Does poor dental health predict becoming homebound among older Japanese?

BACKGROUND: Being homebound is an important risk factor of functional disability in older people. There is a possibility of bidirectional relationship between homeboundness and dental health. This prospective cohort study examined the association of dental health, which includes social function, on homeboundness in the future. METHODS: The participants were ≥ 65 years, responded to two postal surveys conducted in 2006 and 2010, and were not homebound at baseline. Logistic regression analysis was used to estimate the odds ratios for homeboundness, defined as going out of one's home less than o

Dysphagia in Older Adults is Associated With Food Insecurity and Being Homebound.

Objective: Our aim was to evaluate relationships between swallowing difficulty (dysphagia) and social determinants of health (SDOH) in older adults ≥65 years. Method: Cross-sectional analyses were performed in community-dwelling Medicare beneficiaries from the National Health & Aging Trends Study (NHATS). The primary exposure was self-reported difficulty chewing/swallowing in the prior month. Dependent measures included a variety of SDOH outcomes (e.g., food insecurity [FI]). Weighted logistic regression models were estimated to determine associations between dysphagia and SDOH outcomes. Resul

Reducing my risk of intrusion: an intention of old homebound women who live alone.

BACKGROUND: In assessing home safety for older persons, nurses have checked home security or assessed fear of crime but neglected to explore intentions to reduce the risk of intrusion. OBJECTIVES: To describe the intentions of old homebound women relative to reducing the risk of intrusion. METHODS: A descriptive phenomenological method was used. The 40 participants (aged 85-98 years, M = 89.6) met criteria for age, living alone, and homebound status. Over 18 months, a series of tape-recorded interviews was completed, focusing on perceptions about changes in feeling safe at home and precautio

Using Telemedicine to Assess and Manage Psychosis Among Outpatients with Neurodegenerative Disease.

The presence of hallucinations and delusions in patients with neurodegenerative disease correlates negatively with function, cognition, quality of life, and survival. When these patients still have insight, the treatment of mild hallucinations may reduce the risk of progression to more severe symptoms, specifically hallucinations without insight or delusions. On October 22, 2020, a multidisciplinary consensus panel comprising United States-based experts in geriatric psychiatry, geriatric medicine, family medicine, movement disorders, and neuropsychology was convened remotely to discuss best pr

Digital Divide Among Homebound and Semi-Homebound Older Adults.

Using the 2022 National Health and Aging Trend Study data, we examined the digital divide among homebound and semi-homebound older adults. About 11% of older Medicare beneficiaries in 2022 were homebound or semi-homebound. Compared to non-homebound older adults, homebound older adults were significantly less likely to own a cellphone. Among those with any information and communication technology (ICT) device, homebound older adults were also less likely to have used email/texting and gone online. Older age, low income, dementia diagnosis, and moderate/severe depressive/anxiety symptoms contrib

Effect of Telehealth Treatment by Lay Counselors vs by Clinicians on Depressive Symptoms Among Older Adults Who Are Homebound: A Randomized Clinical Trial.

IMPORTANCE: Older adults who are homebound and have low income have limited access to psychosocial treatments because of their homebound state and geriatric mental health workforce shortages. OBJECTIVE: To evaluate clinical effectiveness of a brief, aging service-integrated, videoconferenced behavioral activation (tele-BA) treatment delivered by lay counselors compared with videoconferenced problem-solving therapy (tele-PST) delivered by licensed clinicians and attention control (AC; telephone support calls). DESIGN, SETTING, AND PARTICIPANTS: This 3-group randomized clinical trial using a r

Dysphagia is associated with oral, physical, cognitive and psychological frailty in Japanese community-dwelling elderly persons.

OBJECTIVE: To investigate which domains of frailty are associated with dysphagia in community-dwelling elderly persons. BACKGROUND: Dysphagia is a common cause of aspiration pneumonia in frail elderly persons in Japan. Although frailty is considered to be multidimensional, it is unclear which domains of frailty affect dysphagia. METHODS: The participants were 3475 independent Japanese elderly persons (≥65 years; 1555 men and 1920 women). A self-report questionnaire with a frailty checklist consisting of several domains (lifestyle, physical function, nutrition, oral function, homebound status

Diet and health outcomes in vulnerable populations.

