The outcomes of surgical treatment for lateral Hoffa fracture nonunions.

INTRODUCTION: Lateral Hoffa nonunion are rare injuries. A significant percentage of these nonunions are due to missed acute lateral Hoffa fractures. Operative management of these injuries is difficult and complicated by the presence of bone loss, infection, and soft-tissue contractures. In this study, the aim was to assess clinical and functional outcome in our group of patients with lateral Hoffa nonunion who had undergone operative management and to determine whether variables such as fracture type, infection, and previous surgery affect nonunion/complication rates. MATERIALS AND METHODS: D

Non-union lateral femoral condyle Hoffa fracture: a case report.

An isolated Hoffa fracture of either of the femoral condyle is infrequently encountered. Owing to its rarity, it is often missed and subsequently results in non-union. The knowledge of its surgical management comes from the sporadically reported cases in the literature. The most preferred approach is parapatellar, providing adequate exposure of the fracture and the joint surface. We report a case of non-union of Letteneur-2C lateral condyle Hoffa fracture of the left distal femur in a young adult as a result of missed bony injury. Surgical fixation was done using a posterolateral approach to t

Hoffa fracture: should precautions be taken during fixation and rehabilitation?

A coronal fracture of a femoral condyle (Hoffa fracture) is an unusual injury and there are only a handful of reports discussing it. We report a case of a 52-year-old worker who fell from a height, suffering lower limb injuries, including a Hoffa fracture with comminution, and had problems with malunion during the postoperative period. Clinicians should be aware that rehabilitation programmes need to be tailored to the method of fixation used to manage this uncommon fracture pattern.

A case report of the Hoffa fracture and a review of literature.

The Hoffa fracture is an uncommon fracture. There is a lot of confusion about its diagnosis and management with several conflicting reports in literature. We reported a 25-year-old patient with non-union of Hoffa fracture, and meanwhile tried to develop an algorithm-based treatment for Hoffa fractures. A systematic review of the available literature was performed. Medline, Embase, the Cochrane Library and PubMed were searched for relevant articles. Fifty-five articles were reviewed, and the clinical knowledge base was summarized. The understanding of the mechanism of trauma has become more nua

Irreducible, incarcerated vertical dislocation of patella into a Hoffa fracture.

Rotational dislocations of patella, which involve rotation of the patella around a horizontal or vertical axis are rare. These rotational dislocations of patella are difficult to reduce by close methods. These dislocations can have associated osteochondral and retinacular injury. We report a case of a 20-year-old male who presented with swelling and pain in the right knee following a motor cycle accident. Radiological evaluation using the computed tomography revealed a patellar dislocation with a concomitant Hoffa fracture. Patella was rotated around the vertical axis and was incarcerated into

Total Knee Arthroplasty in Pseudarthrosis of Hoffa Fracture: A Case Report.

CASE: A 55-year-old woman presented with chronic left knee pain and functional disability 1 year after nonoperative treatment of Hoffa fracture of lateral femoral condyle, due to pseudarthrosis and consequent knee osteoarthritis. She underwent single-stage fracture fixation and primary total knee arthroplasty with successful outcome at her 4-year follow-up. CONCLUSION: The combination of Hoffa fracture fixation and primary total knee arthroplasty can achieve satisfactory results in terms of pain relief, functional improvements, and avoid complications related to a 2-stage procedure. This case

Plate fixation for Letenneur type I Hoffa fracture: a biomechanical study.

BACKGROUND: A coronal fracture of the posterior femoral condyle, also known as a Hoffa fracture, is an unusual injury, and there are only a handful of case reports or series exploring it. The optimal fixation method of these intraarticular fractures remains controversial; improper or unstable fixation usually lead to an unsatisfactory prognosis. The use of posterior-anterior or reversed lag screw fixation is still a popular method. Additional buttress plating is also recommended for fixation of these difficult fractures. The purpose of this study was to compare the mechanical strength of four

Arthroscopic Management of a Posterior Femoral Condyle (Hoffa) Fracture: Surgical Technique.

Unicondylar fractures of the lower end of the femur are uncommon injuries that usually occur in the sagittal plane. A coronal (tangential) plane fracture, first described by Hoffa in 1904, is unusual. It is an intrinsically unstable type of intra-articular fracture that warrants operative fixation, usually by an open technique. A simple method for the treatment of lateral femoral condyle coronal fractures with arthroscopic-assisted reduction and internal fixation by cannulated screws is reported. Managing fractures of the posterior femoral condyle with arthroscopic reduction and fixation with

Bicondylar Hoffa fracture: open reduction internal fixation using the swashbuckler approach.

