Maximizing decision-making style and hoarding disorder symptoms.

BACKGROUND: Past research links hoarding disorder (HD) to indecisiveness and difficulty with decision-making. However, it remains unclear what contributes to difficulty making decisions in HD. Decision-making research suggests that some individuals have a maximizing decision-making style (seeking the best option through an exhaustive search of all existing alternatives) while others "satisfice" (choosing options that are satisfactory even without seeing all options). Past work has linked the dispositional tendency to maximize in decisions to elevated depression, anxiety and obsessive-compulsiv

[Diogenes syndrome and Hoarding disorder: Same or different?].

INTRODUCTION: In 2013, the American Psychiatric Association published the DSM-5. In this new version, new diagnoses were proposed including the Hoarding disorder. In the French semeiology, the Diogenes syndrome is described, among other symptoms, by a pathological tendency to accumulate objects called syllogomania which is very close to hoarding. This paper explores the similarities and differences between the two syndromes. DESCRIPTION: The Diogenes syndrome was first described in 1966 but was officially named for the ancient Greek philosopher in 1975 by Clark. Its frequency is around five f

Panic attacks and hoarding disorder: an initial investigation.

Panic attacks (PAs) defined as a discrete period of intense fear or discomfort, occur in the context of numerous anxiety and mood related disorders. Research has suggested that PAs serve as a significant indicator and prognostic factor for overall symptom severity, course, and comorbidity within various conditions. Consequently, a PA specifier is now applicable to all DSM-5 disorders. Despite these clinical and nosological implications, no research to date has examined associations between PAs and hoarding disorder. The current investigation evaluated relationships between PA endorsement and h

Neural mechanisms of decision making in hoarding disorder.

CONTEXT: Hoarding disorder (HD), previously considered a subtype of obsessive-compulsive disorder (OCD), has been proposed as a unique diagnostic entity in DSM-5. Current models of HD emphasize problems of decision-making, attachment to possessions, and poor insight, whereas previous neuroimaging studies have suggested abnormalities in frontal brain regions. OBJECTIVE: To examine the neural mechanisms of impaired decision making in HD in patients with well-defined primary HD compared with patients with OCD and healthy control subjects (HCs). DESIGN: We compared neural activity among patients

Prevalence of hoarding disorder among primary care patients.

OBJECTIVES: Despite the inclusion of hoarding disorder (HD) in the DSM-5, there is little epidemiological data on hoarding from low and middle-income countries. This study, the first from India, examines the prevalence and correlates of HD among primary care patients in the state of Kerala, India. METHODS: To assess correlates, the Hoarding Rating Scale-Interview (HRS-I) and other structured instruments were administered to 7,555 subjects selected by stratified random sampling from 71 primary health centers. RESULTS: The prevalence of HD was 1.02% (95%CI 0.8-1.3). Those with HD were more lik

Electrophysiological dynamics of visuocortical processing in hoarding disorder.

Individuals with hoarding disorder (HD) typically perform worse than peers on neuropsychological tasks involving visual perception. Functional neuroimaging shows diffusely increased activity in the visual cortex, consistent with inefficient visual processing in HD. The temporal locus of these inefficiencies in HD is unknown. This study examined the temporal unfolding of visual event-related brain potentials (ERPs) to help better define the neurophysiological mechanisms underlying visual dysfunction in HD. Thirty-three individuals with HD and 35 healthy controls (HC) were assessed using a 64-ch

Applying attachment theory to indecisiveness in hoarding disorder.

Research shows that individuals who experience distress when discarding their possessions are more indecisive than individuals who do not experience such difficulty. These individuals report more intense emotional responses and greater intolerance to distress when faced with a discarding task. The aim of this study was to determine whether an insecure attachment style contributes to indecisiveness among individuals with discarding difficulties and whether this association is mediated by emotional reactivity and distress intolerance. This study used a within-group cross-sectional design. One hu

The Role of Intrusive Imagery in Hoarding Disorder.

Despite the incidence of trauma in the histories of people with Hoarding Disorder (HD), reexperiencing symptoms, namely intrusive images, have not been investigated in the condition. To address this, 27 individuals who met DSM-5 criteria for HD and 28 community controls (CCs) were interviewed about (a) their everyday experiences of intrusive imagery, and (b) the unexpected images they experience when discarding high- and low-value possessions. Compared to CCs, everyday images described by the HD group were more frequent, had a greater negative valence, and were associated with greater interfer

Hoarding disorder and cerebellum damage. A case study.

OBJECTIVES: The introduction of infliximab biosimilars in the US has helped curb health care costs through competitive pricing. Many insurance payers have mandated switches from the originator to an infliximab biosimilar citing cost savings. This study assessed the cost savings to insurance payers and patients associated with switching infliximab products using a commercial claims database. STUDY DESIGN: This is a retrospective cohort study of commercially insured patients who received multiple infliximab products between 2015 and 2021 using administrative claims data. METHODS: Infliximab ou

Hoarding and its relation to obsessive-compulsive disorder.

Although hoarding is observed in some patients with obsessive-compulsive disorder (OCD), it has not been shown to share a specific relation with OCD. Across two studies, we found that (a) whereas the classic OCD symptoms of checking, rituals, and contamination intercorrelated consistently strongly with one another, hoarding related only moderately to both these OCD symptoms and to depression; (b) OCD patients were distinguished from both other patients and non-patients by classic OCD symptoms, but not by hoarding; and (c) whereas OCD symptoms showed consistent relations with Negative Affect, h

The Hoarding Phenomenon in Schizophrenia Spectrum Disorders.

