Thrombocytopenia is a lesser known but significant complication of heparin therapy. Heparin-dependent, antibody-mediated thrombocytopenia may be accompanied by hemorrhagic or thrombotic sequelae. Thrombosis is more frequent and is associated with significant morbidity and mortality. Diagnostic laboratory findings include a variable reduction in the platelet count during heparin administration and the presence of heparin-dependent platelet-associated antibodies that can induce in vitro platelet aggregation. Discontinuing heparin is the primary treatment, although other therapies may be used. Th

The extensive use of therapeutic doses of heparin to prevent thrombosis in critically ill patients with COVID-19 during the pandemic has led to an increased incidence of bleeding and heparin-induced thrombocytopenia (HIT). In addition, the introduction of the AstraZeneca and Johnson&Johnson vaccines against COVID-19 into clinical practice was associated with the development of a rare but very severe, adverse thrombotic complication, vaccine-induced immune thrombotic thrombocytopenia (VITT). Thrombotic complications of VITT turned out to be similar to HIT both clinically and pathophysiologi

Heparin-induced thrombocytopenia (HIT) is a growing complication of a common medication used to prevent deep vein thrombosis (DVT) in hospitalized patients. The purpose of this article is to review the mechanism that causes paradoxical thrombus formation in HIT and ways to recognize this important complication with various testing modalities and to discuss the approaches to treatment once a diagnosis has been made. HIT is a clinical diagnosis that can be further supported by utilizing the "4 Ts": thrombocytopenia, timing of platelet count fall, thrombosis or other complications, and other caus

Heparin-induced thrombocytopenia (HIT) is not only a common but also a potentially serious drug adverse effect. Unlike other drug-induced thrombocytopenias, HIT does not usually cause bleeding, but instead causes thrombosis. Thrombosis in HIT can lead to limb gangrene (requiring leg amputation) or even death. HIT is mediated by an antibody that recognizes an epitope on the platelet factor 4 (PF4)-heparin complex. The antibody-PF4-heparin complex binds to FcgammaRII receptors on the platelet surface and cross-links the receptors. This induces intense platelet activation and platelet aggregation

Thrombocytopenia is a common adverse effect of heparin therapy. Two types of heparin-induced thrombocytopenia (HIT) are observed clinically--an early onset mild thrombocytopenia (Type I) in which the patients remain asymptomatic and a delayed onset severe thrombocytopenia (Type II). Patients with Type II HIT have an increased risk of thrombotic complications which frequently cause crippling disability e.g. limb amputation or even death. Type I HIT, the commoner of the two types, is believed to be due to the platelet proaggregating effect of heparin itself but Type II HIT is generally agreed to

Cutaneous necroses, acute renal failure and an adrenocortical insufficiency appeared during heparin therapy in a 47 year old patient with myocardial infarction. The thrombocytes fell to 73,000/microliter and displayed intensified spontaneous aggregation. In histological investigation of the cutaneous necroses, intensive thromboses of the cutaneous and subcutaneous blood vessels were found. The findings indicate a thromboembolic etiology of the acute renal failure and the adrenocortical insufficiency. This case involves a complex example of a heparin-induced thrombopenia with thromboembolic com

The major side effect of heparin when administered therapeutically is bleeding. There is now considerable evidence that the antithrombotic and the hemorrhagic effects of heparin can be dissociated by using low molecular weight heparins, and heparinoids, and by using heparin with low affinity to AT III. The current evidence supports the suggestion that the hemorrhagic effect of heparin is contributed to by a reversible platelet functional defect which is relatively less important than its AT III dependent anticoagulant effect for preventing experimental venous thrombosis. Whether these promisin

OBJECTIVE: To evaluate heparin re-exposure practices and safety in patients with prior HIT.
BACKGROUND: Heparin-induced thrombocytopenia (HIT)-associated antibodies are transient, often declining to undetectable levels within approximately 3 to 6 months. Therefore, patients with a remote history of HIT are unlikely to retain immune memory to heparin.
METHODS: This single-center retrospective study evaluated patients with prior HIT admitted between October 1, 2018, and September 30, 2024. HIT was defined as a positive serotonin-release assay result; positive platelet factor 4 immunoassay resu

The activated partial thromboplastin time (APTT) assay is essential for evaluating coagulation. However, prolonged APTT may result from pathological conditions or contamination with unfractionated heparin (UFH), which can occur due to improper phlebotomy or use of heparinized lines-even in patients not on UFH therapy. Distinguishing between true pathology and heparin contamination is critical for patient care and appropriate laboratory reporting. This study assessed the effectiveness of Antihepca HRRS (AHCa, a Heparin Resistant Recalcifying Solution (HRRS)), in neutralizing heparin interferenc