Coronal plane fractures of the femoral condyle are infrequent injuries and are often missed. Unilateral bicondylar coronal plane fractures are even rarer, with only eight reported cases in scientific literature, and are often associated with other injuries. We present here a case of unilateral bicondylar Hoffa fracture that presented in our emergency department and was managed with open reduction and internal fixation by lateral parapatellar arthrotomy using the swashbuckler approach with satisfactory results.

The Hoffa-like fracture of the tibial plateau: a clinical study.

BACKGROUND: Given the contrapositive link between the posterior tibial plateau and the femoral condyle and the similar injury cause, the fracture type should be the same as the well-known Hoffa fracture of the femoral condyle. This study aims to report a case series with Hoffa-like tibial plateau fractures to improve understanding. METHODS: We analysed six consecutive patients presented with Hoffa-like tibial plateau fractures in our hospital between October 2014 and December 2020, a period in which 1924 tibial plateau fractures were treated. Patients' data on demographics, radiographs, surgi

[Hoffa fractures : Rare, often overlooked, prone to complications].

Hoffa fractures are coronal plane fractures of the femoral condyle and overall represent a rare fracture entity. These fractures are predominantly caused by high-energy trauma und usually involve the lateral femoral condyle. An axial load to the femoral condyle with the knee in 90° or more of flexion produces the typical fracture pattern. In the initial radiological diagnostics the fracture is often overlooked, therefore, a computed tomography (CT) examination is indicated for the diagnostics and planning of surgery (selection of the access and the implant). The operative treatment with an equ

Hoffa Fractures: Current Understanding and Surgical Treatment Strategies.

Coronal fractures of the distal femur, although uncommon in isolation, represent a distinct fracture pattern different from other distal femur fracture subtypes. Popularly known as the Hoffa fractures, these injuries are characterized by obliquely oriented fracture lines in the coronal plane with or without articular comminution. These partial articular fractures are commonly seen on the lateral condyle as a result of translational shearing forces on the posterior femoral condyle with the knee in flexion. These fractures were often missed before the advent of routine use of CT scans because th

Neglected Busch-Hoffa fracture following the patellar dislocation: a rare case report.

INTRODUCTION AND IMPORTANCE: Busch-Hoffa fractures are rare intra-articular coronal plane fractures of the femoral condyle that often remain unnoticed, particularly when complications develop. Due to their instability and risk of long-term problems, these fractures require immediate anatomical reduction and stable fixation. The occurrence of a Busch-Hoffa fracture in conjunction with patellar dislocation is extremely rare and has been rarely reported in the current literature. CASE PRESENTATION: We report a case involving a 21-year-old male who experienced a Bagaria type 2 Hoffa fracture of t

Non-Union of Isolated Medial Condyle of Femur Hoffa Fracture: Case Report.

Isolated non-united Hoffa fracture of the femur is a rare finding. They are often missed due to the nature of the fracture and when not assessed appropriately. This is a case report of a 40-year-old male who encountered a high-velocity trauma; the fracture was probably missed on plain radiographs following the trauma. The patient presented to us eight months following the trauma with complaints of pain and decreased range of motion of his right knee (10 to 80 degrees of flexion) and the patient was unable to bear weight on the affected limb. On evaluation, the patient was found to have a non-u

Distal femoral osteotomy for genu valgum deformity caused by malunited Hoffa fracture.

Hoffa fractures are unstable intra-articular fractures of the femoral condyle that occur in the coronal plane.Insufficient anatomical reduction and internal fixation may lead to non-union or malunion. A 39-year-old man was involved in a traffic accident while riding a motorcycle and was diagnosed with left Hoffa fracture and avulsion fracture of the femoral attachment of the medial collateral ligament. Open reduction and internal fixation were performed 5 days after injury. The patient experienced intermittent knee pain, joint contracture and deformity, and attended our hospital for further tr

Surgical management of complex intra-articular distal femoral and bicondylar Hoffa fracture.

Bicondylar Hoffa's fractures of distal femur are rare. We report the case of an 18-year-old male who sustained a closed complex fracture of both femoral condyles of the left femur on the sagittal plane (Hoffa fracture), combined with distal metaphyseal fracture. He was treated using parallel plates and interdigitating screws, with our technique based on an extrapolation of the principles of distal humerus fixation. The fracture united clinically and radiologically at 16 weeks. Twelve months postoperatively, the range of movement in the knee was 5°-115°. In this complex case, our technique prov

A rare case of open bicondylar Hoffa fracture with extensor mechanism disruption.

The incidence of open bicondylar Hoffa fractures is extremely rare. We report one such case of a 42-year-old woman who presented to the emergency department with an open injury over the knee. Imaging revealed bicondylar Hoffa fracture. The patient was taken up for debridement and internal fixation. Intraoperative findings included an entrapped patella between the fracture fragments and extensor mechanism disruption. Hoffas fracture was fixed with lag screws and patellar tendon repaired on to the inferior patella. The patient was started on early postoperative range of motion exercises. The fra
