[The Koblenz algorithm for thoracoabdominal wounds in a military hospital].

Despite available armored personal protection in troops, the incidence of abdominal wounds in modern wars is 6.6-9.0%. Of these, penetrating abdominal injuries comprise 75-80%. Thoracoabdominal injuries occupy a special place with incidence up to 88%. We present the first case of the "Koblenz algorithm" in the treatment of a patient with mine explosion wound, combined injury of the head, limbs, thoracoabdominal trauma, widespread peritonitis, small intestinal obstruction and septic shock in a military hospital. This algorithm was implemented under import substitution considering the peculiarit

The role of transvaginal ultrasound in a referral military hospital.

In an attempt to overcome some of the difficulties encountered with transabdominal ultrasonography we report on 55 patients who underwent 88 transvaginally directed sonograms. In 24 patients in whom a diagnosis of an early intrauterine pregnancy was made, the mean crow-rump length (CRL) was 10.7 mm (range, 3 to 27). In six patients (25%) the CRL was 5 mm or less. These data suggest that the use of transvaginally directed sonography may permit an earlier diagnosis of pregnancy than the abdominal approach does. This earlier diagnosis should allow the exclusion of ectopic pregnancy in many of the

Clinical Partnering - When Should Host Nation Patients Not be Directed to Coalition Military Hospitals?

This case commentary explores whether and when it is justifiable NOT to direct injured or ill host nation patients to international coalition military hospitals that are better equipped than host nation hospitals to meet patients' health needs in order to ensure sufficient patients for success in clinical partnering. The case discusses Medical Rules of Eligibility as a policy to determine the destination of patients who present for medical care to international military medical services. It also discusses the ethical challenges for international military healthcare workers when working as clin

Recovery of anaerobic bacteria from clinical specimens in 12 years at two military hospitals.

Examination of 15,844 clinical specimens submitted over 12 years (1973 to 1985) to the anaerobic microbiology laboratories in two military hospitals demonstrated the recovery of anaerobic bacteria in 4,458 (28.1%) specimens. The specimens yielded 6,557 anaerobic isolates (1.47 isolates per specimen). Bacteroides spp. accounted for 43% of all isolates; anaerobic gram-positive cocci, 26%; Clostridium spp., 7%; and Fusobacterium spp., 4%. Bacteroides spp. predominated in abscesses, obstetrical and gynecological (OBG) infections, abdominal infections, cysts, wounds, and tumors. Members of the Bact

[Hospital admissions for digestive diseases: 1995-2003 morbidity in a military hospital].

Digestive diseases are still currently one of the leading causes of illness. The study was conducted of those individuals admitted to hospital during the 1995-2003 period, whose main diagnosis at admission to hospital had been encoded according to the International Disease Classification as a digestive disease. The highest rate of admissions was in civilian males (the military hospital assures health care services for militaries and their families), due to duodenal ulcer and liver diseases. The superior level of education could be considered as a source of stress, and a risk factor for duodena

Hospital-acquired device-associated infections at a deployed military hospital in Iraq.

BACKGROUND: To date, there have been no published studies describing rates of device-associated infections over extended periods at deployed military hospitals. We describe the rates of utilization and device-associated infections at the Air Force Theater Hospital (AFTH) in Iraq. METHODS: This is a retrospective review of infection control (IC) records at the AFTH intensive care unit (ICU) from November 2006 through December 2007. Monthly device utilization and infection rates (per 1000 device days) were analyzed for trend and compared with pooled means for US trauma ICUs. RESULTS: Central l

The epidemiology of blast lung injury during recent military conflicts: a retrospective database review of cases presenting to deployed military hospitals, 2003-2009.

Blast injuries are becoming increasingly common in military conflicts as the nature of combat changes from conventional to asymmetrical warfare and counter-insurgency. This article describes a retrospective database review of cases from the UK joint theatre trauma registry from 2003 to 2009, containing details of over 3000 patients, mainly injured in Iraq and Afghanistan. During this period, 1678 patients were injured by explosion of whom 113 had evidence of blast lung injury. Of the 50 patients who survived to reach a medical facility, 80 per cent required ventilatory support. Injuries caused

Neonatal Intensive Care Outcomes in the Military Health System: Comparison of Military and Civilian Hospital Births.

INTRODUCTION: Military Health System (MHS)-insured newborns receive care in military and civilian hospitals. Differences in delivery location and corresponding payment schemas raise questions regarding possible health system effects on utilization and outcomes. We hypothesize that newborn utilization and clinical outcomes differ between military and civilian hospitals and that the differences may be more pronounced among lower risk newborns (i.e., late preterm and non-preterm infants). MATERIAL AND METHODS: The newborn cohort comprised live births captured in DoD Birth and Infant Health Resea

Hypertensive Disorders of Pregnancy in a Military Hospital Birth Cohort.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) are risk factors for maternal and fetal complications with long-term sequelae for mother and children. HDP are not clearly understood; however, there appears to be a relationship with maternal weight gain. The effects of maternal weight gain and pregnancy outcomes, including HDP, are understudied. Few studies have assessed maternal weight gain in service-connected women and its effects on HDP. This study aimed to evaluate the relationship between blood pressure and birth outcomes in women who delivered their infants at a military hospital.

Meningitis admitted to a military hospital: a retrospective case series.

Meningitis is a common admission diagnosis. No case series or descriptive studies on meningitis have recently been published. Additionally, no recent data exist on meningitis in the U.S. Military Health System. We reviewed charts of adult patients admitted to Naval Medical Center San Diego between January 2004 and December 2008 with an admission diagnosis of meningitis. Charts were excluded if they did not meet our case definition of meningitis, if missing data, or if meningitis was nosocomial or iatrogenic. We reviewed results of cerebrospinal fluid cultures during this period. We compared ra