OBJECTIVES: Heparin-induced thrombocytopenia is a known complication of heparin exposure with potentially life-threatening sequelae. Direct thrombin inhibitors can be substituted for heparin in patients with heparin-induced thrombocytopenia that require anticoagulation. However, the use of direct thrombin inhibitors as a substitute for heparin has not been widely reported in the neuroendovascular literature.
MATERIALS AND METHODS: Here we report the first use of the direct thrombin inhibitor bivalirudin in a neuroendovascular procedure as a substitute for heparin in a patient with a ruptured

The use of prosthetic grafts in below-knee bypasses may be necessary in patients with no available autologous vein and critical limb ischemia not amenable to angioplasty. Such conduits, however, have generally yielded disappointing results. A new, heparin-bonded, expanded polytetrafluoroethylene graft (Gore Propaten vascular graft) designed to provide resistance to thrombosis may be associated with decreased early graft failure and increased patency. A concern with exposure to heparin, and therefore heparin-bonded prostheses, is the development of heparin-induced thrombocytopenia (HIT). Althou

We studied the effects of phytolaccosides, saponins from Phytolacca americana, on the intestinal absorption of heparin in vitro and in vivo. The absorption enhancing activity of these compounds (phytolaccosides B, D2, E, F, G and I) was determined by changes in transepithelial electrical resistance (TEER) and the transport amount of heparin disaccharide, the major repeating unit of heparin, across Caco-2 cell monolayers. With the exception of phytolaccoside G, all of them decreased TEER values and increased the permeability in a dose-dependent and time-dependent manner. In vitro, phytolaccosid

The specificity of endothelial binding sites for heparin was investigated with heparin fractions and fragments differing in their Mr, charge density and affinity for antithrombin III, as well as with heparinoids and other anionic polyelectrolytes (polystyrene sulphonates). The affinity for endothelial cells was estimated by determining I50 values in competition experiments with 125I-heparin. We found that affinity for endothelial cells increases as a function of Mr and charge density (degree of sulphation). Binding sites are not specific receptors for heparin. Other anionic polyelectrolytes, s

The hallmark of heparin-induced thrombocytopenia (HIT) is the presence of immunoglobulin G (IgG) antibodies against platelet factor 4/heparin (PF4/H) complexes, typically detected by PF4/H enzyme-linked immunosorbent assay (ELISA); thus, negative ELISA results are commonly used to exclude this diagnosis. Here, we report a prevalent yet previously unrecognized subset of antibodies that are undetectable by PF4/H ELISA (ELISA-) but activate platelets in the PF4-dependent P-selectin expression assay (PEA+). In 11 patients with clinically confirmed HIT who tested positive in both PF4/H ELISA and pl

We present a case of an 82-year-old female with a significant medical history of hypertension and Alzheimer's disease who developed heparin-induced hemorrhagic bullous dermatosis during treatment for a subsegmental pulmonary embolism. The patient was admitted with lower extremity edema and cyanosis, diagnosed with a subsegmental pulmonary embolism, and started on therapeutic doses of unfractionated heparin. On the sixth day of heparin therapy, she developed abdominal bloating and a diffuse exanthematous rash, which progressed to hemorrhagic bullae on the plantar and dorsal aspects of her feet,

Heparin-induced thrombocytopenia (HIT) requires clinical evaluation and laboratory testing using immunological assays with confirmation by functional assays such as serotonin release assay (SRA). The HIT-IgG chemiluminescence immunoassay (CLIA) is a rapid immunological assay; we aimed to assess the effect of heparin on this assay. The effect of low-dose (0.1 U/ml) and high-dose (100 U/ml) heparin on the CLIA HIT-IgG assay was evaluated, these concentrations aligning to those used for SRA. A cohort of available pretested patient material (plasma or serum) comprised samples negative for HIT-IgG

The interaction of unfractionated heparin (UFH) with universal heparin reversal agent 7 (UHRA-7) is investigated. UHRA-7 is composed of a hyperbranched polyglycerol core onto which an array of methylated tris(2-aminoethylamine) (Me-TREN) charged groups is grafted, which in turn are shielded with a layer of small chain poly(ethylene glycol) methyl ether (mPEG) chains. This system has previously been shown to be biocompatible and to be effective at neutralizing heparin. The binding constant Kb was determined from isothermal titration calorimetry experiments, at temperatures ranging from 278 to 3
