Salvage of failed hip osteosynthesis for fractures with modular hip prosthesis.

BACKGROUND/PURPOSE: Treating options of failed internal fixation include revision fixation and salvage arthroplasty. The purpose of this study was to evaluate the results, complications and the final functional outcome associated with modular hip arthroplasty. METHODS: Modular arthroplasty was performed in 11 patients due to failed treatment of peritrochanteric fracture. Each patient examination included Trendelenburg test, Harris Hip Score, SF-36, and X-rays of the hip. Examinations were recorded and a paired t test was applied for further statistic analysis. RESULTS: Two years postoperativ

[Arterial vascular injuries in hip prosthesis implantation].

Implantation of total endoprosthesis (TEP) is an established method for the operative therapy of acute and chronic hip dysfunction. Beside septic complications iatrogenic injuries of the vascular system regularly occur. Immediate and adequate therapy is necessary in order to prevent loss of an extremity or even patients death. We present three patients with major vascular injuries during hip joint operations. In two patients was the external iliac artery and in one the femoral common artery injured. The cases were managed with PTFE- or Dacron-interposition. The problem of vascular injuries ass

Cementless total hip arthroplasty with PCA prosthesis.

Eighty-one cases of cementless porous-coated total hip arthroplasties in 74 patients were analyzed. Hip rating scores as described by Harris were used, and AP and lateral radiographs were made serially: preoperatively, 3 months, 6 months, 1 year, and 2 years postoperatively. The average postoperative hip rating score was 91 points (range: 76 to 100 points) at 1 year and 93 points (range: 74 to 100 points) at 2 years. The overall clinical results were satisfactory in these patients at both 2 and 4 years. However, slight to mild thigh pain was observed in 16 (19.8%) of 81 hips at 2 years, and 10

[Periarticular ossification. Complication of total hip prosthesis].

The authors analyse peri-prosthetic ossification, a complication which can occur in the presence of certain risk factors. Grades I and II calcifications are frequent and do not interfere with the final post-operative result. On the other hand, grades II and IV result in a progressive stiffening of the coxo-femoral joint within one year after the operation. They may be bilateral and can sometimes be responsible for serious handicap. These cases should be re-operated in order to restore joint mobility, but surgical treatment should be completed by radiotherapy according to a defined protocol. No

[CT evaluation of total HIP prosthesis infection].

PURPOSE: To evaluate the ability of CT imaging to detect infectious lesions in total hip prosthesis. PATIENTS AND METHODS: CT was performed in 17 cases of suspected total hip prosthesis infection. Images were reviewed to evaluate for the presence of soft tissue abscess, hip joint effusion or fistula and peri-prosthetic lucency or erosion at bone window setting. The feasibility of CT imaging is assessed with regards to metallic artifacts. RESULTS: Infection was confirmed in 10 hips. A soft tissue abscess was present in 8 of these cases, in association to a joint effusion, presumably septic, i

[Traumatic indications for total hip joint prosthesis].

In the years 1978-1986 the authors implanted 207 total prostheses in patients with the fracture of the femoral neck or after the fractures in the area of the hip joint. 50 patients were operated on for an acute fracture of the neck. The surgical technique in these indications does not differ on principle from the surgical technique of coxarthrosis. They point out a higher number of some peroperative complications. They checked 141 patients with and average follow-up of 47 months. In 4.2 per cent of late complications they reported roentgenological symptoms of loosening. Reoperation because of

[Peri-prosthesis fractures in total hip endoprostheses].

Periprosthetic fractures after total hip arthroplasty (THA) were found in 0.77% (33 fractures in 4280 THA). At the time of fracture the prosthesis was loose in about one-third of the cases. In most cases (63%), the fracture was located at the distal end of the prosthesis. Fractures at this location were transverse or short oblique; those situated more distally were spiral. Whereas those located along the prosthesis shaft were long oblique. Open reduction internal fixation was performed immediately for all fractures. Thirty patients were followed an average of 2.7 years (1 year-11 years) after

Total hip replacement in inflammatory hip disease using the Charnley prosthesis.

The Charnley low-friction arthroplasty was performed on 73 hips in 55 patients with inflammatory joint disease, usually for intolerable pain. Sixty-five of these operations were reviewed at a special clinic or by postal questionnaire; 57 hips were painfree or gave rise to only occasional discomfort, in 58 there was an increased range of movement, and in 46 of the 59 hips assessed clinically, the results were excellent or good. The most serious complication was deep infection, which occurred in 7 operations.