Good nutritional health is essential to physical and cognitive function, prevention or management of chronic health conditions, and prevention of disability. Poor nutritional health describes dietary intakes of inadequate amounts of energy, protein, and individual or multiple micronutrients. Homebound older adults are considered the most vulnerable for poor nutritional health; many are poor and experience persistent food insufficiency. This chapter discusses the importance of dietary intakes, especially for homebound older adults, and presents resource-related factors that influence food choic

A Three-Year Evaluation of COVID-19 Vaccination and Testing Patterns Among Homebound Older Adults: Implications for Future Preparedness.

COVID-19 self-test kits were distributed to low-income, homebound older adults with their home-delivered meals annually (2022-2024). A convenience sample of recipients completed follow-up surveys in 2022 (n = 271), 2023 (n = 295), and 2024 (n = 285). In 2023, 28% had ever used a COVID-19 self-test, which increased to 39% in 2024 and is comparable to a national study (27%). Through partnerships with trusted meal-delivery providers, homebound older adults can achieve rates of COVID-19 self-testing comparable to those of the broader population. Shedding Light on an Invisible Population in Geriatric Medicine: A Scoping Review of Older Adults Who Are Homebound.

BACKGROUND: Older adults who are homebound, defined as individuals who rarely or never leave their homes without assistance, represent a vulnerable and growing population with complex social and medical needs. Despite their significant healthcare utilization and unmet care needs, research on this group is fragmented and has not been comprehensively synthesized. This scoping review aimed to summarize the determinants, characteristics, and outcomes of being homebound to identify care gaps and propose directions for future research. METHODS: This review followed the PRISMA-ScR framework, conduct

[Incidence of disability in housebound elderly people living in a rural community].

This study aimed to explore whether being housebound is a risk factor for disabilities and whether low social communication increases incidence of disability in elderly people. A self-reported questionnaire regarding demographic characteristics was administered to 2,046 community-dwelling elderly people (aged 65 and older) in October 2000, and subjects were followed up until March 2003. All subjects were independent in activities of daily living. In this study, being housebound was defined on frequency of going out, with those who left the house once or less per week being classified as houseb

Factors associated with having home care physicians as an integrated source of medications for chronic conditions among homebound patients.

BACKGROUND: Inappropriate management of medications is a major threat to homebound patients with chronic conditions. Despite many efforts in improving medication reconciliation in ambulatory and inpatient settings, little research has focused on home care settings. In 2016, Taiwan initiated the Integrated Home Health Care programme, which was intended to reduce potentially inappropriate medication management and risks of uncontrolled polypharmacy through the integration of different medication sources for chronic conditions among homebound patients. This study investigated factors associated w

Prevalence and conditions of urinary incontinence among the elderly.

In Japan, elderly disorders and diseases have markedly increased in recent years, because of rapid aging and an increasing number of older persons. The situation is creating serious social and community problems. These disorders, particularly dysuria and urinary incontinence (UI), disturb the quality of life (QOL) in latelife. Few reports on UI have been published, but precise investigation into the community level remains to be made. Our presentation is the development, implementation and evaluation of elderly UI in Kumamoto Prefecture. This study includes 2,304 people (male: 856, female: 1,4

Examining the Longitudinal Effects of Therapeutic Alliance Based on Session Rating Scale Scores on Suicidal Desire in Homebound Older Adults.

INTRODUCTION: The Session Rating Scale (SRS) is widely used to assess therapeutic alliance in clinical settings, yet its application in paraprofessional-delivered interventions for older adults remains underexplored. This study examined whether therapeutic alliance predicted improvements in suicide desire among community-dwelling older adults participating in a warm-calling intervention. METHODS: Binomial generalized linear mixed-effects models were used with dichotomized high-score responses for thwarted belongingness (TB) and perceived burdensomeness (PB) to evaluate the role of therapeutic

The Effectiveness of Remote Exercise Rehabilitation Based on the "SCeiP" Model in Homebound Patients With Coronary Heart Disease: Randomized Controlled Trial.

BACKGROUND: While exercise rehabilitation is recognized as safe and effective, medium- to long-term compliance among patients with coronary heart disease (CHD) remains low. Therefore, promoting long-term adherence to exercise rehabilitation for these patients warrants significant attention. OBJECTIVE: This study aims to investigate the impact of remote exercise rehabilitation on time investment and related cognitive levels in homebound patients with CHD. This study utilizes the SCeiP (Self-Evaluation/Condition of Exercise-Effect Perception-Internal Drive-Persistence Behavior) model, alongside
