Supporting Health Care Resilience Through "Reflexive Spaces" in Home Care Services: A Multiple Embedded Case Study.

OBJECTIVES: The aim of this study was to explore where and how managers facilitate arenas for collective reflections and knowledge sharing ("reflexive spaces") in homecare services during the COVID-19 pandemic. Moreover, we sought to understand how these "reflexive spaces" contributed to adaptations to challenges induced by the pandemic. Finally, we aimed to discuss how these spaces might incorporate resilience into health care. METHODS: This multiple embedded case study includes interviews with health care staff (n=16) and managers at different system levels (n=21) from 4 Norwegian municipal

Loneliness Among Older Adults Receiving Long-Term Home Care Services: A Quantitative Study.

INTRODUCTION: Loneliness, which significantly impacts physical and mental health, quality of life, and longevity, is recognized as a global concern among older adults. Although home care services are generally viewed as beneficial, they may also fall short in addressing the psychosocial needs of older adults. This study aims to assess loneliness and its associations with well-being and personal characteristics in older adults in Lithuania receiving long-term home care services. METHODS: A cross-sectional quantitative study was conducted between September and November 2024. A translated versio

Referral and utilization patterns for home-based palliative care services among older adults.

BACKGROUND: Home-based palliative care (HPC) improves quality of life for individuals with serious illness but remains underutilized in the United States, with persistent disparities in access. HPC is also frequently misconceived as appropriate only at the end of life. AIM: This study examined disparities in access to HPC by comparing patients who received services with those who were referred but not enrolled. METHODS: A secondary analysis was conducted using 567 clinical records from a healthcare facility in the eastern United States between 2022 and 2023. Descriptive statistics, chi-squar

The role of home care service in family care of developmentally disabled children: an exploratory study.

Recent concern about the needs of developmentally disabled children living at home has led to increased attention to family support programs. Home care service is one family support option which should be viewed as a basic fundamental support strategy. An exploratory study of a small number of families caring for developmentally disabled children in New York City indicates that publicly-funded home care produces a great number of benefits for this special population.

Aging in the community: Factors associated with home adaptations and receipt of informal care, home care, and community support services among older Canadians.

BACKGROUND: Most older Canadians would prefer to stay in their own homes and communities rather than move to long-term care (LTC) facilities. A growing older population, limited LTC capacity, and costs have increased demand for initiatives that enable people to age in their communities for as long as possible. METHODS: Data from the 2019/2020 Canadian Health Survey on Seniors were used to estimate the prevalence of four types of supports and services (SSs) that assist with living in the community: home adaptations, informal care, home care, and community support services. Separate multivariab

Developing services to support parents caring for a technology-dependent child at home.

BACKGROUND: A group of children with complex health care needs have emerged as a result of medical advances and government policies emphasizing the community as the arena for care. Some of these children remain dependent on the medical technology that enabled them to survive and require care of a complex and intensive nature to be carried out by their parents at home. AIMS: To explore the experiences of families caring at home for a technology-dependent child; to examine their needs for practical and other support; and to examine how far services are currently meeting these needs. Methods In-

Clinical characteristics and patterns of health deficits of centenarians receiving home care and long-term care services.

Centenarians (persons aged 100years and older) are one of the fastest growing cohorts in countries across the world. With the increasing prevalence of centenarians and growing amount of clinical information in large administrative health databases, it is now possible to more fully characterize the health of this unique and heterogeneous population. This study described patterns of health deficits in the centenarian population receiving care from community-based home care services and long-term care facilities (LTCFs) in Ontario, Canada. All centenarians who received home care and were assessed

The use of artificial nutrition among cancer patients enrolled in palliative home care services.

In this study, patients' views and experiences of using home artificial nutrition, and factors associated with use of home parenteral nutrition (HPN) were investigated. Structured telephone interviews with 620 cancer patients enrolled in 21 palliative home care services were analysed. HPN was more common (11%) than home enteral tube feeding (HETF, 3%). Home artificial nutrition (including HPN and HETF) was usually introduced more than four months before death. Three of four HPN recipients also had oral food intake. HPN use was associated with eating difficulties, nausea/vomiting, and fatigue r

Characterizing the Use of Home-Based Behavioral Health Services Among Children in Foster Care.

BACKGROUND: Children in foster care (FC) experience disproportionately high rates of mental health (MH) needs and frequently use behavioral health services. They are overrepresented in restrictive MH settings and are more likely to be prescribed psychotropic medications. Home-based behavioral health services (HBHS) offer a less restrictive alternative for addressing the complex needs of children in FC, but their use and effectiveness remain understudied. METHODS: Using Medicaid data from 28 states, we examined MH service utilization among children aged 3-18 with primary MH diagnoses across th

Effects of home-based long-term care services on caregiver health according to age.

BACKGROUND: Caregiver health is a crucial public health concern due to the increasing number of elderly people with disabilities. Elderly caregivers are more likely to have poorer health and be a care recipient than younger caregivers. The Taiwan government offers home-based long-term care (LTC) services to provide formal care and decrease the burden of caregivers. This study examined the effects of home-based LTC services on caregiver health according to caregiver age. METHODS: This cross-sectional study included a simple random sample of care recipients and their caregivers. The care recipi
