BACKGROUND: The purpose of this study is to present our experience treating patients in the advanced stages of osteonecrosis of the humeral head with partial resurfacing of the humeral head.
METHODS: This is a prospective series of 12 shoulders in 11 patients diagnosed with osteonecrosis of the humeral head who underwent partial humeral head resurfacing. Their mean age was 56 years. Preoperative and postoperative standardized evaluations included history, physical examination, radiographs, and clinical scoring systems, including the Western Ontario Osteoarthritis of the Shoulder index, Should

Despite the introduction of different surgical techniques and implants, osteoporotic varus displaced fractures of the proximal humerus treated with an angular stable plate have still a high risk of recurrence and/or screw penetration leading to revision surgery. Here, we describe a technique where we use an alternative approach to stabilize the humeral head by the use of apical subchondral bone anchors tightened to an angular stable plate. This technique eliminated the need for screws in the upper part of the humeral head, allowing the upper angular stable screws to be redirected to the infero

PURPOSE: Humeral head osteonecrosis (HHN) is a joint-destructive condition, for which cementless humeral head resurfacing (CHHR) offers a bone-preserving treatment option. The aim of this study was to report long-term outcomes and implant survival of CHHR in patients with HHN.
METHODS: Patients with humeral head osteonecrosis treated with cementless humeral head resurfacing (CHHR) between 2004 and 2007 were included. Implant survival was assessed according to Kaplan-Meier analysis. Clinical evaluation included Constant-Murley-Score (CMS), Simple Shoulder Test (SST), Subjective Shoulder Value

The fixation technique of bony fragments is crucial for the bone union of the tuberosities after humeral head replacement (HHR) for a comminuted fracture of the proximal humerus. To increase the bone union rate, we reduce tuberosities to overlap on the humeral shaft by approx. 1 cm and fix them with cable wire. Herein, we retrospectively investigated the clinical and radiographic outcomes of our procedure. Twenty-six patients who underwent cementless HHR for the treatment of comminuted fractures of the proximal humerus were investigated. The Constant-Murley score, active shoulder mobility, and

OBJECTIVE: The aim of this study is to determine the diagnostic accuracy of sonography in order to identify the chondral print on the humeral head.
MATERIALS AND METHODS: In total, 44 patients who had already been candidates for arthroscopic surgery were prospectively studied with ultrasound to assess the presence of humeral subchondral erosion at the level of the biceps pulley; no patient had undergone previous surgery or radiographic calcification of rotator cuff tendons. Using arthroscopy as the reference standard the sensitivity, specificity, positive predictive, negative predictive and d

BACKGROUND: Humeral head replacement has been used successfully for the treatment of osteoarthritis of the shoulder for decades. The purpose of this study was to define the results of this form of treatment, the risk factors for an unsatisfactory outcome, and the rates of failure over time.
METHODS: Between 1978 and 1997, sixty humeral head replacements were performed at our institution for the treatment of osteoarthritis. Five patients (seven shoulders) died less than five years postoperatively, and one patient (two shoulders) was lost to follow-up. Fifty-one humeral head replacements in for

Splitting fractures of the humeral head are rare; part of the humeral head dislocates and the unfractured part remains attached to the shaft. We report eight cases in young patients. In five the diagnosis was made at presentation: three had minimal internal fixation using a superior subacromial approach, one had a closed reduction and one a primary prosthetic replacement. All five patients regained excellent function with no avascular necrosis at two years. In three the injury was initially unrecognised; two developed a painless bony ankylosis and one is awaiting hemiarthroplasty. It is import

One hundred twenty-seven shoulders treated with replacement arthroplasty for osteonecrosis of the humeral head were reviewed and assessed by mailed questionnaire for results, durability, and complications. These included 71 humeral head replacements and 56 total shoulder replacements. Thirty-six shoulders were lost to follow-up because of death of the patient; an additional 3 shoulders were locking survey follow-up. Thus current follow-up was available for 88 shoulders. At an average follow-up of 8.9 years, subjective improvement was expressed for 70 (79.5%) of the shoulders and no to occasion

