Hospice health care: management of a case of bullous pemphigoid utilizing the hospice interdisciplinary team.

An association between bullous pemphigoid and internal malignant neoplasms has been reported by many authors, primarily in case report formats. Stone and Schroeter, however, compared cases of bullous pemphigoid with age and sex-matched control patients with contact dermatitis and psoriasis and found no evidence of an increased association between malignant neoplasms and bullous pemphigoid. A few cases of bullous pemphigoid occurring after radiation therapy for malignant neoplasms have been reported. A case of bullous pemphigoid which occurred after radiation therapy for squamous carcinoma of t

Implementing Education for Community Adult Hospice Nurses to Expand Pediatric Hospice and Palliative Care.

Children with life-limiting illnesses living in nonurban areas have minimal access to community-based pediatric hospice and palliative care. Barriers such as geography, limited continuing education opportunities, and clinician discomfort compound this issue. Pediatric hospice and palliative patients require specially-trained clinicians to provide holistic support in areas such as disease progression, illness trajectory, and goals of care. An asynchronous online educational module, including a pre- and postmodule survey, was developed to provide education on timely pediatric quality-of-life con

Transfusion in Hospice-Palliative Care.

Transfusion in hospice and palliative care remains a complex issue as it is viewed as either a futile life-prolonging treatment or a legitimate intervention for symptom relief. These differing views result in inconsistent clinical practices. Anemia is common in terminally ill patients and is associated with fatigue, dyspnea, dizziness, palpitations, cognitive decline, and an impaired quality of life. Although red blood cell transfusion may provide short-term symptomatic improvement in selected patients, its benefits are often transient; repeated transfusions may be necessary, and its effect on

Thromboprophylaxis in cancer patients in hospice.

Advanced cancer patients in hospice are at notably increased risk of venous thromboembolism (VTE) due to age, local and distal advancement of the malignancy and bed confinement, among other factors. Asymptomatic VTE prevalence among palliative care patients has been found to reach 50%, whereas the clinically overt form occurs in 10%. Hospice patients are frequently given medications increasing VTE risk, for instance megestrol which is a drug commonly used in cancer cachexia. Many of the available guidelines encourage the implementation of thromboprophylaxis (TPX) in cancer patients, e.g., in t

Scabies in an AIDS hospice unit.

Scabies may cause severe problems in immunocompromised persons. Three case histories are presented which illustrate these problems in people with advanced AIDS. Recommendations for treatment and management are made, with a comment about the need for a high level of suspicion about any atypical rash in an immunocompromised patient.

End-of-Life Care and Hospice.

OBJECTIVE: This article provides an overview of what neurologists need to know about hospice and basic symptom management at the end of life, addressing issues relevant to both outpatient and inpatient settings. LATEST DEVELOPMENTS: Randomized, placebo-controlled trials are challenging in this patient population, and many of the recommendations for symptom management represent expert opinion and common practices in hospice and palliative care. More high-quality evidence is needed to guide best practices. ESSENTIAL POINTS: Core competencies in end-of-life care are necessary for any clinician

Hospice Approach to Palliative Care

For the provision of palliative care in the hospice setting in the United States, Medicare covers any care that is reasonable and necessary to manage palliative and hospice care at end of life. To be eligible for this Medicare hospice benefit, a beneficiary must be entitled to Medicare Part A and be certified by a physician to have a life expectancy of 6 months or less if the illness runs its expected course. Medicare covers medications, nursing care, and medical services including care by a physician, physical therapy, social work. This chapter briefly reviews the various aspects of the Medic

An audit of morphine prescribing in a hospice.

AIM OF STUDY: This audit was designed to investigate the morphine prescribing pattern in a hospice. METHOD: A review of 358 medical charts of all existing patients was conducted with a set of questionnaire. The prevailing practice was compared with an established standard guideline. RESULT: One-third (35%) of patients were receiving morphine. Several deficiencies in morphine prescribing were identified. These include omission of breakthrough morphine dosing, use of morphine as p.r.n. (when necessary) alone for chronic pain, absence of review after prescribing treatment, and lack of double do

The psychosocial evaluation of the hospice patient.

