29, 1995. No fractures were detected on skull roentgenograms, and general and neurological examinations failed to reveal any abnormal findings. In early August 1995, the patient began to experience headaches, and on August 5 he developed a fever of 38 degrees C. On August 8 he suffered a left motor seizure and was admitted to our hospital. Laboratory studies revea

Few reports describe contralateral subdural hematoma (SDH) after chronic SDH (CSDH) surgery. In this article, we present a rare case of postoperative complication. The patient was a 76-year-old woman with a history of cerebral infarction due to atrial fibrillation and who was taking an anticoagulant. She presented with a disturbance of consciousness on the Glasgow Coma Scale (GCS) E2V5M6. Computerized tomography (CT) revealed a left CSDH; drainage was performed immediately. After surgery, the CT scan revealed an acute SDH (ASDH) on the contralateral side. Three hours later, the hematoma increa

BACKGROUND: Concomitant intracranial chronic subdural hematoma (CSDH) and spinal subdural hematoma (SDH) are rare, and the etiology has yet to be elucidated. However, migration of the hematoma intracranially to a spinal site or coincidence of both intracranial and spinal CSDHs have been proposed as etiologies. We report a case of concomitant intracranial CSDH and spinal SDH in which spinal hematoma might have migrated from the cranial lesion.
CASE DESCRIPTION: A previously healthy 58-year-old man with previous trauma to the occiput and lumbar spine suffered from headache, lumbago, and left he

Bevacizumab (BCZ), commercially known as Avastin, is a monoclonal antibody that targets vascular endothelial growth factor (VEGF). Initially recognized as a breakthrough in oncology, it has since gained FDA approval for various ocular conditions and more recently, for the treatment of glioblastoma multiforme (GBM). Bevacizumab's ability to inhibit excessive neovascularization suggests it may have a potential role in treating chronic subdural hematomas (cSDH). Recent studies have shown that the pathophysiology of cSDH is more complex than previously understood, with VEGF concentrations in subdu

A 87-year old man was admitted to our hospital with decline in cognition and mobility and visual hallucinations. CT scanning revealed a bilateral chronic subdural hematoma. The hematoma was probably caused by a recent fall, while the patient was using acenocoumarol. Chronic subdural hematoma (CSDH) is a frequently occurring intracranial bleeding in the elderly. It usually occurs after a minor trauma and may present with a variety of symptoms. The differentiation between CSDH and other disorders like dementia can be difficult when the hematoma presents with cognitive decline and neuropsychiatic

The uncommon complication of bilateral blindness resulting from a decompressive neurosurgical procedure is presented by the case of a 51-year old patient suffering from a traumatic chronic subdural haematoma. A breakdown of the altered vasoregulation of the optic nerves due to papilledema at the time of intracranial pressure drop is supposed to be the etiological background of optic atrophy and consequent amaurosis. The causal relationship of accident trauma and blindness in the presented case is pointed out.

Chronic subdural hematoma (cSDH) is predicted to become the most common neurosurgical pathology by 2030. Surgical evacuation is a critical component of cSDH management, and postoperative monitoring is important given high recurrence rates and concerns related to restarting anticoagulation. Given issues related to cost, radiation exposure, and time for computerized tomography (CT) and magnetic resonance imaging examinations, we developed a novel technique to detect, assess, and quantify residual and/or recurrent subdural hematoma with point-of-care, hand-held ultrasound and sonolucent burr-hole

Spontaneous intracranial hypotension (SIH) is caused by cerebrospinal fluid leak and has an incidence of approximately five per 100,000 person-years. SIH leads to a range of clinical symptoms, from debilitating postural headaches to subdural hematomas (SDHs). The pathophysiology of the disease is not fully understood, but has garnered interest in recent years. Although CT myelogram and pan-spine MRIs are currently considered the gold standard for diagnosis of SIH, the variable sensitivity (48-76%) and wide range of hematoma size (2-30 mm) warrant further investigation into improved diagnostic

Organized chronic subdural hematoma (OCSDH) is a relatively rare condition that forms over a longer period of time compared to chronic subdural hematoma and is sometimes difficult to diagnose with preoperative imaging. We resected an intracranial lesion in a 37-year-old Japanese man; the lesion had been increasing in size for >17 years. The preoperative diagnosis based on imaging findings was meningioma; however, pathological findings revealed OCSDH. Clinicians should be aware that OCSDH mimics other tumors and consider surgical strategies for this disease.
