Predictors and contributors of burnout among hospitalists in central Michigan.

Burnout among hospitalists is a growing focus of quality improvement for hospitals around the United States. Burnout has been associated with decreased patient care quality, which may lead to decreased system sustainability. A gap remains however regarding burnout factors in certain region-specific populations such as central Michigan. The objective of this study is to examine the associations between burnout and a collection of potentially impactful variables including gender, age, marital status, years at work, and dependents. Four domains of burnout were investigated among hospitalists at M

Inferior Vena Cava Filters: Relevant Issues for Hospitalists and Intensivists.

Inferior Vena Cava (IVC) filters are medical devices that are inserted into the IVC as a form of mechanical prophylaxis against pulmonary emboli (PE). Placement of IVC filters is controversial, as IVC filters can lead to complications, especially when left implanted for longer than the recommended time. In this review, IVC filter complications are divided into several categories: acute, long-term, and those associated with filter removal. Life-threatening complications of IVC filter use also are examined. Key factors for IVC filter selection are discussed, with a focus on the patients who pres

The Influence of Academic Hospitalists on Labor and Delivery Outcomes.

BACKGROUND: Our goal was to examine differences in maternal and neonatal outcomes following the transition from a private practice to an academic model at a community hospital. METHODS: This is a retrospective cohort study of a high-volume community hospital labor and delivery unit. A private practice hospitalist group was replaced with academic hospitalists. Maternal and neonatal outcomes for patients cared for by these groups were compared. The primary outcome was a composite of maternal morbidity that included blood transfusion, anal sphincter injuries, dilation and curettage, hysterectomy

Adolescent Reproductive Health Care: Views and Practices of Pediatric Hospitalists.

BACKGROUND AND OBJECTIVES: Many hospitalized adolescents are at increased risk for pregnancy complications due to an underlying medical condition, however sexual risk assessment is not consistently performed in this setting. While adolescents and their parents are supportive of sexual health discussion in the inpatient setting, a thorough understanding of factors that influence provision of this care among pediatric hospital physicians is lacking. This formative information is needed to facilitate efforts to improve and standardize clinical care provision. Our objective is to assess the freque

Forgotten but Not Gone: Update on Measles Infection for Hospitalists.

Measles (rubeola) continues to be endemic and epidemic in many regions of the world. Measles is primarily a disease of childhood, but it can also affect adult populations, and therefore it is important that both adult and pediatric hospitalist physicians be able to recognize it. Although the disease is rarely encountered in the United States, measles infection can spread rapidly across vulnerable populations. In addition, infected adults can develop complications that may require hospitalization for treatment. This review summarizes the typical clinical course and complications of measles infe

The Role of Obstetrics/Gynecology Hospitalists in Reducing Maternal Mortality.

The United States experienced a 6.1% annual increase in the maternal death rate from 2000 to 2013. Maternal deaths from hemorrhage and complications of preeclampsia are significant contributors to the maternal death rate. Many of these deaths are preventable. By virtue of their continuous care of laboring patients, active involvement in hospital safety initiatives, and immediate availability, obstetric hospitalists are uniquely positioned to evaluate patients, initiate care, and coordinate a multidisciplinary effort. In cases of significant maternal hemorrhage, hypertensive crisis, and acute p

Nuclear medicine imaging in dementia: a practical overview for hospitalists.

Dementia is a clinical syndrome with diverse presentation, a challenging differential diagnosis, and time-sensitive therapy. The most common cause of dementia in patients aged > 65 years is Alzheimer's disease, which now affects 4 million people in the United States, but is often underrecognized, especially in the inpatient population. The hospitalist may have the opportunity to evaluate a patient's initial presentation of dementia. Addressing the inpatient's dementia symptoms can improve overall care and outcomes, so it is imperative that the hospitalist is abreast of recent developments in t

Community-Acquired Pneumonia for the Hospitalist: Updates and Controversies.

The American Thoracic Society (ATS)/Infectious Disease Society of American (IDSA) guidelines for the diagnosis and treatment of community-acquired pneumonia (CAP) were released in 2019. While most categories had minor updates, one major change was abandonment of the term "healthcare-associated pneumonia" (HCAP). The guidelines also recommended against use of procalcitonin for the decision to initiate antibiotics; recommended against use of corticosteroids in all cases but pneumonia with septic shock; and recommended obtaining blood and sputum cultures in patients with pneumonia at risk for met

Role of albumin in cirrhosis: from a hospitalist's perspective.