Humeral head translations (HHT) play a crucial role in the glenohumeral (GH) joint function. The available shoulder musculoskeletal models developed based on inverse dynamics however fall short of predicting the HHT. This study aims at developing a simulation framework that allows forward-dynamics simulation of a shoulder musculoskeletal model with a 6 degrees of freedom (DOF) GH joint. It provides a straightforward solution to the HHT prediction problem. We show that even within a forward-dynamics simulation addressing the HHT requires further information about the contact. To that end, a def

Osteonecrosis of the shoulder is a devastating disease, particularly in the young patient. Published data is replete with vascularized and nonvascularized bone grafting procedures for joint preservation of the hip; however, these same techniques have not been applied to the shoulder. When treating osteonecrosis of the shoulder, endoscopically-guided thorough debridement of the necrotic bone may promote good outcomes by ensuring that the bone graft, vascularized or nonvascularized, is stabilized against a viable host bed. In addition, this new technique facilitates access to the subchondral pla

A one-year-neutered male cat was presented for veterinary examination because of a left forlimb lameness. A painful left shoulder joint was found on examination and radiology of this joint revealed an erosive lesion of the humeral head and a detached flap of articular cartilage. The detached cartilage was surgically removed after which the lameness resolved. The lesion in this cat resembled osteochondritis dessicans of other species but whether the pathogenesis of the condition in this cat was 115 similar also was not determined.

BACKGROUND: The purpose of this study was to quantitatively evaluate the effect of the prosthetic humeral head shape on rotational range of motion and glenohumeral joint kinematics.
METHODS: Six fresh-frozen cadaveric shoulders were tested in multiple positions under anatomic muscle loading. Specimens were tested for the native head, and then the spherical and non-spherical prosthetic heads were randomly implanted in the same stem to preserve the center of rotation. Rotational range of motion was measured with 3.3 Nm of torque. Glenohumeral joint kinematics was quantified by the position vect

Two skeletally immature female dogs were each investigated for chronic weight-bearing thoracic limb lameness. The first patient was lame for 2 months following a tumble whilst playing, and the second patient had been intermittently lame since 3 weeks of age. In both cases, radiographic examination of the shoulder revealed fissuring of the caudal humeral head consistent with an incomplete proximal humeral Salter-Harris type IV fracture with an Enoki-mushroom-like appearance of the caudal fragment, where two heads rise from a common stem. There was secondary neoarthrosis of the caudal humeral he

A 74-year-old woman was incidentally found to have a left breast mass. The mass could not be adequately compressed to be visualized on mammography. Ultrasonography showed a heavily-calcified rounded mass in the left axillary tail of the left breast. Chest radiograph confirmed that the mass was a migrated humeral head. Remotely-displaced fracture-dislocations of the humeral head are very rare and to our knowledge, displacement into the breast, clinically mimicking a breast mass, has not been previously described.

Three centers' experience with 93 comminuted humeral head fractures were clinically (Constant score) and radiologically (Neer) analyzed. The results revealed fundamental differences between the various types of fractures depending on the number of fragments. For three-part fractures open reduction and internal fixation (mean Constant scores 83 and 91 points, respectively) or conservative treatment (78 points) seem to be indicated. The prognosis for four-part fractures is determined largely by the vascular supply of the head fragment with a high risk of humeral head osteonecrosis. For this reas

The complications of old fractures of the proximal humeral head and fracture dislocations are nonunion, malunion, osteonecrosis, early traumatic arthritis, chronic dislocation and instability with subluxation. We reviewed a consecutive series of 34 shoulders that were treated between 1986 and 1990. We performed 7 hemiarthroplasties and 11 total shoulder arthroplasties (Neer type) in 17 patients (group I). Group II consisted of 12 osteotomies, 3 internal fixations for nonunion (AO technique), and 1 shoulder arthrodesis. The average follow-up was 27 months (group I) and 26 months (group II), res

An intrathoracic humeral head fracture is a rare injury usually caused by a high-energy force to the shoulder, resulting in medial displacement of the humeral head. This injury can be severe and life-threatening due to the proximity of the dislocated and fractured bone to the critical structures within the intrathoracic cavity. There are no current official guidelines for the treatment of such injuries. We report the successful treatment of a patient with an intrathoracic humeral head fracture that lacerated the thoracic aorta.
