IMPORTANCE: A central goal of medical malpractice law is to ensure quality of care by deterring negligent treatment. Although previous research has produced mixed results, much of it finds no clear association between liability exposure and medical treatment. During the COVID-19 pandemic, many states adopted immunity from tort liability for harms to nursing home residents, creating a natural experiment.
OBJECTIVE: To examine the association between immunity from tort liability and nursing home staffing.
DESIGN, SETTING, AND PARTICIPANTS: This cohort study using a difference-in-differences mo

OBJECTIVE: To determine the prevalence of heart failure (HF) in nursing home residents and to gain insight into the clinical characteristics of residents with heart failure.
DESIGN: Multi-centre, observational, cross-sectional study.
METHOD: 501 nursing home residents aged 65 years and over, in a department for chronic somatic diseases or a psychogeriatric department, participated in this study. The diagnosis of HF and the related characteristics were based on data collected from clinical examinations for heart failure (including history, physical examination, ECG, cardiac markers and echoca

UNLABELLED: The mean age of patients with end-stage renal disease increases steadily. The elderly on dialysis have significant comorbidity and require extra attention to meet their dialysis, dietary, and social needs, and some may need to be treated at a long-term care facility such as a nursing home (NH). Providing dialysis and caring for elderly patients in a nursing home (NH) presents a number of challenges. Few data are available in the literature about elderly patients on peritoneal dialysis (PD) in an NH. This paper describes our experience of starting and maintaining a peritoneal dialys

Contractures are a common but preventable consequence of prolonged physical immobility among nursing home residents. Significant for their associated costs in institutions rendering care to frail elder residents, contractures further reduce mobility and increase the risk of other outcomes of decreased mobility, such as pressure ulcers. This secondary analysis examines the prevalence of contractures in 273 residents who participated in an interventional study focusing on reducing restrictive siderails. Almost two thirds of the participants had at least 1 contracture, with the most common locati

INTRODUCTION: This study aims to examine the demography of falls, factors that contribute to falls and fall risk factors in residents of a nursing home for the elderly in Singapore.
MATERIALS AND METHODS: An 18-month follow-up study was carried out on 95 nursing home residents. Information on the subjects' potential risk factors for falls was obtained through a review of their casenotes and physical examination. At the end of the study period, the casenotes were reviewed for any documented fall, the circumstances surrounding each fall and whether complications resulted from the fall. The like

BACKGROUND: Health care providers often believe that individuals with cognitive disturbance are unaware of their deficits. The term unawareness was first used to describe hemiplegia following right hemisphere stroke but has since been applied to unawareness of any neurological or neuropsychological deficit. Clinicians usually rely on their subjective observations to evaluate the patient's awareness of deficits, and few investigators have systematically evaluated this important clinical phenomenon.
OBJECTIVE: The aim of this study was to compare cognition, depression, health, and metamemory (c

Some of the most intensive pharmacotherapy today occurs in nursing homes in very complex and vulnerable patients. The nursing home provides an opportunity for highly effective drug use, but it also presents risks for polypharmacy and adverse events. Nursing homes are complex social institutions, in which physicians, nurses, consultant pharmacists, other health care professionals, aides, and administrators must interact to make decisions about drug use for patients who generally are frail and have numerous comorbid conditions. Federal regulations have recently been implemented to direct the way

Persistent scabies outbreaks in extended care facilities result from multiple factors: atypical presentations in the elderly, leading to delayed or inaccurate diagnosis; incomplete or ineffective treatment of the patient and exposed population; and failure to coordinate notification, education, treatment, and disinfection. A case report of scabies in an elderly resident of an extended care facility illustrates various types of scabies manifestations, the advisability of obtaining diagnostic scrape preparations, and the efficacy and safety of permethrin. The institutional atmosphere and programming of a nursing home can often engender a downward spiral of increasing dependence and learned helplessness in its elderly residents. Elderly patients with chronic medical complaints combined with psychiatric and management problems are especially susceptible. An intermediate-and skilled-care nursing home began a special program in 1974 to deal with these patients. The program features problem-oriented rather than diagnosis-oriented recordkeeping and the use of a multidisciplinary therapeutic team to treat problem residents. Over the first four years o

Nutritional surveys were conducted on all 115 patients of a Medicare-approved nursing home. The techniques of nutritional assessment used included anthropometric measurements (weight/height, triceps skinfold, arm-muscle and arm circumference), biochemical and hematologic tests (serum albumin, serum transferrin, total lymphocyte count, hemoglobin, and hematocrit), and an intradermal skin test for cell mediated immunity. The prevalence of protein-calorie malnutrition (PCM) of moderate to severe degree was very high by currently used standards (weight/height, 43%; triceps skinfold, 37%; arm-muscl

Working in a nursing home leads to specific problems for caregivers. For example, being confronted with an ageing body and a declining mind or enduring inadequate work organisations can cause occupational exhaustion and burnout. The quest for job satisfaction at work then becomes a challenge.

OBJECTIVE: To examine the prevalence of undernutrition, received dietetic treatment and self-perception of nutritional status in older patients admitted to Dutch nursing home rehabilitation wards.
METHODS: Between December 2012-February 2014, we included 190 patients (≥65 y) admitted to seven nursing home rehabilitation wards. Nutritional status in the first week of admission was characterized as: severely undernourished (>10% unintentional weight loss in the past six months and/or >5% unintentional weight loss in the past month and/or BMI < 20 kg/m2), moderately undernourished (5-10% uninten

BACKGROUND: As long-term nursing home care is likely to increase with the aging of the population, identifying chronic medical conditions is of particular interest. Although need factors have a strong impact on nursing home (NH) admission, the diseases causing these functional disabilities are lacking or unclear in the residents' file. We investigated the medical reason (primary diagnosis) of a nursing home admission with respect to the underlying disease.
METHODS: This study is based on two independent, descriptive and comparative studies in Belgium and was conducted at two time points (1993

Sexual behavior is an integral part of human existence. The changes of aging affect all stages of sexual response in both men and women, but the capacity to enjoy sex is not altered with age. Surveys demonstrate that while sexual activity declines with age, a significant proportion of elderly persons, including those in nursing homes, remain sexually active. Barriers to sexual expression for residents of long-term care facilities include lack of privacy, chronic illness, lack of a willing partner, loss of interest, attitudes of physicians and staff, feelings of unattractiveness and an insuffic

Pressure, moisture, shear forces and friction lead to skin ulcer formation. Nursing home and home-bound patients with restricted mobility, poor nutrition, incontinence and chronic conditions such as anemia, diabetes and dementia are at risk for ulcer formation. Bedridden patients should be turned from side to side at 30-degree angles at least every two hours. Mattress and chair cushions, splints and cradle boots may reduce pressure. Good hygiene and barrier ointments, condom catheters, absorptive products and scheduled toileting for incontinence may control moisture. Calorie and protein supple

We believe that a new agenda for the ethics of long-term nursing home care could be set by seeing nursing homes as communities of caring and interdependency. The goal should be not simply to eliminate or minimize dependency whenever possible, but to make a genuinely creative and nurturing use of the dependency that is an inevitable reality for most nursing home residents. Nursing homes are rarely places of curing, but they can and should be places of healing -- of making whole -- of enabling frail or chronically ill persons to use their dependency to grow as human beings... In general, nursing
