Characterizing Emergency Department Visits to Pediatric Hospitals After Local Disaster Declarations.

STUDY OBJECTIVE: Pediatric data on health care utilization following disasters are limited, with most studies focused on adults. This study evaluated changes in pediatric emergency department (ED) volumes, hospital admissions, and diagnoses following Major Disaster Declarations by the Federal Emergency Management Agency. METHODS: This was a retrospective observational analysis of patients aged ≤18 years presenting to a Pediatric Health Information System-participating ED between 2010 and 2023. We paired each Pediatric Health Information System ED with any major disaster that occurred within 5

Is there a "July phenomenon" in pediatric neurosurgery at teaching hospitals?

OBJECT: Concern for patient safety, among other reasons, recently prompted sweeping changes in resident work policies in the US. Some have speculated that the arrival of new interns and residents at teaching hospitals each July might cause an annual transient increase in poor patient outcomes and inefficient care. METHODS: Data were analyzed for 4323 craniotomies for tumor resection and 22,072 shunt operations performed in pediatric patients between 1988 and 2000 in US nonfederal hospitals (Nationwide Inpatient Sample, Healthcare Cost and Utilization Project, Agency for Healthcare Research an

Describing Pediatric Emergency Department Nurse Staffing Models and Unit Characteristics in Pediatric Hospitals in the United States.

INTRODUCTION: Hospitals across the United States experience critical nursing shortages and high turnover across all nursing roles. High nurse vacancy rates present ongoing challenges for emergency departments and may negatively affect patient care. Detrimental effects include overcrowding, longer waiting times, and delayed or missed nursing care. Amid a national nursing crisis, little is known regarding the staffing strategies employed by pediatric emergency departments. The purpose of this study was to provide a foundational description of the current staffing models and unit characteristics

Enteral Access in Pediatric Cardiac Patients: A Survey of Pediatric Surgeons at the Top 50 Cardiac Hospitals.

PURPOSE: Maintaining the nutritional status of pediatric cardiac patients is essential as these children often have higher metabolic needs and predilection for feeding difficulties. Despite the importance in preventing malnutrition in these patients, there remains a lack of consensus in management. We surveyed pediatric surgeons at top pediatric cardiac hospitals to query their practices in enteral access for infants with cardiac disease. METHODS: We utilized the US News & World Report rankings to identify the top rated 50 "Pediatric Cardiac" hospitals and surveyed pediatric surgeons at each

Risk factors for death after adult congenital heart surgery in pediatric hospitals.

BACKGROUND: Despite the central role that pediatric hospitals play in the surgical treatment of congenital heart disease, little is known about outcomes of adult congenital cardiac surgical care in pediatric hospitals. Risk factors for inpatient death, including adult congenital heart (ACH) surgery volume, are poorly described. METHODS AND RESULTS: We obtained inpatient data from 42 free-standing pediatric hospitals using the Pediatric Health Information System data base 2000 to 2008 and selected ACH surgery admissions (ages 18 to 49 years). We examined admission characteristics and hospital

Neonatal seizures: treatment and treatment variability in 31 United States pediatric hospitals.

Neonatal seizures are one of the most common neurological disorders in infants. However, the optimal treatment strategy for neonatal seizures remains controversial and there is little data regarding current treatment of neonatal seizures. In this study we describe the current treatment of neonatal seizures and variation in practice among 31 pediatric hospitals in the United States. We retrospectively identified 6099 infants hospitalized in the first month of life in one of 31 pediatric hospitals participating in the Pediatric Health Information System, with a discharge diagnosis of seizure. Examining Disparities in Pediatric Asthma, Pneumonia, and Bronchiolitis Care in General Hospitals.

BACKGROUND/OBJECTIVE: Prior disparity research has focused on evaluating patient outcomes in freestanding children's hospitals, rather than identifying potential drivers of these disparities. Our objective was to evaluate drivers of disparities in respiratory illness care quality and outcomes in a national sample of general hospitals. METHODS: This retrospective study used pretrial data from hospitals in the SIP trial (Simultaneously Implementing Pathways for Improving Asthma, Pneumonia, and Bronchiolitis Care for Hospitalized Children, NCT05206695). Patients were eligible if they had a prima

High-Risk Medicines and Technology-Related Prescribing Errors in 2 Pediatric Hospitals.

OBJECTIVE: High-risk medicine (HRM) prescribing errors in pediatrics are a safety concern. Technology-related errors (TREs) occur when the design of computerized provider order entry (CPOE) facilitates errors. We aimed to measure the proportion of HRM errors that were TREs and the HRM TRE rate at 2 pediatric hospitals, describe TRE types, and examine their underlying mechanisms. METHODS: We identified 7014 HRM orders from retrospective medical record review of 18 951 medication orders for 2192 pediatric inpatients at 2 hospitals with the same CPOE in 2020. HRM prescribing errors were classifi

Survey of nutrition screening practices in pediatric hospitals across the United States.

BACKGROUND: Nutrition screening on admission is critically important to systematically identify patients with undernutrition given the known relationship with deleterious clinical outcomes. Limited data exist regarding optimal processes and criteria for pediatric nutrition screening. Therefore, we sought to characterize nutrition screening practices in pediatric hospitals. METHODS: A total of 365 inpatient pediatric hospitals in the United States were identified, eligible, and contacted. Eligible hospitals included general pediatric hospitals, adult hospitals with pediatric units, and special

Impact of telemedicine on the practice of pediatric cardiology in community hospitals.

BACKGROUND: Tele-echocardiography has the potential to bring real-time diagnoses to neonatal facilities without in-house pediatric cardiologists. Many neonates in rural areas, smaller cities, and community hospitals do not have immediate access to pediatric sonographers or echocardiogram interpretation by pediatric cardiologists. This can result in suboptimal echocardiogram quality, delay in initiation of medical intervention, unnecessary patient transport, and increased medical expenditures. Telemedicine has been used with increased frequency to improve efficiency of pediatric cardiology care
