An unusual outcome of a giant ventral hernia.

Hernias are routine general surgical problems that may present in any age group, regardless of the patient's socioeconomic status. We present a rare case of a complicated ventral hernia leading to short bowel. This is an unusual case and is very rarely reported in the literature. This current case report describes a 54-year-old gentleman who presented to the hospital with a giant strangulated ventral hernia causing massive bowel ischemia and resulting in a short bowel. The literature on large abdominal wall hernias leading to short bowel is reviewed, and a discussion on short bowel syndrome is

The laparoscopic ventral and incisional hernia repair.

We retrospectively reviewed the first twenty laparoscopic ventral and incisional hernia repairs performed at the 2nd Department of Surgery, St. Anne's Teaching Hospital, Brno from March 2003 till April 2004. Laparoscopic repairs were performed using Biomesh mesh (Cousin Biotech, France) with a minimum of 3 cm overlap circumferentially with normal fascia and secured with spiraltacks every 3-4 cm and transparietal sutures. All adhesions on the parietal defect are released, using scissors, and whenever possible we attempted to reduce the hernia sac. The mean follow-up period was 7 months with a r

Retrorectus Ventral Hernia Repair Utilizing T-line Hernia Mesh: Technical Descriptions.

The T-line hernia mesh is a synthetic, polypropylene mesh with mesh suture extensions designed to prevent anchor point failure by evenly distributing tension across the soft tissue. Previous studies have demonstrated the success of onlay ventral hernia repair with T-line hernia mesh, but retrorectus applications of the mesh have not yet been characterized. This technique article illustrates technical descriptions and clinical applications of the T-line hernia mesh in the retrorectus plane.

Hernia-to-neck ratio is associated with emergent ventral hernia repair.

BACKGROUND: The ratio of hernia size to fascial defect size, termed the hernia-to-neck ratio (HNR), has been proposed as a novel predictive factor for umbilical hernia complications. HNR ≥ 2.5 has been suggested to warrant surgery due to association with bowel strangulation, incarceration, and necrosis. The aim of this study was to evaluate the association between HNR and emergent ventral hernia repair at our institution. METHODS: A retrospective cohort study was performed of consecutive patients with ventral hernias evaluated at a large safety-net hospital from 2017 to 2019. Patients who req

Port Site Hernias Following Laparoscopic Ventral Hernia Repair.

BACKGROUND: Port site hernias (PSH) are underreported following laparoscopic ventral hernia repair (LVHR). Most occur at the site of laterally placed 10-12-mm ports used to introduce large pieces of mesh. One alternative is to place the large port through the ventral hernia defect; however, there is potential for increased risk of surgical site infection (SSI). This study evaluates the outcomes when introducing mesh through a 10-12-mm port placed through the hernia defect. METHODS: This was a retrospective case series of patients who underwent LVHR in three prospective trials from 2014-2017 a

Incidence and prevention of ventral incisional hernia.

OBJECTIVE: Ventral incisional hernia is a common complication of abdominal surgery. The incidence ranges from 2% to 20% and varies greatly from one series to another. The goal of this study was to determine the incidence, risk factors, and preventive measures for ventral incisional hernia. MATERIALS AND METHODS: An analysis of the surgical literature was performed using the search engines EMBASE, Cochrane Library, and PubMed with the keywords: abdominal hernia, wound dehiscence, incisional hernia, incidence, trocar site hernia, and hernia prevention. RESULTS: The overall incidence of incisio

[Laparoscopic repair of abdominal ventral hernias].

This paper presents the surgical technique for ventral abdominal hernia repair, including median incisional hernia, umbilical hernia and epigastric hernia. The main stages of the surgical procedure are as follows: pinpointing the parietal defect, insufflation of pneumoperitoneum and placing the trocars, inspection and adhesiolysis of the peritoneal cavity, closure of the defect with extracorporeal transparietal U reverse stitches, preparing the mesh, introducing the mesh in the peritoneal cavity and fixing it with transfascial sutures and tackers. Postop care measures, postop complications and

Ventral hernia repair by the laparoscopic approach.

An analysis of these results indicates that laparoscopic hernia repair can be performed safely by experienced laparoscopic surgeons, and with lower perioperative complication rates than for open hernia repair. Although the follow-up period for the laparoscopic repair is only 2 or 3 years, the recurrence rate is likely lower than with open repair. Most patients with ventral hernias are candidates for this laparoscopic repair if safe access and trocar placement can be obtained. The choice of mesh often provokes a debate among surgeons, but little practical difference in the results seems to exis

Ventral hernia in the treatment of omphalocele and gastroschisis.

