Biomaterial-Based and Surgical Approaches to Local Hemostasis in Contemporary Oral Surgery: A Narrative Review.

Effective local hemostasis is essential in oral surgery to prevent complications such as delayed healing, infection, and the need for re-intervention. Postoperative bleeding occurs in 4-6% of cases, increasing to 9-12% in patients receiving anticoagulant or antiplatelet therapy. This review evaluates the efficacy, safety, and clinical utility of local hemostatic agents based on 51 studies published between 1990 and 2023. Traditional agents, such as oxidized cellulose and gelatin sponges, control bleeding in over 85% of standard cases but offer limited regenerative benefits. Autologous platelet

Hemodilution and surgical hemostasis contribute significantly to transfusion requirements in patients undergoing coronary artery bypass.

OBJECTIVE: We sought to determine the incidence of and risk factors for the development of low intraoperative hematocrit levels and of excessive postoperative bleeding in patients undergoing coronary artery bypass grafting, whether the risk factors are the same, and their effect on blood product transfusions. METHODS: We performed a prospective cohort study of 613 adult patients who underwent coronary artery bypass grafting in 3 tertiary, university-affiliated hospitals during the period from October 1, 2000, to March 31, 2001. RESULTS: Low intraoperative hematocrit levels (<19%) were found

Biomimetic recombinant collagen with multicopy dual-targeted hemostatic motifs for enhanced hemostasis in surgical trauma.

Collagen has superior bioactivity in hemostasis but is limited by a few active amino acids and noncustomizable motifs. Here, we propose a biosynthetic strategy to construct multicopy dual-targeted recombinant collagen. Using human collagen type III alpha 1 chain (COL3α1) as a template, we integrated hemostatic motifs GFPGER and GPP, which bind integrin α2β1 and glycoprotein VI to enhance platelet adhesion, activation, and red blood cell (RBC) capture. The multicopy design further improves binding affinity, stability, and hemostatic activity. The recombinant collagen significantly promotes plat

Application of calcium sulfate in surgical-orthodontic treatment of impacted teeth: a new procedure to control hemostasis.

PURPOSE: Calcium sulfate (CaS) is a simple, biocompatible material with a very long, safe clinical history in several different fields of medicine. CaS is a rapidly resorbing material that leaves behind a calcium phosphate lattice, which promotes bone regeneration and hemostasis. The aim of this study was to perform a clinical evaluation of a possible hemostatic effect of calcium sulfate hemihydrate (CaSO(4)), commonly known as plaster of Paris, in the surgical and orthodontic treatment of impacted teeth. MATERIALS AND METHODS: Forty-three patients with 66 impacted cuspids and premolars were

Balancing anticoagulation and hemostasis in an infant with severe hemophilia A during cardiac transplantation: Review of the literature and development of a surgical protocol.

Hemophilia A is a disorder resulting in a deficiency of clotting factor VIII that can lead to life-threatening bleeding. Evidence-based guidelines for surgical interventions like cardiac surgery are limited. Anticoagulation is necessary for cardiac bypass, thus risk of bleeding in a patient with hemophilia is increased and requires careful attention to maintain hemostasis. We report the first infant with severe hemophilia A and dilated cardiomyopathy who underwent successful cardiac transplantation, and review the literature on previous cardiac transplant cases in congenital hemophilia. To ens

[OPEN SURGICAL HEMOSTASIS FOLLOWING TRANSARTERIAL EMBOLIZATION (TAE) FOR RENAL INJURY AFTER EXTRACORPOREAL SHOCK WAVE LITHOTRIPSY (ESWL): A CASE REPORT].