[Infected hip joint prosthesis. Viewpoints for 1-stage and 2-stage prosthesis exchange].

When treating an infected hip prosthesis, there are a number of medical, technical and biomechanical considerations. No ideal solution for all problems is available. In individual cases an optimal compromise must be found. The means for curing the infection and the one for maintaining hip stability can counteract each other. Basically, three options are available: removal of the prosthesis without replacement and one- or two-stage replacement. With all three options thorough surgical debridement is vitally important. The introduction of antibiotic cement and cementless titanium implants, toget

Treatment of dysplastic and congenitally dislocated hips with the Zweymueller total hip prosthesis.

Uncemented Zweymueller total hip prostheses were implanted in 35 dysplastic or dislocated hips in 33 patients. Sixteen hips were dislocated and 19 hips were dysplastic; in 12 hips, an intertrochanteric or pelvic osteotomy was performed in early childhood. In all cases, the titanium screw socket was implanted at the level of the original cotyloid cavity. Osteotomy of the greater trochanter, shortening osteotomy, or roof acetabuloplasty were not performed. In cases in which the femoral cavity was too narrow for the Zweymueller stem, an anterolateral longitudinal window-shaped osteotomy was perfo

Cobalt hip prosthesis intoxication mimicking an autoimmune disease.

Cobalt-containing hip prosthesis may cause systemic toxicity due to the release of cobalt from metal-on-metal (MoM) joint arthroplasty into the bloodstream. High cobalt blood levels can lead to a variety of clinical manifestations, mimicking other disorders, especially autoimmune, hematologic, and infectious diseases. Our purpose is to describe a clinical case of cobalt hip prosthesis intoxication mimicking an autoimmune disease, with systemic inflammation signs, arthro-myalgias unrelated to overt synovitis, and multiple autoantibody positivity. A 69-years-old woman presented with a 1-year his

[Main concepts and approaches in total hip prosthesis].

MAIN CONCEPTS AND APPROACHES IN TOTAL HIP PROTHESIS. Hip prothesis are composed of an acetabular cup and a femoral stem articulating with each other to reproduce an artificial spherical joint. If the geometry of the cups between the different manufacturers is similar, it differs more importantly between the different types of femoral stems. Cemented prothesis can be distinguished from cementless prothesis, each of which has its own specificities, without one of these two techniques being able to show its superiority in functional or mechanical long-term results. Different types of femoral head

Actinomyces gerencseriae hip prosthesis infection: a case report.

INTRODUCTION: Actinomyces bacteria are part of the human oropharyngeal microbiota. They have been associated with abdominal, cervicofacial and thoracic infections and a few cases of joint infections have also been described. In particular, Actinomyces gerencseriae, formerly described as Actinomyces israelii serovar II, has rarely been associated with human infections, mostly involving cervicofacial lesions and periodontal diseases. Here, we report one case of hip prosthesis infection due to A. gerencseriae. CASE PRESENTATION: A 72-year-old Caucasian male developed an inflammatory collection o

[Experience with hybrid hip prosthesis walter-motorlet/poldi.].

In 1987-1992 at the Orthopaedic Clinic IPVZ 108 hybrid endoprostheses of the hip joint, type Walter-Motorlet/Poldi were implanted (non-cemented socket Walter-Motorlet, cemented shank Poldi). The authors submit evaluations of 75 endoprotheses made 2-7 years after surgery (mean follow-up period 3 and a half years). The mean age of the patients was 49 years (range 39-62 years), the most frequent preoperative diagnosis was secondary postdysplastic coxarthrosis (52%), other diagnoses were primary coxarthrosis (24%), idiopathic necrosis of the head of the femur (20%) and posttraumatic coxarthrosis (

[Butel's isoelastic hip prosthesis. A limited prospective study].

INTRODUCTION: A limited prospective study was undertaken at the Centre Hospitalier de l'Université Laval to assess the outcome of 21 isoelastic prostheses described by Butel. MATERIAL AND METHODS: The femoral component consisted of a one-piece forged stainless steel stem with four metallic rods ranging in diameter from 11 to 18 mm, in 1 mm increments. The two lateral and medial rods, 20 cm in length, were linked distally. The 2 mm gap between the medial and lateral rods was obliterated at insertion, allowing the component to be firmly seated in the femoral canal. Twenty-one Butel isoelastic p