Twenty-three patients with gastroschisis or omphalocele undergoing delayed ventral herniorrhaphy were reviewed. The ventral defects resulted from elective skin flap coverage in 15 patients, prosthetic silo failure in five, and nonoperative management using escharotic agents in three. Herniorrhaphy was performed at 2 months to 15 years of age. Fascial closure was achieved in a single procedure in 15 patients (65%) and with multiple operations involving prosthetic material in five patients (22%). Three patients (13%) had prosthetic material left in their defects without sequelae. There was no op

[Planned Ventral Hernia After Open Abdomen Therapy: Complex Incisional Hernia Repair].

A planned ventral hernia after open abdomen therapy is a rare hernia entity because the fascial closure rate has been increased due to established concepts for open abdominal treatment. Nevertheless, fascial closure is not always successful, and a planned ventral hernia has to be treated later. Preoperative optimisation and planning are essential for such challenging abdominal wall repairs. In a single centre retrospective analysis, all incisional hernias from 2013 to 2023 (n = 632) were identified and planned hernias after a laparostomy were selected (n = 11). The data on surgical management w

Intraluminal Mesh Migration After Ventral Hernia Repair.

Hernia repair is the most commonly performed operation by general surgery. We present a case of intraluminal mesh migration into the transverse colon and distal ileum after ventral hernia repair. An ileo-transverse anastomosis was performed after the mesh was removed and the abdominal wall was reconstructed. This case demonstrates a rare but possible complication after hernia repair with mesh. In these patients, physicians should keep a high index of suspicion for mesh migration when all other causes of GI symptoms have been ruled out.

Cecal bascule after laparoscopic ventral hernia repair.

INTRODUCTION: We present a case of a cecal bascule, a rare type of cecal volvulus, as a complication of a laparoscopic ventral hernia repair. CASE DESCRIPTION: A 73-y-old male with a ventral hernia underwent an uneventful elective laparoscopic repair. He developed an acute abdomen on postoperative day 6, and imaging demonstrated a cecal bascule. Exploratory laparotomy revealed a cecal bascule with an ischemic and perforated cecum, and a right hemicolectomy was performed. DISCUSSION: Laparoscopic tension-free ventral hernia repairs have become more common, especially for obese patients. We di

Current status of laparoscopic ventral hernia repair.

Ventral abdominal wall hernias are a common problem for the general surgeon. Historically, the best results have been obtained with the open Rives-Stoppa approach. This is done by fixing a large piece of prosthetic mesh behind the rectus muscle. Extensive dissection is required and can lead to postoperative pain and wound complications. A laparoscopic approach allows similar mesh placement with minimal dissection. Several small comparative studies have found laparoscopic ventral hernia repair to have fewer complications, a shorter length of stay, and possibly a lower recurrence rate when compa

[Intestinal obstruction in extensive postoperative ventral hernia].

The work is based on an experience with surgical treatment of 1017 patients. Intermittent intestinal obstruction was observed in more than 40% of them and was not always dependent on the character of commissural processes. The most frequent cause of its appearance is a combination of commissures with a considerable ptosis and formation of flexions, angular deformities and incomplete torsion of intestinal loops. Supermedial and subcostal hernias with most pronounced commissural processes are comparatively rarely followed by intestinal obstructions (12.5%). It was more often observed in middle-

"Slim-Mesh" Technique for Giant Ventral Hernia.

BACKGROUND AND OBJECTIVE: We devised a sutureless "Slim-Mesh" technique to treat ventral hernias, including large-giant/massive ones, reduce intra- and postoperative complications, and lower operation time. METHODS: Between September 1, 2009 and October 31, 2020, 43 patients with large (10 - 14.9 cm)-giant (15 - 19.9 cm) and massive (≥ 20 cm) ventral hernia were operated at our Department with the above technique. This was a prospective (79%)-retrospective study. RESULTS: This study comprised 22 males and 21 females. Mean age was 63 years. Large-giant and massive hernias were found intraoper

Robot-Assisted Transabdominal Preperitoneal Ventral Hernia Repair.

BACKGROUND AND OBJECTIVES: We believe that complications due to the mesh used in ventral hernia repairs can be reduced by using the natural barrier afforded by the peritoneum. This can be challenging to do laparoscopically, however we felt that the robot-assisted laparoscopic approach reduces the difficulty in placing the mesh in the preperitoneal space, and we want to share our early experiences with this approach. We describe the surgical technique used in robot-assisted laparoscopic transabdominal preperitoneal (TAPP) ventral hernia repair with mesh. In addition, we evaluate its feasibility