Albumin, a negatively charged globular protein encoded on chromosome 4, is one of the most abundant proteins in the plasma and accounts for approximately 75% of plasma oncotic pressure. The role of albumin in the management of various disease states has shown to be beneficial historically. Low serum albumin is a predictor of mortality and poor outcomes. In cirrhotics undergoing paracentesis, albumin infusion prevents rapid re-accumulation of ascitic fluid while simultaneously decreasing the risk of post-paracentesis related circulatory dysfunction. Additionally, albumin is utilized in patients

Shining light on the hospitalist work after dark.

Hospitalists work day and night, but the workloads of each shift are anything but uniform. Evening and overnight ("after-hours") work is not as visible to the health system. We aimed to increase the visibility of the after-hours cross-cover work by implementing standardized documentation practices that also facilitated appropriate billing for hospitalist-provided patient care. Our education session and simple electronic medical record (EMR) note template successfully increased visibility as measured by work relative value units (wRVUs) and trackable after-hours clinical documentation. This vis

Trigger-induced atrial fibrillation for the hospitalist: a review.

Atrial fibrillation (AF) is a common arrhythmia that a hospitalist may encounter. It is known to occur transiently or in response to specific triggers (trigger-induced AF), which presents a challenge to clinicians, especially when attempting to ascertain if it warrants assessment of recurrence risk and long-term management. The management of trigger-induced AF is an emerging area of study. In this review, we discuss questions that a hospitalist should consider and address in the management of trigger-induced AF. We discuss recommendations regarding anticoagulation along with rate and rhythm co

An Academic Hospitalist-Run Outpatient Paracentesis Clinic.

BACKGROUND: Patients needing large-volume paracenteses (LVPs) can occupy inpatient hospital beds and unnecessarily use inpatient resources. METHODS: We describe an outpatient paracentesis clinic that was part of a quality assurance initiative at the Veterans Affairs Pittsburgh Healthcare System in Pennsylvania. A retrospective review was conducted that included patient age, sex, etiology of ascites, amount of ascites removed, time of the procedure, complications, and results of ascites cell count and cultures abstracted from the electronic health record. RESULTS: Over 74 months, 506 paracent

Effects of hospitalist co-management for hip fractures.

BACKGROUND: Early surgery is recommended for patients with hip fractures. Average time to surgery in Japan is 3.8 days. Such delay could be caused by the burden of preoperative assessment and management of geriatric comorbidities upon orthopedic surgeons. Hospitalist co-management has potential benefits in comprehensive perioperative care that could positively affect time to surgery in patients with hip fractures. We compare clinical outcomes of patients with hip fractures managed by orthopedic surgeon-led care (conventional group) vs. those of patients with hip fractures managed by hospitalis

Point-of-Care Ultrasound for the Pediatric Hospitalist's Practice.

Point-of-care ultrasound (POCUS) has the potential to provide real-time valuable information that could alter diagnosis, treatment, and management practices in pediatric hospital medicine. We review the existing pediatric POCUS literature to identify potential clinical applications within the scope of pediatric hospital medicine. Diagnostic point-of-care applications most relevant to the pediatric hospitalist include lung ultrasound for pneumothorax, pleural effusion, pneumonia, and bronchiolitis; cardiac ultrasound for global cardiac function and hydration status; renal or bladder ultrasound

The hospitalist perspective on opioid prescribing: A qualitative analysis.

BACKGROUND: Pain is a frequent symptom among patients in the hospital. Pain management is a key quality indicator for hospitals, and hospitalists are encouraged to frequently assess and treat pain. Optimal opioid prescribing, described as safe, patient-centered, and informed opioid prescribing, may be at odds with the priorities of current hospital care, which focuses on patient-reported pain control rather than the potential long-term consequences of opioid use. OBJECTIVE: We aimed to understand physicians' attitudes, beliefs, and practices toward opioid prescribing during hospitalization an

Ketosis-prone diabetes mellitus: A phenotype that hospitalists need to understand.

Diabetes has been classified mainly into types 1 and 2. Some type 2 diabetes patients, when developing ketosis, have been labeled as having atypical diabetes. Lately, syndromes of ketosis-prone diabetes, primarily in patients who we previously classified as type 2 diabetics, have emerged, and calls are being made to even reclassify diabetes. This mini-review will extensively deal with the historical, molecular, phenotypical, and clinical basis of why ketosis-prone diabetes is different than the traditional principles of type 1 and 2 diabetes and should be classified as such. Clinicians, especi