We report a case of open surgical hemostasis following transarterial embolization (TAE) that failed to stabilize the hemodynamics for renal injury after extracorporeal shock wave lithotripsy (ESWL). A 48-year-old man presented with severe left renal colic pain 1 day after ESWL for a left renal stone. Computed tomography revealed arterial bleeding from the lower pole of the left kidney and retroperitoneal hematoma. TAE was successfully performed for the lower poler bleeding. However, we were unable to complete the procedure for bleeding from an aberrant artery to the lower pole of the kidney th

Editorial Commentary: Hemostatic Agents, Surgical Hemostasis, and Hypotensive Anesthesia May Conserve Irrigation Fluid and Improve Visualization During Shoulder Arthroscopy.

Intraoperative bleeding impedes visual clarity during shoulder arthroscopy and leads to longer operative times, greater use of irrigation fluid, and greater blood loss. Epinephrine in the irrigation fluid and intravenous tranexamic acid are common interventions that have been found to maintain hemostasis and improve visual clarity during arthroscopy. Intravenous carbazochrome sodium sulfonate may also offer similar benefit during shoulder arthroscopy, but more robust study of effectiveness and safety as well as Food and Drug Administration approval are needed before it can be recommended for u

Injectable Stimuli-Responsive Amphiphilic Hydrogel for Rapid Hemostasis, Robust Tissue Adhesion, and Controlled Drug Delivery in Trauma and Surgical Care.

Uncontrolled bleeding in trauma and surgical settings requires rapid, minimally invasive materials that can effectively stop bleeding and provide durable wound sealing. Here, we introduce an injectable, pH-responsive amphiphilic hydrogel designed for quick hemostasis, strong wet-tissue adhesion, and controlled therapeutic release. The hydrogel is prepared via a mild nucleophilic substitution reaction between tertiary amines of Poly(2-(dimethylamino)ethyl methacrylate (PDMA) and gallic acidfunctionalized branched polyethyleneimine PEI(GA), using chloride-terminated Pluronic F-127 (Cl-Plu-Cl) as

[Hemorrhagic shock. A new concept of endoscopic hemostasis and prospects of the non-surgical treatment of ulcer and gastroduodenal hemorrhages complicated by hemorrhagic shock].

A new direction with the use of endoscopic hemostasis (EH) was offered in the complex program of metabolic rehabilitation of patients with ulcerous gastroduodenal bleedings (UGDB) and hemorrhagic shock (HS). There was a study of the results of treatment of 44 patients (29 men and 15 women at the age from 61 to 77) with severe concurrent diseases and UGDB complicated with HS. Recurrences of UGDB in case of HS are a factor of high operative-anesthetic risk. Endoscopic preventive metabolic hemostasis is a prospective direction in rehabilitation of patients with UGDB complicated with HS. It is nec

Vigilant surgical hemostasis challenged by ketamine or hypervolemia induced hemodynamic stress to minimize postoperative hemorrhage in patients with parasagittal/or parafalcian meningiomas: an interventional study.

BACKGROUND: Prevention of postoperative intracranial hemorrhage, a serious and sometimes fatal complication remains the cornerstone for successful neurosurgical procedures. The aim of this study was to test the hypothesis whether vigilant rehemostasis challenged by ketamine or hypervolemia-induced hemodynamic stress could minimize postoperative hemorrhage. METHODS: During a five-year period between 1995 and 2000, in an interventional study, 37 patients having parasagittal or parafalcian meningiomas were randomly allocated into three groups. After accomplishing tumor resection and securing ini

Surgical and procedural bleed risk stratification for anticoagulated patients undergoing planned surgery: guidance from the ISTH SSC Subcommittee on Perioperative and Critical Care Thrombosis and Hemostasis.

Patients often need to interrupt anticoagulation for invasive procedures or surgery. Periprocedural bleeding can contribute to substantial morbidity and mortality. Procedural bleed risk stratification informs whether anticoagulation needs to be interrupted, for how long, and when to restart anticoagulation after procedure. Guidance on procedure-specific bleeding risk varies and contributes to discordant perioperative anticoagulation management. To address this important knowledge gap, the Perioperative and Critical Care Thrombosis and Hemostasis Subcommittee of the International Society on Thr
