Controlled human malaria infection in adults identify combinations of merozoite antigens associated with clinical immunity.

Two vaccines are currently licensed against Plasmodium falciparum malaria but offer only partial protection that wanes, necessitating repeated dosing that is challenging to implement where it is needed most. We previously identified a subset of Kenyan adults with naturally acquired clinical immunity using controlled human malaria infection studies (CHMI, n = 86/142; ClinicalTrials.gov - NCT02739763). Subsequent studies showed that clinical immunity was strongly correlated with IgG Fc-mediated functional activity against whole merozoites. Here, to zoom in on specific targets associated with cli

Lung mucosal and systemic responses at single-cell resolution in an aerosolized Mycobacterium bovis BCG human challenge model.

Tuberculosis (TB) vaccine development is hindered by limited understanding of human immune responses to Mycobacterium tuberculosis. Using an aerosolized Mycobacterium bovis BCG human challenge model, we perform single-cell and bulk immune profiling of bronchoalveolar lavage and blood samples to define the temporal immune dynamics in BCG-naive individuals. Rapid changes in cellular composition and gene expression occur in both compartments, with TB-associated gene signatures evident on days 2 and 7 post-challenge. T cell receptor clones that expand in blood at day 7 persist in the lung mucosa u

Evaluation of Salmonella Paratyphi specific antibody quantity and function after vaccination and controlled human infection.

Salmonella Paratyphi A is a major cause of paratyphoid fever. Despite growing global concern, knowledge of S. Paratyphi A immunity remains limited, and no licensed vaccine exists. In a controlled human infection model (CHIM) homologous S. Paratyphi A rechallenge showed a non-significant reduction in risk of acute disease. This study evaluates humoral immunity following experimental S. Paratyphi A exposure and vaccination with a bivalent S. Typhi-S. Paratyphi A conjugate vaccine (Sii-PTCV). Serum samples from 2 UK CHIM studies and a Phase I trial in India of Sii-PTCV were analyzed. S. Paratyphi

The effect of pneumococcal conjugate vaccine and pneumococcal polysaccharide vaccine on nasopharyngeal colonisation following human infection challenge with serotype 3 and serotype 6B (PREVENTING PNEUMO 2): a double-masked, randomised, controlled, phase 4 trial.

BACKGROUND: Although evidence suggests some direct protection from the 13-valent pneumococcal conjugate vaccine (PCV13) against Streptococcus pneumoniae serotype 3 (Spn3), Spn3 remains a frequent cause of pneumococcal disease in the UK, potentially due to lower vaccine effect on colonisation, shorter duration of protection, or the emergence of more successful Spn3 clades. To test these hypotheses, we compared PCV13 and 23-valent pneumococcal polysaccharide vaccine (PPV23) protection against prevalent Spn3 clades. Additionally, we assessed the long-term protection offered by pneumococcal vaccin

Sex-based differences in Plasmodium infection in the control groups of controlled human malaria infection trials in malaria-naive populations in the USA and the Netherlands: a pooled analysis.

BACKGROUND: Before infecting red blood cells and causing the clinical manifestations of malaria, the hepatotropic parasite Plasmodium falciparum completes a complex liver stage. Sex-based differences in pathogenesis by hepatotropic micro-organisms are well documented but unstudied for P falciparum in humans. We aimed to evaluate the effect of sex on the time to blood-stage positivity and initial blood-stage parasite densities as indicators of liver-stage dynamics and parasite replication. METHODS: We conducted a pooled analysis of data from malaria-naive participants in control groups from co

Controlled human pneumococcal infection in the Netherlands: Colonisation, antibody responses and the impact of viral co-infection.

OBJECTIVES: Colonisation of the upper airways by Streptococcus pneumoniae is a prerequisite for disease development and shapes immune responses that vary between geographical settings. This first controlled human pneumococcal infection study in the Netherlands assessed colonisation, antibody responses, respiratory symptoms, and concurrent viral infections. METHODS: Twenty healthy adults (19-47 years) were enrolled between May 2022 and February 2023 and inoculated intranasally with 160,000 colony forming units of S. pneumoniae serotype 6B (strain BHN418). Participants were monitored for 28 day

Evaluation of Serum Antibodies as Correlates of Protection Against Norovirus Infection and Disease.

BACKGROUND: Norovirus is a major cause of acute gastroenteritis. Several vaccines are in development. Identifying a reliable immunological correlate of protection (CoP) would aid vaccine development and evaluation; however, no strong CoP has been established to date. METHODS: We evaluated the relationships between prechallenge genogroup I, genotype 1 (GI.1) and GII.4 norovirus antibodies and clinical outcomes in two human vaccine-challenge studies. We analyzed genotype-specific histo-blood group antigen blocking titers, serum IgA and IgG, and total antibody ELISA. We used Bayesian regression

Protective immunity against Shigella sonnei in vaccinated and challenged adult Thai volunteers.

A safe, effective, and affordable vaccine that can prevent Shigella-induced diarrhea could have a significant impact on reducing morbidity and mortality in populations at risk. WRSS1, a live attenuated Shigella sonnei vaccine candidate, was well tolerated and immunogenic in adult Thai volunteers, achieving 40% efficacy against wild-type (WT) S. sonnei. We performed an in-depth analysis of mucosal and systemic antibodies in these individuals following WRSS1 vaccination and S. sonnei challenge, including a broader analysis of antibody specificity, functional features, and associations with clini

Na-GST-1 adsorbed on Alhydrogel co-administered with different Toll-like receptor agonists in hookworm-naive adults using a controlled human infection model in the USA: a phase 2, double-blind, randomised controlled trial.

