[Extent and use of administrative hospital data in the German DRG system].

In 2003 the mandatory introduction of diagnosis related groups as an overall payment system started for all hospitals in Germany, except for departments of psychiatry and psychotherapy as well as psychosomatics and psychotherapy. All hospitals are legally obliged to deliver extensive data on hospital treatment, including demographic data, diagnoses, comorbidities, complications, and procedures, to the "Institute for the Hospital Remuneration System" which uses the data for yearly adaptation of the German DRG system (G-DRG). This article describes legal issues of data collection, parameters and

The effect of pre-hospital administration of intravenous nalbuphine on on-scene times.

The use of intravenous nalbuphine in pre-hospital settings by paramedics has been demonstrated to be safe and effective. We assessed the effect of this additional intervention by comparing the time spent on-scene by ambulance crews treating patients with fractures of the tibia and fibula who received intravenous nalbuphine with those who had placement of an intravenous cannula alone and those who had neither cannula nor nalbuphine. The mean on-scene times were 17.1 min without cannulation, 29.9 min for cannulation without nalbuphine and 37.5 min for cannulation and administration of nalbuphine

Frequency and pattern of radionuclide administration to medical patients in a general hospital.

As part of a drug surveillance programme details of nuclide administration were recorded for 5288 consecutive patients admitted to the medical wards of a large general hospital. Overall use of nuclides amongst this group was found to be low, and multiple exposure to radiation as a result of nuclide administration is not a major hazard when compared to the diagnostic information provided.

Granular Cell Tumor: Report of 13 Cases in a Veterans Administration Hospital.

INTRODUCTION: Granular cell tumors (GCTs) are of neuroectodermal origin and account for 0.5% of soft tissue tumors. They are most prevalent in African-Americans in the fourth to sixth decades, with a predilection for the head and neck regions. GCTs usually resemble more common lesions and hence are difficult to diagnose preoperatively. The tumor is readily identified on needle biopsy. Although usually benign with a malignancy rate of only 2-3%, the malignant form has a reported 3-yr mortality of 60%. MATERIALS AND METHODS: Clinical records of the Gulf Coast Veterans Healthcare System were sur

How albumin administration for cirrhosis impacts on hospital albumin consumption and expenditure.

AIM: To assess the impact of guidelines for albumin prescription in an academic hospital, which is a referral center for liver diseases. METHODS: Although randomized trials and guidelines support albumin administration for some complications of cirrhosis, the high cost of albumin greatly limits its use in clinical practice. In 2003, a multidisciplinary panel at Sant'Orsola-Malpighi University Hospital (Bologna, Italy) used a literature-based consensus method to list all the acute and chronic conditions for which albumin is indicated as first- or second-line treatment. Indications in hepatolog

Naloxone administration associated with improved survival in PEA out-of-hospital cardiac arrests.

INTRODUCTION: The prevalence of opioid-associated out-of-hospital cardiac arrest (OA-OHCA) is increasing in the United States. Naloxone administration during OA-OHCA has been associated with improved clinical outcomes, but identifying OA-OHCA in the field remains challenging. Previous work demonstrated association between non-shockable cardiac rhythms and OA-OHCA, suggesting that cardiac rhythm may help identify patients likely to benefit from naloxone. In this work, we assessed the association of naloxone with clinical outcomes for patients with OHCA, stratifying by presenting cardiac rhythm.