A psychosocial evaluation of a patient is an important first step in the provision of care to hospice patients and their families. It serves as a guide for intervention, communicates social work concerns to other caregivers, and articulates important features of the patient/family system from an interdisciplinary perspective. The psychosocial evaluation addresses the patient's problems in a historical framework by identifying important developmental features as they pertain to the tasks of dying. It identifies physical resources and needs and evaluates the patient's psychosocial functioning, w

Mental Health Service Integration in Hospice Organizations: A National Survey of Hospice Clinicians and Medical Leadership.

Background: Unmet mental health needs are associated with a range of negative consequences for individuals at the end of life. Despite the high prevalence of mental health needs among individuals enrolled in hospice, there is a paucity of data describing mental health service integration in hospices in the United States. Objectives: 1. To identify patterns of mental health service integration in hospice organizations nationally; 2. To characterize gaps in mental health service delivery in hospice settings as perceived by hospice clinicians and medical leadership. Methods: A cross-sectional sur

Characteristics of Prison Hospice Patients: Medical History, Hospice Care, and End-of-Life Symptom Prevalence.

Increasing numbers of prisoners in the United States are dying from age-related and chronic illnesses while incarcerated. This study is among the first to document characteristics of a population of prison hospice patients. Retrospective review of medical records for all patients admitted to the Louisiana State Penitentiary prison hospice program between January 1, 2004, and May 31, 2012 (N = 79) examined demographics, medical history, hospice diagnosis, length of stay, and end-of-life symptom prevalence on admission and during final 72 hours before death. Resulting data were contrasted with c

Hospice Care in the Era of AI: Hospices' Views on Data-Driven Tools to Support Live Discharge Decisions.

BACKGROUND: Live discharge occurs for 20% of hospice enrollees, resulting in loss of support and disruptive care transitions, with higher risk for patients with Alzheimer's disease and related dementias (ADRD). Little is understood about how data-driven clinical decision support tools (e.g., predictive algorithms) might support decision making regarding live discharge. As hospices adopt value-based care, identifying opportunities and challenges for data-driven tools to predict and support live discharge holds great potential to support hospice patients and their caregivers. METHODS: Semi-stru

CMS Hospice Care Index Live Discharge Quality Measurement: Insights From Nonprofit Hospice Leaders.

BACKGROUND: Hospice live discharge affects approximately 20% of enrollees and often disrupts supportive care, increasing patient and family burden. The CMS Hospice Care Index (HCI) assesses hospice quality, including four live discharge-focused measures: early (<7 days), late (>180 days), Type 1 burdensome transition (discharge-hospital-readmission) and Type 2 (discharge-hospital-death). Little is known about the utility and implications of the HCI live discharge measures on clinical care and policy since their implementation. METHODS: This study analyzed responses from semi-structured interv

Relevance of clinical ethics support services in specialized outpatient palliative care teams and inpatient hospices.

BACKGROUND AND OBJECTIVES: Clinical ethics support services (CES) may support palliative care teams in ethical conflicts. However, it is not known to what extent these services are available, used and how their relevance is perceived by the teams. The study focuses on the accessibility and evaluation of ethics consultations in specialized outpatient palliative care teams and inpatient hospices. METHODS: This study conducted a cross-sectional 21-item anonymous survey among teams of registered specialized outpatient palliative care services (n = 304) and inpatient hospices (n = 260) in Germany

Social Model Hospice: Providing Hospice and Palliative Care for a Homeless Population in Salt Lake City, Utah.

Health care for the homeless population is a complex challenge and represents a significant gap in care, particularly for those at the end of life. Hospice care may be desired but is rarely an option for people without residences, social support, and payment sources. Social model hospice is a unique paradigm of care delivery that offers a viable solution to make hospice and palliative care possible for this population. In this historical report from interviews with early organizers, prior and current leadership (n = 6), the evolution of The INN Between in Salt Lake City, Utah, is described. In
