Advocacy for homebirth is based on the strong assumption that birthing is a physiological process and does not require medical interventions unless things turn "wrong." Let us assume that something might always go wrong, for instance during Clara's birth when the placenta was still retained after three hours. What needs to be done? The moment the midwife entered the house she was endowed with a responsibility for any problem caused by her failure to give proper guidance. With this weight on her shoulder, and according to her training and experience, there was no other way for her than to sugge

A comprehensive, qualitative approach to research was utilized to explore the ways in which birth territory theory can illuminate the birthing experiences of women living in Baringo and Nakuru counties, and how birth territory theory can be decentered through an African case study. Research was conducted using a combination of qualitative methods including participant observation, semi-structured interviews, and focus groups with adult women living in Baringo or Nakuru counties in Kenya who have previously given birth. Results suggest that multiple societal factors contribute to the experience

This article examines the development of a collaborative model of home-based reproductive caregiving in Ireland from 1900 to 1950, focusing on the interactions of different practitioners in childbirth cases in the domestic sphere. In Ireland the move to obstetrics and trained nursing and midwifery was gradual, complicated by the needs and wants of ordinary women, who were reluctant to give up their trusted care givers and who actively sought to maintain long-standing domestic health care traditions. The result was a hybrid and collaborative model of domestic reproductive health care, requiring

Fear of childbirth has gained importance in the context of increasing medicalization of childbirth. Belgian and Dutch societies are very similar but differ with regard to the organization of maternity care. The Dutch have a high percentage of home births and low medical intervention rates. In contrast, home births in Belgium are rarer, and the medical model is more widely used. By comparing the Belgian and Dutch maternity care models, the association between fear of childbirth and medicalization can be explored. For this study an antenatal questionnaire was completed by 833 women at 30 weeks o

The debate regarding the role of out-of-hospital childbirth (home births) remains controversial in many countries, including Austria; consequently, the Executive Board of the OEGGG, acting on behalf of our professional society, has decided to issue an updated recommendation concerning planned out-of-hospital births. This recommendation aims to articulate our position within the context of the forensic and clinical requirements of modern obstetric care and to support a revision of legislation governing out-of-hospital childbirth. It was unanimously approved by the entire OEGGG Executive Board f

BACKGROUND: The majority of women in Sokoto, Nigeria prefer homebirths, but midwives are reluctant to provide care in the home setting. As such, many women continue to give birth at home alone or assisted by untrained attendants, which is associated with an increased risk for maternal and neonatal morbidity and mortality.
METHODS: A randomized controlled trial was conducted among 226 midwives from 10 health care facilities. The intervention group received an educational program on home birth. A validated questionnaire that evaluated knowledge, attitudes, norms, perceived control, and intentio

INTRODUCTION: Over the last decade, planned home births in the United States (US) have increased, and have been associated with increased neonatal mortality and other morbidities. In a previous study we reported that neonatal mortality is increased in planned home births but we did not perform an analysis for the presence of professional certification status.
PURPOSE: The objective of this study therefore was to undertake an analysis to determine whether the professional certification status of midwives or the home birth setting are more closely associated with the increased neonatal mortalit

OBJECTIVE: To determine the prevalence of fecal incontinence after childbirth and to identify the risk factors.
METHODS: This was a prospective observational study with a consecutive inclusion of 525 women who delivered over a three months period. Women were questioned about their fecal continence four days and six weeks after delivery.
RESULTS: The incidence of fecal incontinence four days and six weeks after childbirth was respectively 8.8% and 3.3%. The risk factors for fecal incontinence at 4 days after childbirth were instrumental delivery by forceps (adjusted odds ratio 8.64, 95% confi

OBJECTIVE: Emergency ambulance services attend a wide array of medical and trauma patients. Infrequently, this includes imminent or out-of-hospital births (OOHBs). This scoping review explores emergency ambulance clinician involvement with OOHBs, and patient and clinician experiences with birthing in the out-of-hospital setting.
DESIGN: Scoping review using the Joanna Briggs Institute framework and 'participant, concept, context' criteria.
DATA SOURCES: CINAHL, Embase, Medline, Web of Science and Wiley Online were searched until 20 February 2024.

Homebirth is legal and has been regulated by law in Hungary since 2012. Despite the obvious advantages of homebirth, it has not yet been broadly accepted, due to various opinions related to safety and risks associated with giving birth outside of a hospital. Our study aimed at exploring both real maternal and feto-neonatal characteristics associated with Hungarian homebirths. A total of 2997 cases were considered in support of our retrospective cohort study. In the examined period, there was a significant, continual rise in the number of homebirths by a rate of 0.22% on average per year. Aggre

BACKGROUND: In New South Wales (NSW), Australia there are three settings available for women at low risk of complications to give birth: home, birth centre and hospital. Between 2000 and 2012, 93.6% of babies were planned to be born in hospital, 6.0% in a birth centre and 0.4% at home. Availability of alternative birth settings is limited and the cost of providing birth at home or in a birth centre from the perspective of the health system is unknown.
OBJECTIVES: The objective of this study was to model the cost of the trajectories of women who planned to give birth at home, in a birth centre

Case from Sri Lanka describes a chain of catastrophic complications in a patient whose main problem--a hydrocephalic fetus with macrocephaly in breech presentation, was not diagnosed during antenatal care. The delivery was taking place at home up to shoulders and arms, the head was not delivered. The patient with threatening uterine rupture was transferred to District Hospital and seen by doctor 10 hours later. Destructive operation of hydrocephalic head was performed, the procedure was accompanied by rupture of uterus and followed by cardiac arrest. The patient was resuscitated and hysterecto

Diaphragmatic rupture during labor is uncommon and generally occurs in patients with a history of congenital diaphragmatic hernia or traumatic abdominal or chest injury. We present a case of a 41-year-old woman who presented with abdominal pain, vomiting and hypoventilation four days after a full-term home delivery. Chest radiography suggested the presence of a ruptured diaphragm, and laparotomy revealed a congenital left Bochdalek defect with herniation of the stomach, transverse colon and spleen into the left pleural cavity. Diaphragmatic hernia rupture during labor is a serious but rare com

A retrospective analysis was made of 32 neonates with peri-umbilical cellulitis seen at Wesley Guild Hospital, Ilesa, Nigeria from 1979 through 1989. All were full term and delivered after a normal pregnancy. The male:female ratio was 1.3:1. On admission, the mean (Standard Deviation) post-natal age, weight and duration of symptoms were 7.3 (2.7) days, 3.21 (0.5) kg, and 1.9 (1.3) days respectively. Major features were abdominal distension, extensive peri-umbilical induration and inflammation, and foul-smelling discharge. Most mother had antenatal care, but 60 pc were delivered at home or in t