BACKGROUND: Necator americanus glutathione S-transferase-1 (Na-GST-1) performs a crucial enzymatic step in hookworm blood feeding and is a leading target for vaccine development. Phase 1 trials in Brazil, Gabon, and the USA of recombinant Na-GST-1 adsorbed on Alhydrogel (Na-GST-1/Al), co-administered with or without AP 10-701 (Toll-like receptor [TLR] 4 agonist) or CpG 10104 (TLR9 agonist), showed the vaccine was well tolerated, safe, and immunogenic. A controlled human hookworm infection (CHHI) model was previously developed to evaluate the effect of candidate hookworm vaccines on infection.

Is routine axillary lymph node dissection needed to tailor systemic treatments for breast cancer patients in the era of molecular oncology? A position paper of the Italian National Association of Breast Surgeons (ANISC).

BACKGROUND: De-escalation of axillary surgery in breast cancer (BC) management began when sentinel lymph node biopsy (SLNB) replaced axillary lymph node dissection (ALND) as standard of care in patients with node-negative BC. The second step consolidated ALND omission in selected subgroups of BC patients with up to two macrometastases and recognized BC molecular and genomic implication in predicting prognosis and planning adjuvant treatment. Outcomes from the recent RxPONDER and monarchE trials have come to challenge the previous cut-off of two SLN in order to inform decisions on systemic ther

Distinct time-dependent S. Typhi-specific CD8+ T cell programs are associated with clinical outcome following Ty21a immunization and wild-type S. Typhi challenge in humans.

INTRODUCTION: CD8+ T cells are essential components of the adaptive immune system that play a critical role in determining the clinical severity of typhoid disease by directly killing Salmonella enterica serovar Typhi (S. Typhi)-infected cells, producing cytokines, chemokines, and establishing memory responses that have been linked to clinical outcome in a controlled human infection model (CHIM) of typhoid. Despite decades of extensive research, the role of memory CD8+ T (TM) cells and its subsets as correlates of protection against S. Typhi infection and typhoid disease remains incompletely u

Discovery of a novel and highly selective JAK3 inhibitor as a potent hair growth promoter.

JAK-STAT signalling pathway inhibitors have emerged as promising therapeutic agents for the treatment of hair loss. Among different JAK isoforms, JAK3 has become an ideal target for drug discovery because it only regulates a narrow spectrum of γc cytokines. Here, we report the discovery of MJ04, a novel and highly selective 3-pyrimidinylazaindole based JAK3 inhibitor, as a potential hair growth promoter with an IC50 of 2.03 nM. During in vivo efficacy assays, topical application of MJ04 on DHT-challenged AGA and athymic nude mice resulted in early onset of hair regrowth. Furthermore, MJ04 sign

Adherence challenges with drugs for pre-exposure prophylaxis to prevent HIV infection.

BACKGROUND: There are 34 million people living with human immunodeficiency virus (HIV) worldwide and each year this number increases. Until a vaccine is discovered, the prevention of new HIV infections remains an urgent priority. Several trials studying the use of oral and topical agents for the prevention of HIV infection have already been completed. Adherence has proved to be a major challenge in achieving product efficacy. AIM OF THE REVIEW: To provide the clinical pharmacist with an understanding of the oral pre-exposure prophylaxis (PrEP) and topical microbicide product pipeline whilst e

Approaches and Challenges in SARS-CoV-2 Vaccine Development.

The explosive spread of SARS-CoV-2 suggests that a vaccine will be required to end this global pandemic. Progress in SARS-CoV-2 vaccine development to date has been faster than for any other pathogen in history. Multiple SARS-CoV-2 vaccine candidates have been evaluated in preclinical models and are currently in clinical trials. In this Perspective, we discuss three topics that are critical for SARS-CoV-2 vaccine development: antigen selection and engineering, preclinical challenge studies in non-human primate models, and immune correlates of protection.

Basic and applied immunology in cestode infections: from Hymenolepis to Taenia and Echinococcus.

In larval cestode infections, it is well established that the intermediate mammalian host infected with egg-derived metacestodes in the tissue becomes completely immune to reinfection with eggs, whereas autoinfection has been conceived to occur in Hymenolepis nana/mouse (and human) and Taenia solium/human systems when these hosts are initially infected with metacestode-derived adult tapeworms in the lumen. In this review paper, the first topic is immunobiology of H. nana/mouse system on the reinfection immunity in order to get critical information as to how the initially ingested parasite (egg

Who should be included in first-in-human trials? A systematic review of reasons.

BACKGROUND: First-in-human trials mark a significant turning point in translational research, with novel therapies being tested in humans for the first time. Who should be included in FIH trials is a topic of ongoing debate amongst researchers, clinicians, ethicists, and sponsors. Yet, no comprehensive overview of the literature on this topic has been constructed before. METHODS: A systematic review of reasons was conducted, following the methodology outlined by Stretch and Sofaer for conducting systematic reviews of argument-based literature. Six online databases were consulted across variou

Advancing precision medicine for acute respiratory distress syndrome.

Acute respiratory distress syndrome (ARDS) is a heterogeneous clinical syndrome. Understanding of the complex pathways involved in lung injury pathogenesis, resolution, and repair has grown considerably in recent decades. Nevertheless, to date, only therapies targeting ventilation-induced lung injury have consistently proven beneficial, and despite these gains, ARDS morbidity and mortality remain high. Many candidate therapies with promise in preclinical studies have been ineffective in human trials, probably at least in part due to clinical and biological heterogeneity that modifies treatment
