Thermal disinfection of hotels, hospitals, and athletic venues hot water distribution systems contaminated by Legionella species.

Legionella spp. (> or = 500 cfu liter(-1)) were detected in 92 of 497 water distribution systems (WDS) examined. Thermal disinfection was applied at 33 WDS. After the first and second application of the disinfection procedure, 15 (45.4%) and 3 (9%) positive for remedial actions WDS were found, respectively. Legionella pneumophila was more resistant to thermal disinfection than Legionella non-pneumophila spp. (relative risk [RR]=5.4, 95% confidence intervals [CI]=1-35). WDS of hotels with oil heater were more easily disinfected than those with electrical or solar heater (RR=0.4 95% CI=0.2-0.8).

Red blood cell distribution width and systemic immune-inflammation index in older adults with sepsis: Associations with in-hospital mortality.

The prognostic value of red blood cell distribution width (RDW) and the systemic immune-inflammation index (SII) in older adults with sepsis remains uncertain. We evaluated their associations with mortality and incremental value beyond clinical predictors. This single-center retrospective cohort study included patients aged ≥65 years admitted to the intensive care unit (ICU) with sepsis between January 2020 and December 2025. RDW and SII were assessed within 24 hours of admission. In-hospital mortality was the primary outcome, and ICU mortality was the secondary outcome. Receiver operating cha

The presence of a specific genotype of Legionella pneumophila serogroup 1 in a hospital and municipal water distribution system over a 12-year period.

The genotypic distribution of Legionella pneumophila serogroup 1 was investigated in the water distribution system of a 450-bed Swedish hospital and the surrounding community. A single genotype identified by amplified fragment length polymorphism (AFLP) analysis, was found in all 34 hospital isolates and in 18 out of 20 community isolates over a 12-y surveillance period. All isolates were either monoclonal antibody subtypes Benidorm or Bellingham. In a geographical comparison, the hospital genotype was also identified in 2 out of 6 Swedish hospitals, both located within 100 km of the studied c

Long-Term Environmental Surveillance of Pseudomonas aeruginosa in a Hospital Water Distribution System: Trends in Prevalence, Biofilm Formation, and Antimicrobial Susceptibility.

BACKGROUND: Hospital water systems are major environmental reservoirs of Pseudomonas aeruginosa, but evidence on the long-term impact of Water Safety Plans (WSPs) on environmental prevalence, antimicrobial resistance, and biofilm-forming ability remains limited. This study evaluated the long-term occurrence of P. aeruginosa in a hospital water distribution system over a 15-year surveillance period. METHODS: A 15-year environmental surveillance study was conducted in a tertiary-care hospital in Northern Italy. Water samples were cultured for P. aeruginosa, and recovered isolates were assessed

Monochloramine use for prevention of Legionella in hospital water systems.

Eradication of Legionella species from water distribution systems especially in hospital settings has proven to be challenging. Legionella species causes Legionnaire's disease that is a potentially fatal respiratory disease often acquired through the aerosolization of contaminated water. Monochloramine has been used successfully in the municipal water systems to eradicate Legionella and there is currently limited data to support its use in the hospital setting. This technology appears to be affordable, safe and effective at penetrating biofilm in water distribution systems.

Disinfection of water distribution systems for Legionella.

Hospital-acquired legionnaires' disease arises from the presence of Legionella in hospital water systems. Legionella not only persists in hot water tanks but is also found in the biofilm throughout the entire water distribution system. Conditions within water systems that promote Legionella colonization include water temperature, configuration and age of the hot water tank, physicochemical constituents of the water, plumbing materials, and commensal microflora. Hospital-acquired legionnaires' disease has been prevented by instituting control measures directed at the water distribution system.

The high prevalence of Legionella pneumophila contamination in hospital potable water systems in Taiwan: implications for hospital infection control in Asia.

BACKGROUND: The major sources of Legionnaires' disease (LD) are the potable water systems of large buildings including hospitals, nursing homes, and hotels. Culturing the hospital water system for Legionella allows a preventive approach for hospital-acquired LD. However, hospital-acquired LD is rarely reported in Taiwan, and environmental cultures of Legionella in hospital water systems in Taiwan have never been systematically performed. OBJECTIVE: The objective of this study was to determine if Legionella is present in hospital water systems in Taiwan. Water quality analysis was also perform

Legionnaires' disease: seek and ye shall find.

Legionella pneumophila is among the top three or four microbial causes of community-acquired pneumonia, yet is often misdiagnosed and inadequately treated. New laboratory tests should simplify the diagnosis. Also, contrary to common perception, the disease is usually spread via aspiration of water from contaminated hot water distribution systems, not from air conditioning systems. The treatment of choice has shifted from erythromycin to the newer macrolides and quinolones. Routine culturing of the hospital water supply is a requisite first step in preventing hospital-acquired Legionnaires' dis

The provision of urethral catheters: an equipment audit.

Although medical and nursing audit is attaining a high profile in UK hospitals, the development of objective measures remains a challenge. The availability and storage or urinary catheters was recorded by a point prevalence survey in a District General Hospital. A "long-term" and "short-term" catheter were provided in all areas, but seven further brands of catheter were found. Female length catheters were rarely available. With some limited exceptions there were no written or verbal policies concerning selection of catheters in the wards. Excess numbers of catheters were stored by wards respon

Quantile regression: a statistical tool for out-of-hospital research.

The performance of out-of-hospital systems is frequently evaluated based on the times taken to respond to emergency requests and to transport patients to hospital. The 90th percentile is a common statistic used to measure these indicators, since they reflect performance for most patients. Traditional regression models, which assess how the mean of a distribution varies with changes in patient or system characteristics, are thus of limited use to researchers in out-of-hospital care. In contrast, quantile regression models estimate how specified quantiles (or percentiles) of the distribution of
