The use of medicinal leeches in the salvage of flaps with venous congestion.

The use of medicinal leeches (Hirudo medicinalis) for the salvage of tissues with venous congestion has been intermittent over the last two centuries. During the last decade, interest in leech therapy has undergone a resurgence. Hirudo's tricuspid bite injects a highly potent anticoagulant. The site usually bleeds for 1 to 2 hours and under special circumstances may bleed for up to 24 hours. This collective series presents four patients in whom immediate postoperative venous congestion threatened the survival of three tissue replants and one latissimus dorsi flap. Two of the replants were comp…

Changes in the Frequency of Rhythmic Excitation of Retzius Cells during Thermal Stimulation of Leech Skin.

Thermal stimulation of various parts of the skin in Hirudo medicinalis increases the frequency of spontaneous rhythmic excitation of Retzius neurons in leech ganglia. It was shown that the frequency of spontaneous rhythmic excitation of Retzius cells in the segmental ganglion increases only in response to thermal stimulation and returns to initial values upon cooling. This effect was also detected in neurons that are not directly connected by nerve fibers with the particular skin area. Changes in the frequency of spontaneous rhythmic excitation of Retzius cells in the segmental ganglion were o…

Internal and External Morphological Characteristics of the Medicinal Leech Species Hirudo sulukii and Hirudo verbana.

OBJECTIVE: The aim of this study was to investigate the internal and external morphological structures of two species of medicinal leeches (Hirudo sulukii and Hirudo verbana). METHODS: Leech specimens were collected from various regions of Turkey between the years of 1995-2016. The leeches were first examined alive and later were stunned with 10% ethyl alcohol and fixed in 70% ethyl alcohol or 4% formaldehyde. After fixation, the internal morphology of leeches was examined by dissecting them under a stereo microscope. RESULTS: Both species of leeches are dorso-ventrally flattened. H. verbana

A group of related surface glycoproteins distinguish sets and subsets of sensory afferents in the leech nervous system.

The distribution of 4 surface glycoproteins on axons of peripheral neurons was studied in the leech Hirudo medicinalis through monoclonal antibodies. All 4 glycoproteins have a similar molecular weight of 130 kDa. Immunohistochemical localization of these glycoproteins on tissue sections of nerves and neuropil reveals tracts of afferent axons organized as nested sets. Their distribution suggests a possible role for these molecules in mediating axon fasciculation.

Use of the medicinal leech in the treatment of ear diseases.

Medicinal leeches (Hirudo medicinalis) and the native secretion of their salivary glands diluted with saline (5 times) were successfully used for the treatment of some ear diseases: tinnitus caused by inner-ear affections, acute external otitis and chronic otitis media. The effect of diluted leech saliva injected into the region of the mastoid process by microelectrophoresis was 25-30% lower than that of the medicinal leeches.

Medicinal leech therapy (hirudotherapy): a brief overview.

Hirudotherapy is a treatment using medicinal leeches. Hirudo medicinali, have been used to treat patients for centuries. In the past, leeches have proved to be an effective treatment for a number of conditions including battle wound treatment. Currently leeches may be used to assist in the treatment of abscesses, arthritis, glaucoma, myasthenia gravis, thrombosis and some venous disorders. Medical leeches may also be used in plastic surgery and in some blood circulatory problems. During feeding, leeches secrete a complex mixture of different biologically and pharmacologically active substances

Thrombotic microangiopathy as a complication of medicinal leech therapy.

Medicinal leeches (hirudo medicinalis) are increasingly used as a useful therapeutic option in various medical and surgical settings. The potential complications associated with this therapy include infections with Aeromonas species, bleeding, anemia, and allergic reactions. We describe a patient who developed thrombotic microangiopathy and acute renal failure following leech therapy. It is proposed that the series of nonhuman proteins within leech saliva may be capable of injuring the endothelium.

The leech nervous system: a valuable model to study the microglia involvement in regenerative processes.

Microglia are intrinsic components of the central nervous system (CNS). During pathologies in mammals, inflammatory processes implicate the resident microglia and the infiltration of blood cells including macrophages. Functions of microglia appear to be complex as they exhibit both neuroprotective and neurotoxic effects during neuropathological conditions in vivo and in vitro. The medicinal leech Hirudo medicinalis is a well-known model in neurobiology due to its ability to naturally repair its CNS following injury. Considering the low infiltration of blood cells in this process, the leech CNS…

The role of leech water sampling in choice of prophylactic antibiotics in medical leech therapy.

Medical leech therapy (MLT) with Hirudo medicinalis is well established as a treatment for venous congestion of tissue flaps, grafts, and replants. Unfortunately, this treatment is associated with surgical site infections with bacterial species, most commonly Aeromonas hydrophila, which is an obligate symbiot of H. medicinalis. For this reason, prophylactic antibiotics are recommended in the setting of MLT. After culturing Aeromonashydrophila resistant to ciprofloxacin from a tissue specimen from a patient with a failed replant of three digits post-MLT, we performed environmental surveillance …

Blood Relatives: Linking Evolutionary History and Conservation of Medicinal Leeches (Hirudo spp.).