The presence of hoarding behavior among patients with schizophrenia has been known for more than a century. Nevertheless, only a few studies have examined the hoarding phenomenon among patients with schizophrenia, and no studies have examined the potential motivation. Hoarding disorder became a separate diagnosis in DSM-5. Studies about hoarding disorder use primarily quantitative approaches (e.g., scales, self-administered questionnaires, and structured interviews) when assessing the patients. The main objectives of this study were to examine the meaning of hoarding for patients with schizoph

From the Cluttered Room to the Full Hard Drive: The Relationship Between Hoarding Disorder and Digital Hoarding.

The rapid expansion of digital information has given rise to new forms of hoarding that share features with hoarding disorder, yet their underlying psychological mechanisms remain insufficiently understood. Although hoarding disorder (HD) has been extensively studied, digital hoarding (DH) has received far less empirical attention within the behavioral sciences. This study examines whether the factors associated with hoarding disorder also account for digital hoarding and whether hoarding disorder influences digital hoarding. Using partial least squares structural equation modeling (PLS-SEM),

Obsessive-compulsive disorder with hoarding behavior: unravelling key differences.

OBJECTIVE: Hoarding disorder remains underexplored and poorly understood despite its significant impact on individuals and communities. While historically linked to obsessive-compulsive disorder (OCD), emerging evidence suggests they may be distinct conditions. This study aims to investigate these differences. METHODS: Adults aged 18-88 years old (M = 39.86, SD = 14.86) were part of the Genomic Psychiatry Cohort study, including 1247 individuals with OCD without presumed hoarding disorder (OCD-pHD) and 663 individuals with OCD and presumed hoarding disorder (OCD+pHD). Multifaceted impulsivity in obsessive-compulsive disorder with hoarding symptoms.

OBJECTIVE: Hoarding is common in obsessive-compulsive disorder (OCD), and OCD with hoarding may have poorer prognostic features than OCD without hoarding. The aim of this study was to investigate the relationship between multifaceted impulsivity and hoarding symptoms in individuals with OCD. This relationship is important to be able to understand the psychopathological mechanisms of hoarding symptoms in OCD patients. METHODS: The study included 136 individuals with OCD classified as OCD with high hoarding symptoms (OCDwHH, n = 41) and OCD with low/none hoarding symptoms (OCDwLH, n = 95), toge

Examining subjective sleep quality in adults with hoarding disorder.

Hoarding disorder (HD), characterized by difficulty parting with possessions and functionally impairing clutter, affects 2-6% of the population. Originally considered part of Obsessive-Compulsive Disorder (OCD), HD became a distinct diagnostic entity in the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) in 2013. While sleep impacts OCD, little is known about sleep in HD. As HD patients often report poor sleep in clinical settings, understanding global subjective sleep quality and disturbances may lead to novel therapeutic targets. To address this gap, the authors used a sample o

Understanding DSM-5 Hoarding Disorder: A Triple Vulnerability Model.

OBJECTIVE: Whereas a triple vulnerability model has been used to evaluate several anxiety- and mood-related conditions, no research has purposefully examined hoarding from this perspective. METHOD: Using a multivariate approach, the current study evaluated the unique relations between hoarding and a general biological (e.g., neuroticism), psychological (e.g., anxiety sensitivity), and hoarding-specific vulnerability (e.g., saving cognitions) within 526 individuals. RESULTS: A structural equation model (SEM) including all three vulnerabilities as well as hoarding symptoms provided good fit to

An exploration of economic reasoning in hoarding disorder patients.

Current models of hoarding disorder (HD) emphasize problems of decision-making. Evidence for neuropsychological impairment in HD, however, has been mixed. The present study examined whether HD patients show problems of economic reasoning that could be associated with decision-making problems. Forty-two HD patients, 29 obsessive-compulsive disorder (OCD) patients, and 36 healthy control participants completed the Iowa gambling task (IGT), a computerized card playing game that assesses participants' ability to learn and utilize a rule of sacrificing short-term gain for long-term gain, and a cogn

The distinctiveness of compulsive hoarding from obsessive-compulsive disorder.

The present study investigated the relation of compulsive hoarding to other obsessive-compulsive disorder (OCD) symptoms in a sample of 162 patients with OCD. Obsessions and compulsions reported on the Anxiety Disorders Interview Schedule for DSM-IV were submitted to an exploratory factor analysis. Results suggested a four-factor model: "Certainty," "Contamination," "Obsessions," and "Numbers/Ordering." Hoarding did not load on any factor. The sample was divided into three groups: pure hoarding, nonhoarding OCD, and mixed OCD and hoarding. The hoarding group endorsed significantly less anxiety

Reaction time variability and attentional impairment in hoarding disorder.

Attention appears to be impaired in hoarding disorder (HD), although it remains unclear whether this can be conceptualized as co-occurring ADHD or intrinsic to hoarding pathology. Self-reported cognitive impairment tends to be inconsistent in HD, with individuals tending to overestimate cognitive impairment compared with performance on neuropsychological tests. A behavioral index of attentional impairment during decision making, ex-Gaussian modeling of reaction times, has been commonly used as an index of inattention in ADHD, with more skew (i.e., higher tau values) suggestive of attentional l

State of the Science: Hoarding Disorder and Its Treatment.

This state-of-the-science review describes the relatively new diagnosis of hoarding disorder (HD), characterized by difficulty discarding possessions and resulting clutter in living spaces. We review current theoretical models of HD, including a cognitive-behavioral model, a biopsychosocial model, an attachment model, and an addictions model. We then describe interventions for HD, focusing largely on cognitive-behavioral therapy for hoarding disorder (CBT-HD), the only treatment with conclusive evidence of efficacy. We review the components of CBT-HD and their rationale, noting that clinical r