Medicinal leeches of the genus Hirudo are both historically important in medicine and increasingly studied as models in ecology and biomedicine. However, the taxonomy, population structure and evolutionary history of this genus remain partially unresolved. Here, we present the most comprehensive phylogenetic analysis of the Hirudo complex to date, incorporating 350 COX1 sequences (103 newly generated) and 247 complete mitogenomes from across the distribution range. In Hirudo verbana, three major clades corresponding to Iberian (A), Western (B) and Eastern (C) haplogroups were found, with sub-h

HIRUDOTHERAPY IN RECONSTRUCTIVE SURGERY: CASE-REPORTS AND REVIEW.

Medicinal leeches (Hirudo medicinalis, Hirudo verbana) have been used in the field of medicine to treat various diseases for thousands of years. Popularity of their use changed over time and in Europe, it peaked at the beginning of the 19th century. In modern medicine, application of leeches on flaps with venous congestion was first used and described by Deganc and Zdravic in 1960. A certain renaissance of leech use is currently taking place, especially in the field of reconstructive surgery. In general, use of leeches is indicated during critical post-operative period, in which the microcircu…

Production, Purification, and Characterization of a Novel Cysteine-Rich Anticoagulant from the Medicinal Leech and the Functional Role of Its C-Terminal Motif.

The saliva of the medicinal leech Hirudo medicinalis contains a wide range of biologically active compounds, including multiple anticoagulants. Previously, we identified a novel cysteine-rich anticoagulant protein (CRA) from leech saliva and produced it recombinantly in Escherichia coli, demonstrating its potential as a basis for new anticoagulant drugs. In this study, we developed an optimized procedure for scalable production and purification of recombinant CRA. The purified protein was analyzed for common contaminants originating from E. coli, such as endotoxins, bacterial proteins, and DNA…

A case of irritant contact dermatitis with leech.

Medicinal leeches are invertebrated parasites that have been used for medical purposes for centuries as one of the traditional treatment methods. Medicinal leeches are used for wound and flap healing in the plastic and reconstructive surgery, in the treatment of venous insufficiency, varicosities, hemorrhoids, and in many other diseases. However, due to medicinal leech therapy (hirudotherapy) complications such as anaphylaxis, local allergic reactions, anemia, and mucosal synechiae can occur. In this article, we report a case of an irritant contact dermatitis in a patient who experienced sever

Cutaneous complications of improper leech application.

INTRODUCTION: The medical leech (Hirudo medicinalis) has been used throughout the centuries and continues to be used today, mainly in reconstructive surgery and microsurgery. Easy access to these animals may entail an improper use of this therapeutic method by patients as a form of self-treatment. CASE REPORT: A man who presented with skin erythema and oedema due to the application of a medical leech. DISCUSSION: Infection is considered the most common complication of hirudotherapy, even though bacteriological examination of leech applied by the patient showed neither bacterial nor parasitic…

Antibiotic sensitivities of Aeromonas hydrophila cultured from medicinal leeches.

Increased use of medicinal leeches (Hirudo medicinalis) for the treatment of venous congestion in flaps and replanted parts has coincided with reports of soft tissue infections following leech application. We cultured the gullets of 20 medicinal leeches to re-examine the antibiotic sensitivities of Aeromonas hydrophila, the leech enteric organism associated with reported infections. These isolates reflected reported resistance to penicillin and first generation cephalosporins as well as sensitivity to gentamicin, tetracycline and chloramphenicol. Additionally, the cultures were sensitive to ce…

Reimagining the "Eastern Aspirin": Medicinal leech derivatives in multidimensional modern therapeutics.

Despite aspirin being hailed as the cornerstone of modern medicine, its"ceiling effect" in antithrombotic therapy-including aspirin resistance, bleeding risk, and the lack of protection for end organs-has emerged as a severe clinical challenge. Against this backdrop, medicinal leech therapy, with a legacy spanning thousands of years, is undergoing a scientific transformation enabled by modern pharmacology and evidence-based medicine. Hirudin and other leech derivatives have collectively been described as the natural 'Eastern Aspirin' have been acclaimed as the natural "Eastern Aspirin". This r

Clinical and scientific considerations in leech therapy for the management of acute venous congestion: an updated review.

Any surgical intervention that involves the manipulation of veins, large or small, carries the risk of acute venous congestion. Venous congestion is the product of an imbalance between arterial inflow and venous outflow, and results in the stasis of blood in the tissues that are normally drained by the affected veins. The resultant lack of tissue perfusion causes hypoxia, acidosis, and arterial thrombi formation, which can potentially progress to tissue necrosis and wet gangrene. In the past several decades, the use of leeches (Hirudo medicinalis) has been rediscovered as an effective method o…
